(Critical) Observed the following items in the refrigerators without use-by dates: open gallon jug of milk, coney chili, sliced tomatoes. Ensure your ready to eat, potentially hazardous foods are provided with a label indicating the last day of use (seven days maximum). This is calculated by adding six days to the day of prep/opening (i. E. If tomatoes are sliced or milk is opened on 5-2, then mark the container with 5-8)

The bar sink faucet is in need of cleaning now and more frequently in the future to eliminate the slime growth

Observed both of the dial type air thermometers in the kitchen refrigerator to be inaccurate. Provide replacement thermometers

Observed mouse feces in the se base cabinet in the kitchen. Ensure staff are regularly checking for evidence of pests in the facility and taking necessary action to eliminate any issues. Please take the time in the near future to remove all items from the kitchen cabinets to thoroughly clean them.

Observed the spout for the bar soda dispenser and the interior of the ice making machine to be in need of more frequent cleaning (wash, rinse and sanitize)

Observed plastic storage bags being saved for re-use (zip-lock bags). Discontinue this practice and either only use these bags one time or purchase and utilize multi-use rigid plastic storage containers

(Critical) Observed three bottles of liquor (blackberry brandy, mango parrot bay rum, canadian club whisky) with pour spouts installed to have small flies in the bottoms of them. Corrected by discarding this liquor. Consider purchasing some liquor pour spout caps to prevent access to the liquor by flies (to be kept on the bottles all the time except when actively pouring liquor

Observed the tied up drain lines for the ice machine and ice bins at the bar to have slipped down. Re-establish the drain line air gaps at their discharge location

Observed the interior of the microwave oven to be overdue for cleaning. Please clean this unit now and more frequently in the future

Observed the interior of the ice machine to be in need of more frequent cleaning. Please wash, rinse and sanitize now (per the manufacturer's directions--some disassembly required) and on a periodic basis in the future to maintain clean

(Critical) Observed a spoiled head of lettuce in upright cooler in kitchen. Also observed an open container of rte parm. Cheese in same upright cooler with a use by date of 3/19/09. All food in facility must be safe and unadulterated at all times. Employees must be monitoring food quality more frequently, rotating stock and must discard all food that has expired use by dates or is visually contaminated. (Critical) Observed several soiled pots, pans and utensils from yesterday stored in empty 3 compt sink and on floor in dishwashing area. The dishwashing frequency in the facility needs to be changed immediately. All soiled pots, pans and utensils must be washed, rinsed and sanitized after use and every night before leaving. Do not leave soiled dishes in kitchen overnight (soiled dishes may attract unwanted pests and rodents). Observed the top of prep table in kitchen soiled with debris. Also observed interior of several cabinets in kitchen with an accumulation of debris. Interior of display cooler in bar area soiled with a sticky residue. To correct, please clean now and on a more frequent schedule.

Observed plate of deep fried chicken wings and a cooked egg roll left on counter from last night. All food must be stored properly or discarded if not used. Do not leave exposed food in kitchen overnight as this attracts unwanted insects and rodents
