(Critical) On follow up inspection found the dishwashing machine sanitizing dishes and quat dispenser dispensing at proper concentration. Pic states he has logsheets for staff to fill out, and request staff use test kits to check dishwashing machine and quat dispenser at least once a day. Violation corrected.

(Critical) On follow up inspection used a chlorine test kit and found a new dishwashing machine chlorine sanitizing dishes. Pic states a new dishwashing machine was installed and properly working. Violation corrected. On follow up inspection use a quat test kit and found the sanitizer flowing from the quat dispenser at 200 ppm, manufacturer's recommended range. Pic states dispenser was adjusted to dispense at proper concentration. Violation corrected.

(Critical) Used a chlorine test kit and found the dishwashing machine not chlorine sanitizing dishes. Used a quat test kit and found the sanitizer flowing from the quat dispenser in the 3 compartment sink below manufacturer's specification of 200 ppm. To correct call for repair, and manually wash, rinse, and sanitize dishes, and use test kits to verify proper concentration. ,

(Critical) Found the dishwashing machine not chlorine sanitizing dishes and quat dispenser in the 3 compartment sink dispensing below manufacturer's specification and staff not using available test kits to verify proper sanitizer concentrations. To correct train staff to use test kits to verify proper concentration for the dishwashing machine and quat sanitizing solutions. ,

Found the edges and bottom of drawer on the prep table soiled with accumulation. Found the areas surrounding spouts of the juice dispenser in the waitstation soiled with splash and build up. Found the bottom of cooler #2 soiled with spills and accumulation. Found the hood filters on the cooking hood soiled with dust and grease build up. To correct clean all noted areas and clean on a more frequent schedule.

Found tape wrapped around sink stopper handle and 3 compartment sink post, to keep stopper in place. To correct repair sink stopper to work properly.

Found employee personal drinks in freezer and cooler. To correct designate employee storage areas and store employee food in designated areas only.

Found the floor under the cooking and prep line soiled with accumulation of debris and build up. Found the floor under equipment, edges and corners in the dishwashing room and storage room soiled with accumulation of debris. To correct thoroughly clean floor and clean on a more frequent schedule.

(Critical) Found a y type shutoff valve installed downstream of the atmospheric vacuum breaker on the mop sink faucet. To correct either remove shutoff valve or install a hose bibb vacuum breaker downstream of the y type shutoff valve. Found the hose bibb on the outside wall by the back door area without back flow prevention device. To correct install a hose bibb vacuum breaker on hose bibb. ,

(Critical) Found the drain lines of the ice machine laying on top of the floor drain cover without air gap. Found the drain line of the ice cream dipper well draining into a funnel like device which was laying on it's side leading to the floor drain without air gap. To correct provide an air gap of at least 1 inch from the drain lines and the rim of the floor drain. ,

Observed employee with very little hair working in the cooking/prep area. To correct provide hat/hair restraint and remind employee (even with little or no hair) that food employees shall wear hair restraints.

Found an employees hat and glasses stored on top of a stack of doilies on the storage room shelf. To correct store employee personal clothing in employee lockers. Note: pic discarded doilies.

Found no lids on the dumpster in the dumpster enclosure. To correct replace missing lids on dumpster.

Observed the presence of flies in the dishwashing and storage areas. Observed the presence of dead ants under shelving/racks in storage room. To correct keep facility clean and dry, in good repair and continue having pest control company target these pest.

Found the floor in the cooking and prep area soiled with accumulation of debris and build up. Found the floor under equipment, edges and corners in the entire kitchen and storage areas soiled with accumulation. To correct thoroughly clean floor and clean on a more frequent schedule.

Found several covered carafes with water on bottom in the clean utensil storage shelf. To correct allow carafes to drain and air dry prior to covering and storing.

Found the dishwashing hood soiled with dusty build up. Found the counter top under the cookie display case soiled with accumulation of debris. Found the door gaskets and surface sealing areas of cooler doors soiled with build up and staining. Found the edge of the ice cream dipper well soiled with splash and build up. To correct clean all noted areas and clean on a more frequent schedule.

Found the pitcher of blender with chips and cracks and the lid melted and distorted. To correct replace blender with a commercial grade blender.

(Critical) Found dented cans on ready to use shelf. Pic corrected during inspection by discarding affected cans.

(Critical) Observed the blade on the food slicer with an accumulation of debris. Pic corrected during inspection by having employee disassemble and wash, rinse, and sanitize the slicer. Since this violation was found during the last routine inspection a risk control plan will need to be developed and submitted to our office for review and approval by october 17, 2011.

Observed drawer containing multiple clean food handling utensils stored in a jumbled manner. Correct by organizing the utensils so all handles face the same direction and only the handles are touched by employees

Observed catch tray under range with an accumulation of debris. Also, observed underside of shelf above hot holding area to have an accumulation of grease and dust. Pic corrected during the inspection by thoroughly cleaning the tray and underside of the shelf.

(Critical) Found the underside of the blade and blade plate soiled with accumulation of debris. Pic corrected by having employee break down slicer, and clean and sanitize slicer and all food contact areas. Pic has been instructed to monitor that employees are properly breaking down slicer and cleaning slicer. ,

(Critical) Found a drain line of the ice machine with out air gap, that is not at least 1 inch above the rim of the floor drain. To correct provide an air gap of at least 1 inch from the tip of the drain line and the rim of the floor drain. ,

(Critical) Found time/temperature control for safety (tcs) food, wrapped cut and skinned honey dew and cantaloupe 59°F in the bottom portion of the 2 door prep top cooler. Pic corrected by discarding honey dew and cantaloupe, and has been instructed not to use this cooler until repaired/replaced to hold tcs food at 41°F. Note: this is not a repeated cold hold violation, as this is a different situation and cooler. ,

Found the floor (under equipment, edges and corners) on the cooking line and prep area soiled with accumulation. Found the ceiling vents in the kitchen soiled with dusty build up. To correct clean floor and ceiling vents, and clean on a more frequent schedule.

Found the bulk tube of a bag of milk in the cold holding milk dispenser cut longer than 1 inch from the chilled dispensing head, or not cut to manufacturer's directions. To correct cut bulk tube of milk to manufacturer's directions not to exceed 1 inch from the chilled dispensing head.

Observed employee wearing bracelets while preparing food in the cooking/prep area. To correct remove bracelets prior to working with food.

Found food (milk and other food items) stored on the floor under shelves in the walk in cooler. Found a case of potaotes stored on the floor of the storage room. To correct store food on shelves at least 6 inches off the floor.

Found 2 spray bottles of seasoning without label on the shelf above the prep sink. To correct provide labels.

Observed flies in multiple areas of kitchen (drain area of prep sink, and storage rooms). To correct keep areas clean and dry, in good repair, and continue fly treatments.

Found the 2 door prep top cooler in the cooking/prep area not hold tcs food at 41°F. To correct repair/replace cooler to hold tcs food at 41°F. Found several hood filters above the oven not facing correct placement direction. To correct provide and install hood filters that are designed for your application.

Pic corrected by discarding tcs food, and has been instructed not to store tcs food in the walk in cooler, until repaired/replaced to hold tcs food at 41°F. Found garbage can in front of prep area handsink, and a food container in the cooking area handsink. To correct keep all handsinks easily accessible.
