(Critical) Found time/temperature control for safety (tcs) food, cream cups 48°F to 49°F in a covered container in the top portion of the right 2 door prep top cooler. Pic states cream may have been placed back into cooler after being left out, as other items in cooler were at 41°F. Pic corrected by discarding cream cups and has been reminded not to leave cream cups out on counter top or place unused cream cups back into cream storage container. Found tcs food, a container of hash browns 53°F to 58°F in the left 2 door sectional reach in cooler/freezer, and found a container of home fries 53°F in the right 2 door sectional reach in cooler/freezer. Pic states these items may have been left out on counter top and placed back into cooler, as other items were at 41°F in these cooler. Pic corrected by discarding hash browns and home fries and has been reminded to keep items at 41°F in coolers. This is a repeated cold violation and a risk control plan has been issued.
Found the pop nozzles and diffussers in the right waitstation pop machine soiled with slight accumulation. To correct follow manufacturers directions and clean pop nozzles and diffussers.
Found the upper spray nozzles of the dishwashing machine clogged with plastic strips. Pic corrected by removing plastic strips and has been instructed to monitor and ensure dishwashing machine spray nozzles are cleaned as needed.
Found the insulated cold hold beverage dispenser used to hold cream run through the dishwashing machine without disassembling nozzle and with cream running back in to dispenser from the nozzle. To correct follow manufacturer's directions and disassemble beverage dispenser nozzle and then properly clean all food contact surfaces.

Pic corrected by discarding raw shell eggs and states eggs should be stored in cooler. Pic has been instructed to monitor and ensure eggs are stored in cooler at 41°F or lower.
Found the painted edges of the cooking hood with peeling paint. To correct repair edges to be smooth and cleanable. Found excess water pooling on the bottom of the right 1 door prep top cooler. To correct determine water source and repair as needed. Found the 2 door salad prep top cooler in the kitchen not in use or working. To correct repair cooler to function properly and hold tcs food at 41°F or lower.
Found the floor under equipment, edges and corners on the cooking line soiled with accumulation and build up. To correct thoroughly clean floor and keep clean.
(Critical) Observed cook working on cooking line wearing gloves handle raw hamburger, bacon and then handle and place lettuce on plate. Pic corrected by having cook discard entire order, change gloves and remake order. Pic has been instructed to monitor that employees are changing gloves when task changes, that is when handling raw food items, gloves must be change when ready to eat foods are to be handled.
Found the shelves and counters under cooking equipment on the cooking line soiled with heavy accumulation and build up. To correct thoroughly clean shelves and counters and clean on a schedule to preclude accumulation. Found the interior of microwaves through out facility soiled. To correct clean at least daily, and clean as needed.
Found portioned bags of cooked pasta cooling stacked in a pan. Pic corrected by placing portioned bags of pasta in a shallow pan in the walk in cooler.

Found the exterior thermometer of the upper hot holding unit not working properly. To correct either repair/replace thermometer or provide an internal hanging thermometer.
Found the tiled floor in various areas of the kitchen with worn tile grout. To correct repair/replace tile grout to be durable, smooth, and easy to clean.

Pic corrected by having employee discard eggs, refilled container with ice, and place a fresh container of liquid egg in the ice bath in the top portion well of the central cooler. Pic has been instructed to monitor that liquid eggs are held at 41°F or lower, and employees are monitoring and filling ice as it melts. Found hot tcs food, a container of baked potatoes 105°F, and sweet potatoes 97°F, in the right drawer hot holding unit. Pic corrected by discarding potatoes and states hot holding unit was not turned on. Pic has been instructed to monitor that hot tcs food is held at 135°F or above and employees are turning on equipment. ,

Found the drain line of the walk in cooler lacking cover/insulation. To correct provide and install insulation/covering on the drain line leading to the wall of the walk in cooler.
Found the main entrance area lacking no smoking sign. To correct provide and post no smoking sign.

Found a dented can of tomatoes on the bottom of the can rack not clearly identified as dented can, return to supplier for credit. Pic corrected by clearly identifying can as return to supplier for credit.
Found no handwashing sign in the mens public restroom. To correct provide sign reminding employees to wash their hands at all locations where they may wash their hands.
Found the left cooking hood missing a filter/spacer. To correct replace missing filter/spacer.

Found the shelves of clean utensil storage shelves and the bottom of storage utensil tubs on the storage shelf soiled with accumulation. Found the fan covers of the evaporator coils of coolers soiled with dusty accumulation. To correct clean as soon as possible, and clean on a more frequent schedule.
Observed the presence of flies in the mop sink area. To correct keep areas clean and dry, in good repair, and continue fly control treatments.
Found the ceiling in the back kitchen prep area peeling and patchy. To correct repair and finish ceiling to be smooth and easy to clean.
Observed server handle cups and mugs with hands on the upper part of cups (lip contact surfaces). To correct remind and re-train servers to handle cups and mugs on mid to lower parts of cups and use handles on mugs.
