(Critical) On second follow up inspection found the bar, east ice bin with it's own drain line, the bar pop gun holder with it's own drain line, and both drain lines air gapped at least 1 inch above the floor drain. Pic states pop machine company provided the pop gun holder with it's own drain line and capped tee of the ice bin drain line. Violation corrected. Note: administrative conference notice for failure to comply with plumbing violation has been withdrawn.
(Critical) On follow up inspection found tcs food, a covered container of facility made pizza sauce 60°F cooling covered in the walk in cooler. Pic corrected by having employee pour pizza sauce in 2 shallow pans and placed in top shelf in walk in cooler for rapid cooling. This violation has corrected on routine inspection, and had to be corrected on follow up. On follow up inspection, found no cooked vegetables cooling covered in the walk in cooler. This violation was corrected on routine inspection, and reverified on follow up.
(Critical) On follow up inspection found the ice bin drain line of the bar east ice bin still draining into a shared drain line, that is the pop gun holder drains into the same drain line that the ice bin drains into. Pic has requested additional time to correct violation but has not completed plumbing task. An administrative notice for failure to comply with plumbing violation has been issued. ,
(Critical) On follow up inspection found no tcs food, tomato sauce in a crockpot on the table by the bar. Pic states crockpot has been removed, and they hold tomato sauce in a pan, in reach in cooler when customers request tomato sauce. I did not observe any tomato sauce in reach cooler at time of inspection. Violation corrected at this time.
(Critical) Found time/temperature control for safety (tcs) food, a covered 5 gallon container of facility made pizza sauce 60°F cooling in the walk in cooler. Pic corrected by pouring pizza sauce in shallow pans and placed uncovered on walk in cooler shelves. Found tcs food, numerous covered pans of cooked onions 50°F in the walk in cooler. Pic corrected by removing most of the plastic covering on pans allowing cooked onions to rapidly cool. Pic has been instructed to monitor and ensure tcs food is cooled properly.

(Critical) Found time/temperature control for safety (tcs) food, tomato sauce 119°F in a crock pot on the prep table by the bar. Pic corrected by discarding tomato sauce, and has been instructed to keep tomato sauce at 135°F and above.
Found the floor, under equipment, edges and corners in the kitchen , waitstation, and bar soiled with accumulation of debris. Found the wall in the dishwashing area soiled with moldy build up. Found the ceiling air vent covers and surrounding areas soiled with dusty build up. To correct thoroughly clean floor, walls and ceilings and clean on a schedule to preclude accumulation.
Found the cutting board on the prep top coolers with deep grooves. To correct either resurface cutting boards or replace cutting boards.
Found salad bowls made of "bamboo type material", which are not smooth, durable and easily cleanable. To correct replace salad bowls with commercial grade bowls that are durable, smooth and cleanable. Found facility using a household crock pot to hot hold tomato sauce. To correct replace household crock pot with a commercial grade hot holding unit.
Found opened and unopened bags of coal for pizza oven on the floor by the pizza oven. Found unfolded pizza boxes stored on the floor in the waitstation. To correct store coal and unfolded pizza boxes on shelf at least 6 inches off the floor.
Found the holder on the slicer taped with duct tape. Found the tile on the exterior panels of the pizza oven missing and falling off panels. To correct repair/replace slicer and exterior of pizza oven.
Found no handwashing sign in the bar handsink area. To correct provide sign reminding employees to wash hands.
Found the hood filters and hood soiled with dusty build up. Found the door gaskets, interior surfaces and exterior surfaces of coolers soiled with build up. To correct clean hood filters, hood and coolers and clean on a more frequent schedule.
Found 2 semi-frozen packages of sausage thawing in a container of standing water in the 3 compartment sink. Pic corrected by turning faucet on to provide running water to thaw sausage. Pic has been instructed to monitor and ensure employees are thawing frozen food under running water.
Observed the filters for the pizza oven with a heavy accumulation of dust and grease. Correct by cleaning now and on a more frequent basis to prevent the accumulation of grease
At the pizza oven observed several missing tiles. The tiles are over the opening to the oven. Pic indicates the remaining tiles are unstable and will be replaced in the near future with stainless steel.
On the main make line at each of the prep top coolers observed cutting boards with deep gouges. Correct by replacing the boards or having them resurfaced
(Critical) Observed small flying insects at the following three locations within the facility: 1. Around the empty beer bottles held for deposit refund. The greatest concentration of flying insects was found around the bottles. 2. Around and behind the dough mixer 3. Near the ice machine and dough tray storage area. Correct by eliminating the pests from the facility. Remove the pest harborage; empty beer bottles and wet cardboard boxes. Also, a professional pest control agency should be called.
(Critical) At the in use mechanical dishwashing machine observed the residual chlorine concentration of the final rinse at 0 ppm. Educated staff about the importance of using test strips to ensure proper sanitizer concentration. Item corrected.
(Critical) Observed the food slicer with an accumulation of food debris in the area between the blade guard and the blade. Correct by cleaning now and on a more frequent basis to prevent the accumulation of food debris. The slicer should be disassembled, cleaned, and sanitized after every use before storage.
(Critical) At the in use mechanical dishwashing machine observed the residual chlorine concentration of the final rinse at 0 ppm. The sanitizer reservoir is full and the tubing is properly routed. The unit was put out of service. A maintenance contractor was called to repair the machine. ***until the unit is repaired, all dishwashing must occur using the three compartment sink by properly washing, rinsing, and sanitizing*** correct by repairing the machine with in 10 days. A followup inspection will be conducted.
Observed the floors in the following area with an accumulation of food debris: 1. In the walk in cooler 2. Under the dough mixer 3. Under the empty beer bottle storage 4. At the bar, on the end nearest the martini glass storage 5. Under the pizza landing table near the pizza oven correct by cleaning now and on a more frequent basis to prevent the accumulation of food debris
(Critical) Observed no thermometer with a small diameter probe suitable for thin masses such as chicken breasts or sausage. Correct by obtaining an approved thermometer. ,
At hand sink in rear prep area observed drain pipe leaking on the floor. Correct by repairing or replacing pipe to prevent leak
