(Critical) Observed no date marking on ready to eat, potentially hazardous foods (such as mashed potatoes, cooked greens, deli ham, sliced deli turkey, etc. ). Please provide proper date marks on your rte, phfs

(Critical) Observed a homemade bbq grill made out of two old metal chemical barrels. Remove this grill and provide a commercially manufactured grill that is made of virgin food safe materials

(Critical) 1) a spray bottle of chlorine sanitizer was too old and weak--no chlorine concentration was registering on the test papers. 2) no chlorine sanitizer was available in the restaurant. Both items above were corrected by purchasing chlorox bleach and by providing 50-150 ppm chlorine in the spray bottle and the three compartment sink

Observed single service plastic food containers to be saved, washed and held for re-use. These containers are for one time use only. Please dispose of them after use. Consider using sandwich bags for individual portions of food, and multi-use plastic commercial food containers for your prep cooler

Within three months provide a food safety certified manager that works full time at this facility (at least 30 hours per week)

Observed the small diameter floor drain for the walk in cooler to be plugged. Please clean out this drain so it accepts the condensate from the walk in cooler

Upon arrival the front door was propped open (this facility does not have air conditioning). Exterior solid doors must be kept closed unless a screen door that properly seals in installed with a door closer

1) observed household type rolls of paper towel at both kitchen sinks to be resting on counters/edge of sink. Please either purchase a paper towel holder for this style of towel, or purchase commercial paper towels that fit in the provided towel dispensers. 2) observed to-go containers to be stored upright on a shelf above a prep table. Corrected by inverting these containers

One of the two walk in cooler evaporator fan motors is missing. Replace this motor asap so the unit works properly--currently some ice has built up on this side of the coil

Observed the wet mop head to be stored in the mop wringer. Please hang up the mop so it is hung above the mop sink to air dry

(Critical) Observed an orange force cleaner/degreaser to be stored in a spray bottle labeled as a quaternary ammonia sanitizer. Corrected by crossing out the sanitizer label and by labeling the bottle with the correct chemical name

(Critical) Observed pizza by the slice in a pizza box on top of the pizza oven to be at 91f. This pizza was discarded. Pizza by the slice may not be offered until your operation has the equipment to maintain it at 135f and above. Obtain a pizza merchandiser with temperature and humidity controls to maintain the pizza at the proper temperature and keep it from drying out

(Critical) Only one prep cooler, and therefore one supply of food is provided to prepare pizzas, subs and salads. The supply of foods that are dispensed from the prep cooler with clean, bare hands for baked items (such as pizza and hot subs) may not also be used for topping subs (after they are baked) or for preparing cold salads. Either provide a separate cooler with your ready to eat foods for sub toppings & salad preparation (lettuce, diced tomatoes, olives, chicken, feta, etc. ) or provide a separate supply of small food containers with your ready to eat foods in the reach in portion of the pizza prep cooler. This way staff only need to use single use gloves with the separate supply of rte foods

(Critical) Observed a spray bottle of chlorine sanitizer that contained no chlorine. Corrected by adding chlorine bleach to provide +/- 100 ppm chlorine

(Critical) Observed a small tub of feta cheese (can be used for greek salads) in the prep cooler with a date of opening of 10-20. The last useable date for this cheese on a ready to eat food would have been 11-26. Corrected by discarding. Suggest staff mark your rte, phfs with an opening date and a last date of use (add six days to the day of opening/prep) to keep track of the food's shelf life

Please keep your dumpster lids closed to keep out pests. Corrected by closing the lid

The exhaust hood filters for the conveyor pizza oven are due for cleaning to remove oil and dust accumulation. Please clean these filters now and more frequently in the future

Observed two of the three pizza prep cooler door gaskets to be torn. Please install replacement door gaskets to provide a smooth and easily cleanable surface that doesn't trap food debris

Observed the walk-in cooler to be operating at a slightly elevated temperature. One of the two walk-in cooler evaporator coil fans is inoperable. Repair/replace the fan to maintain foods at 41f and below

No paper towel was provided in the dispensers at the front or back prep area hand washing stations. Corrected by an employee purchasing paper towel at a store for this shop. Please ensure your facility has plenty of soap and paper towel for your hand washing stations

(Critical) Observed an open tub of feta cheese, and an open package of ham without use-by dates. Begin date marking your rte, phfs once the original packaging is opened.

(Critical) Two concerns: 1) observed bare hand contact with green peppers during the preparation process. Some of the green pepper is used as a rte food on salads. Corrected by utilizing single use gloves. 2) observed only one supply of toppings for all foods made out of the three door prep cooler (foods that will be cooked and ready to eat foods). Gloves are not utilized for dispensing all of these items. As a means of correction, provide a separate small supply of toppings for your rte foods (salad items, and for subs after being cooked) that you keep in the reach in portion of your prep cooler. Staff will only need to use gloves for dispensing these rte items. Staff with clean bare hands may dispense the other toppings for pizzas and hot subs. Gloves need to be utilized during prep for all rte foods and for dual use foods (items that can be used for either rte foods or for items that will be cooked).

Observed an "amp" energy drink cooler (counter top model) in use on the floor with only short legs. Provide either 6" legs, casters or place this unit on a counter top or a dunnage rack with 6" high legs minimum

Observed the door gaskets on the three door cooler to be torn. Please replace these gaskets to be smooth and easily cleanable, and so they do not trap food debris

1) provide a grate over the open duct in the dry storage room. 2) finish the tile/grout work in the dry store room floor. 3) install wall coving to cover the bare wood at the base of the wall in the dry store room
