You need to have a licensed pest company out to assess your facility for entry points and to get rid of the present population. I want you to save me copies of the pest company work orders for my review when i come back in 3 weeks to verify you do not have any flies in your store. (see other violations for entry points that need to be eliminated).

The spray rinse arm is extending below the top rim of the sink due to coming loose from wall clamp. Reclamp so that the spray rinse arm hangs above the top rim of the sink to minimize potential for backsiphonage into the water supply. ,

Gaskets on the prep unit doors are torn. Ocne torn they are no longer cleanable. There is excess build up inside the gasket. Replace split gaskets and then clean routinely

The pizza oven rollers need cleaning. Clean now and place on a more frequent cleaning schedule.

The condensate line in the beer cooler is plumbed to a pickle jar. This is prohibited as the condensate waste may not terminate inside the unit and must be plumbed to the sewer system. Via an airgap. Install a pump so that the condensate can get to the sewer system if gravity feed is not possible.

Togo pizza boxes found on the floor in the pizza prep/storage area. Keep all boxes and other togo containers stored a minimum of 6" above the floor to allow access for routine cleanings.

The following non food contact surfaces need cleaning: the hood filters, the tops of cans in storage area, and the gaskets on the doors of the prep unit opposite the cooking equipment,. Clean now and place on a more frequent cleanng schedule.

The following non food contact surfaces need cleaning: the hood filters, the interior of the prep unit, the interior of the upright freezer, the interior of the dough cooler, the outside of the dough mixer and dough maker, the non food contact surfaces of the ice machine, the fryer cabinet handles, the condensate insulation inside the walk in cooler has flour dust build up, the interior of the beer cooler has beer overflow on the bottom of the unit. Clean now and place on a more frequent cleaning schedule.

The gaskets on the prep unit doors are split. Once torn they are no longer cleanable. Replace split gaskets.

The concensate waste line is plumbed to a pickle jar in the beer cooler. This is prohibited as the condensate waste may not termiante inside the unit and must be plumbed to the serw system. Plumb to the sewer system via an airgap. A pump may be required if gravity flow cannot be maintained. At time of inspection the pickle jar was full and had overflowed into the bottom of the unit.

(Critical) The ice machine has build up on the splash guard and on the inside of the door. Clean now and place on a more frequent cleaning schedule.

(Critical) The sub sauce container was not clean after washing in the jackson dishwasher. The container is not being properly pre rinsed with high pressured water. The container had food debris build up inside that will most likely not be able to be removed as the neck on the container does not allow for a proper manual dishwash. Replace with an approved food grade container that can be properly cleaned.

(Critical) The jackson glasswasher is not operating properly. The utensils are not being properly sanitized by the hot water which is not reaching 160 degrees f at the plate surface. The heat tape did not turn indicating 160 degrees f. Although gauge indicates that the water will heat up to 180 degrees f, during the rinse cycle the water gradually drops 20 degrees f as the water is used. This may be an indication that the pressure reducing valve is not working properly and may need to be replace. This is further evidenced by excessive steam in the glass washer when cycle is finished. Correct by repair/cleaning in order to have utensil surfaces be properly sanitized with a minimum of 160 degrees f water.

The following non food contact surfaces need cleaning - the sides of the pizza oven, fryer cabinets, interio of the 3 door beverage air cooler, the interior of the dough cooler and the gaskets on the dough cooler doors. Also the tops of cans on the can storage rack in the dough prep area. Clean now and place on a more frequent cleaning schedule.

The newly installed domestic frigidaire freezer is not approved for use in a food service establishment. This unit is to be removed from the facility and be replaced with a commercial nsf freezer.

All of the food in the upright two door freezer in the cooler was thawd out. Air temperature was at 30 degrees. Pic had found the freezer not functioning in the am and food had thawed. Temped food at ph foods at 35 degrees f. Repair freezer to hold potentially hazardous foods frozen solid.

The bulk flour container is not labelled as to it's identification. Properly label bulk food containers to identify the product.

The upright two door freezer was not working properly in the am and was reset by the pic. Food had thawed. Ph foods temped at 35 degress f. Ambient air of 30 degrees f. Repair/adjust so that ph foods are held frozen solid.

Aluminum foil trays are being reused for storage of spices. Discontinue this practice as the aluminum tray is a single use article and is not considered cleanable for reuse. You may substitute with an approve food grade tub with lid that is cleanable.
