(Critical) On follow up inspection found a t-valve/sidekick on the faucet, and a y type shutoff valve on t-valve/sidekick, and both water supply line to chemical dispenser, and water supply line to hose connected to y type shutoff valve. This is an acceptable setup. Violation corrected. Backflow risk control plan has been reviewed and should work if followed.

(Critical) On follow up inspection used a quat test kit and found sanitizing solution flowing from quat dispenser at about 150 ppm, between manufacturer's recommended range of 150 to 400 ppm. Pic states quat dispenser was repaired and adjusted. Violation corrected.

(Critical) On follow up inspection found all paper towel dispenser stocked in all kitchen handsinks. This violation was corrected on routine inspection, and reverified on follow up.

Health Report - 04/10/2015 (3 critical incident(s))

(Critical) Found a y type shutoff valve installed downstream of the mop sink faucet with an atmospheric vacuum breaker on the faucet. To correct install a t-valve/side kick on the faucet, then install y type shut off valve on t-valve/side kick, and connect water supply line to chemical dispenser and hose to y type shutoff valve. This is a repeated violation and a risk control plan must be submitted. Note: water must be turn on when in use, and must be shut off when task completed. ,

(Critical) Found no paper towels in the paper towel dispenser in the waitstation. Pic corrected by restocking paper towels in paper towel dispenser and was reminded to train staff to either fill paper towel dispenser or notify appropriate staff member to fill to fill paper towel dispenser. ,

(Critical) Used a quat test kit and found the sanitizing solution flowing from quat dispenser and in sanitzing buckets through out facility below manufacturer's recommended range of 150 to 400 ppm. To correct call for repair and manually mix sanitizer until dispenser is repaired. Pic states sanitizer was checked this morning. ,

Observed the presence of small flies in the bar counter top areas. To correct keep facility clean and dry, in good repair, and continue fly control measures.

Found the floor under the ice machine and bar counter soiled with accumulation of debris. To correct thoroughly clean floor and clean on a more frequent schedule.

Found several paper towel dispensers in various handsinks with paper towel, but paper towel not dispensing when turned or pulled, and required extra effort to dispense. To correct repair paper towel dispenser to dispense as designed.

Found light accumulation on the spout of the ice tea urn. To correct follow manufacturer's directions and clean ice tea urn and spout.

Found the light above the dishwashing area not working. To correct repair/replace light.

Found a gap along the side and bottom of the back kitchen door. To correct repair door sweep and door seals to eliminate gap.

Found the interior surfaces of microwave oven on the cooking line soiled with accumulation. Found the rack in the cheese melter on the cooking line soiled with heavy build up. To correct follow manufacturer's directions and clean microwave and cheese melter and clean as needed.

Found numerous stacked pans stored wet on the clean utensil shelf. To correct allow clean pans to air dry prior to stacking and storing.

(Critical) Found a y type shutoff valve installed downstream of the mop sink with an atmospheric vacuum breaker on the faucet. To correct either remove y type shutoff, as shutoff valves are not allowed downstream of an atmospheric vacuum breaker or install a hose bibb vacuum breaker downstream of shutoff valve. ,

(Critical) Used a chlorine test kit and found the dishwashing machine not chlorine sanitizing dishes. Pic has called for repair, and has been instructed to use the 3 compartment sink to wash, rinse and sanitize dishes. ,

Found the hood filters, edges and some surfaces of hoods soiled with grease and dust. Found some door gaskets of cooler soiled with accumulation, and the bottom of some coolers with pooling water. Found the exterior surfaces and and edges of ovens soiled with accumulation. To correct clean hood filters, hood surfaces, door gaskets, coolers and ovens

Found the paper towel dispenser in the kitchen waitstation broken and not working, but paper towels available. Found the left ice machine in the kitchen not working. Found the left fan in the evaporator coil of the beer walk in cooler not working. To correct repair paper towel dispenser, ice machine and walk in cooler fan.

Found slight accumulation in the pop gun nozzle in the center of the bar. To correct follow manufacturer's directions and clean pop nozzle.

Found the handle of ice scoops in several ice bins in facility stored on top of ice. To correct store ice scoops with handles up or in a container/holder. Found the pop guns in the waitstation by the bar laying on top of ice in the ice bin. To correct securely place pop guns in holder.

Found the drain line of the ice bin in the bar missing and water dripping on the floor. To correct repair/replace drain line and ensure the drain line is at least 1 inch above the rim of the floor drain.

Found 1 light in the cooking hood not working. Found the lights in the walk in freezer blinking and dim. To correct repair/replace lights in hood and in the walk in freezer

Some single-service items in storage room opened from wrong end and coffee filters found without cover. Please open single-service items from bottom to prevent contact with food-contact or lip-contact end and store coffee filters in original packaging or covered container

Ice cream uncovered in ice cream chest freezer. Please cover when not in use to prevent door condensation from dripping back into ice cream

Spoons stored improperly on server's side. Please store inverted with handle up to prevent potential contamination of food-contact end

In-use utensils at bar stored in water. Please store dry in a clean, protected location with handles up and clean and sanitize as required throughout the day. The bar also has potential for a dipper well for utensil storage; if you would like to use the dipper well please extend the water inlet over the utensil container with a proper air gap and provide water at sufficient velocity to flush particulates to the drain. The drain line would also need to be air gapped, currently an air break. The air gap would need to be twice the pipe diameter above the flood rim

(1) drain line for steam table on cook line not centered over drain. Please adjust so water empties into drain properly. (2) handwash sink in dishwash area draining slowly. Please repair and maintain in good repair

(1) no paper towel dispenser in men's public restroom. Please provide paper towels in a paper towel dispenser that dispenses easily. Per pic paper towel dispenser broke this morning. (2) outside storage sheds in poor repair. Please replace to prevent potential attraction and harborage conditions for insects and rodents

Some clean inset pans stacked wet on clean dish storage racks. Please allow clean dishes and utensils to air dry/drain prior to stacking

Please repair to provide smooth, easily cleanable surface and to prevent potential harborage conditions for pests

Light shield on cook line cracked and not installed properly to protect light bulbs. Please replace and install properly

Light bulb burnt out in hood canopy on right side of cook line. Please replace to provide adequate lighting in this area

These areas in need of cleaning: some refrigeration door gaskets, handle area some refrigeration units on cook line, exterior spice shaker containers on cook line, hood filter/hood canopy above sautee station, floor/equipment juncture including castors on fryer next to handwash sink and interior some of the outdoor storage sheds. Traulsen 2 door defrost cycle set during lunch serve period. Please reset defrost cycles so they do not interfere with busy service times to help in maintaining the food at proper temperature. Food slightly elevated at time of inspection; relocated all potentially hazardous food items to walk-in-cooler until defrost cycle reset. Repair person called.

Dishwash area handwash sink and men's employee restroom handwash sink draining slowly. Please repair and maintain so handwash sinks drain properly at all times

Some fruit flies observed in facility mainly near mop sink. Please continue to work with your extermination company, eco-lab to eliminate the flies.

Some food containers cracked on top ledge, color coded plastic handles on tongs on cook line worn, some color coded sheet trays worn and bottom of some baking sheet pans no longer smooth and easily cleanable. Please replace these items to provide durable, smooth easily cleanable surfaces; continue to monitor and replace as needed

(Critical) Some potentially hazardous food items on top of refrigerated unit above cook line (broiler unit) found at improper temperature. Potentially hazardous food shall be maintained at 41°F or below or 135°F or above. Corrected at time of inspection by discarding potentially hazardous food items found >41°F. Some food items observed in shallow pans at 52°F. This unit is designed to be used with ice and deep pans; when unit was set up deep pans were not used for all potentially hazardous food items and the ice level was lower than "normal". Deep pans were provided and additional ice added. Please continue to monitor with food thermometers which are available on the cook line to assure potentially hazardous food maintains at 41°F or below at all times.

There appears to be adequate shelving in this facility for air dying clean dishes and utensils. Facility did remove an extra piece of equipment that was not in use which made room for additional shelving unit in dishwash area for drying. Thank you.

(Critical) Inadequate sanitizer contact time at manual warewashing operation at bar. Critical corrected: at time of inspection observed inadequate sanitizer contact time for clean glasses at bar manual warewashing operation. Per manufacturer's label use instructions contact time is one minute in sanitizer solution; pic will discuss with all employees. New sanitizer packets they will be using this week state a two minute contact time

These areas in need of cleaning: floors under/near rethermalizer area end of cook line, underside expo window and microwave oven handle area end of cook line closest to fry station. Please clean now and as often as necessary to maintain in clean condition

(Critical) Some metal bowls found in poor repair on clean dish storage rack. Food-contact surfaces shall be smooth and easily cleanable. Critical corrected: at time of inspection discarded severely bent metal bowls. Please monitor all food-contact surfaces and discard as needed if no longer smooth and easily cleanable

(Critical) No sanitizer present in sanitizer compartment of manual warewashing operation at bar. Sanitizer shall be provided at proper concentration at all times. Critical corrected: at time of inspection some confusion as to what sanitizer was being used at bar; emptied sink and set up sanitizer solution at proper chlorine concentration at manual warewashing operation.

Some plastic lids cracked and some tray corners in poor repair (plastic coated lining wearing). Please continue to sort through all non-food contact surfaces and replace as needed

Corrected at time of inspection by relocating bus tubs stored on floor in front of handwash sink. Floor tile grout missing in some sections of the kitchen. Please re-grout to eliminate standing water and to provide smooth, easily cleanable surface.

Drain line for one of the ice bins at bar unattached to underside of ice bin. Please reattach and maintain in good repair. Please maintain proper air gap when repaired

(1) observed leak in roof in prep kitchen entrance; leak is not effecting any food preparation activities; is confined to a section of floor between 3-comp sink and shelving unit. Roof to be repaired this week. Please maintain in good repair. (2) observed leak in walk-in-cooler near entrance; no food stored under leak. Please repair ceiling to elimate leak and maintain in good repair

These areas in need of cleaning: floor/wall juncture under cooking equipment, floor/equipment juncture in some areas (including castors on equipment), interior fryers on cook line, interior garland oven that is not in use, left catch tray on garland oven and floors under recycler unit in dishwash area. Please clean now and as often as necessary to maintain in clean condition

(Critical) Observed the upper level of the raised rail refrigerated unit with insert pans of cheese at 52 degrees f. And yogurt at 46 degrees f. Pic person in charge corrected this item by discarding time/temperature control for safety food items/potentially hazardous foods from raised rail section. Refrigerated draw section of refrigerator was found to hold cold foods at 41 degrees f. Or less as required.

Observed the ice bins located at the bar with ice being used for both keeping containers of food products cold and for human consumption. Pic corrected this item during this inspection by proper use of ice bins at the bar area by dedicating one ice bin for ice for human consumption and the other ice bin for cold holding food products.

Observed bulk food bins of white powder located in the dry food store room without labeling to identify contents by common name. Pic corrected this item by instructing an employee to properly label the containers, please note during this routine inspection, labels to identify the contents of the food in the containers were placed on the sides of the bulk containers to properly identify the food products as required

Observed the dry food store room with a light shield for the light fixture removed. This item was corrected during this inspection by the pic instructing an employee to re-install the light fixture shielding.

Obsereved the cooks area preparation refrigerator with a raised rail section located opposite of the broiler improperly holding cold foods at temperatures above 41 degrees f. To correct schedule service to the equipment to properly cold hold foods at 41 degrees f. Or less as required for time/temperature control for safety food items
