(Critical) On follow up inspection found a t-valve/sidekick on the faucet, and a y type shutoff valve on t-valve/sidekick, and both water supply line to chemical dispenser, and water supply line to hose connected to y type shutoff valve. This is an acceptable setup. Violation corrected. Backflow risk control plan has been reviewed and should work if followed.
(Critical) On follow up inspection used a quat test kit and found sanitizing solution flowing from quat dispenser at about 150 ppm, between manufacturer's recommended range of 150 to 400 ppm. Pic states quat dispenser was repaired and adjusted. Violation corrected.
(Critical) On follow up inspection found all paper towel dispenser stocked in all kitchen handsinks. This violation was corrected on routine inspection, and reverified on follow up.

To correct install a t-valve/side kick on the faucet, then install y type shut off valve on t-valve/side kick, and connect water supply line to chemical dispenser and hose to y type shutoff valve. Found the interior surfaces of microwave oven on the cooking line soiled with accumulation. Found the rack in the cheese melter on the cooking line soiled with heavy build up. To correct follow manufacturer's directions and clean microwave and cheese melter and clean as needed.
Found numerous stacked pans stored wet on the clean utensil shelf. To correct allow clean pans to air dry prior to stacking and storing.

Found the hood filters, edges and some surfaces of hoods soiled with grease and dust. Found some door gaskets of cooler soiled with accumulation, and the bottom of some coolers with pooling water. Found the paper towel dispenser in the kitchen waitstation broken and not working, but paper towels available. Found the left ice machine in the kitchen not working. Found the left fan in the evaporator coil of the beer walk in cooler not working. To correct repair paper towel dispenser, ice machine and walk in cooler fan.
Found slight accumulation in the pop gun nozzle in the center of the bar. To correct follow manufacturer's directions and clean pop nozzle.
Found the handle of ice scoops in several ice bins in facility stored on top of ice. To correct store ice scoops with handles up or in a container/holder. Found the pop guns in the waitstation by the bar laying on top of ice in the ice bin. To correct securely place pop guns in holder.

In-use utensils at bar stored in water. Please store dry in a clean, protected location with handles up and clean and sanitize as required throughout the day. The bar also has potential for a dipper well for utensil storage; if you would like to use the dipper well please extend the water inlet over the utensil container with a proper air gap and provide water at sufficient velocity to flush particulates to the drain. The drain line would also need to be air gapped, currently an air break. The air gap would need to be twice the pipe diameter above the flood rim

Please replace these items to provide durable, smooth easily cleanable surfaces; continue to monitor and replace as needed
(Critical) Some potentially hazardous food items on top of refrigerated unit above cook line (broiler unit) found at improper temperature. Potentially hazardous food shall be maintained at 41°F or below or 135°F or above. Corrected at time of inspection by discarding potentially hazardous food items found >41°F. Some food items observed in shallow pans at 52°F. This unit is designed to be used with ice and deep pans; when unit was set up deep pans were not used for all potentially hazardous food items and the ice level was lower than "normal". Deep pans were provided and additional ice added. Please continue to monitor with food thermometers which are available on the cook line to assure potentially hazardous food maintains at 41°F or below at all times.
