(Critical) The ice machine upstairs and the ice bin in the basement bar both are not air gapped. Have your plumber out to air gap both of these lines within 10 days. Fax me over a copy of their work order and take pictures of both lines so i can see they are properly air gapped. You need to submit within 2 weeks.
(Critical) There were 2 containers of unopened half and half in the upper bar cooler with a sell by date of 2/4/15. You need to watch dates and be sure to discard outdated product. Corrected by discarding.
You need to thoroughly clean the interior of the beer walk in cooler in the basement. Be sure to thoroughly clean all the lines and pipes in there as there is mold growing on them.
You need to have the flush valves for the urinals keep the water running for several seconds to thoroughly flush the urine from the troughs. Have your plumber do this and fax me over a copy of the work order reciept within 2 weeks.
(Critical) There is a significant number of small flies at both bars. You need to contract with a licensed pest control operator to get you on a plan to rid the flies from your establishment. Save all work orders from your pest company for me to review on your follow-up inspection. I will be back in 3 weeks to verify no flies are present in your facility.
You are missing coving at the floor/wall juncture under the sinks at the 8 ball bar. Provide coving within 3 weeks.
Post a handwash sign at the blind pig bar handsink.
The flush valves in the 8 ball male restrooms don't stay on to thoroughly flush the urinals. Have new push valves put on so the water runs for several seconds on its own.
(Critical) No sanitizer detected in the dishmachine. Must be dispensing 50-100ppm chlorine. Staff must be using test papers everyday you are open to verify proper operation. Do not use dishwasher until it is dispensing 50-100ppm. I will be back in 2 weeks to verify.

I observed the ice scoop stored on top of the ice machine, which is a dirty surface. Store the ice scoop in a clean container between uses. Highly recommend putting an ice scoop holder on the side of the ice machine.
Water is leaking onto the floor in the room the ice machine is in. Find the cause and repair it. You also need to repair the wall where the water is because it's rotted out the drywall and base coving.
(Critical) Manager did not know where test papers were when asked. Eventually she found one unopened chloring and one quat test papers. For both bars, you are using chlorine based sanitizers, so you need chlorine test papers for both. Discard quat test papers, as they are not needed here. Corrected by providing chlorine test papers for both bars. Highly recommend installing something on the wall to hold the test papers in so they don't "disappear"
The interior of the microwave in the basement bar was left dirty. Staff need to be wiping these out with sanitizer cloths often!
(Critical) There is a large infestation of drain flies at both bars. You need to have your licensed pest company out once a week for the next month to treat your bars. I will be back in 4 weeks to verify they have been removed from your establishment. Save copies of their reports they give you for me to look at on re-inspection.
(Critical) There is no hot water coming to any of the handsinks. It appears the hot water tank is not working properly. Have plumber out to repair asap. Fax in copy of their work order by wednesday, oct 3, 2012.
(Critical) All the cleaning agents were stored on the drainboard side of the sink in the basement bar. Do not store cleaning agents on or near the drainboard. Also, there was a gallon of glass cleaner in a cabinet above clean glasses in the upper bar. Store toxic items in safe locations. Corrected by storing safely.
No paper towels were stocked at the upper bar handsink. Staff need to always re-stock. Corrected.
One of the soda guns has duct tape on it. Replace this nozzle so you don't have to use duct tape to hold it together.
There is an area above the upstairs bar that is not closed off from the ceiling. You need to clean all the dust that has accumulated and then provide additional drop ceiling to close off that area.
(Critical) The auto dishmachine had empty containers of detergent and sanitizer. You must be inspecting the dishmachine and product containers on a daily basis to assure working properly. Use chlorine test papers everytime before you use the machine each day to assure there is 50-100ppm chlorine coming into the dishtank. Manually wash in triple sink until eco lab is out to re-stock your product. I will be back in 30 days to verify!!!
(Critical) There is a large infestation of fruit/drain flies in both bars. You need to contract with a licensed pest control operator that performs integrated pest management procedures to find the causes of this infestation and set you up on a detailed mitagation plan. I will be back in 30 days to re-inspect your facility to verify the flies are no longer in your store. Faith is to keep me updated weekly via e-mail on how things are progressing.
(Critical) I observed chemicals stored unsafely in the dishwash room. A gallon of bleach was stored on top rack above clean dishes. Also, chemical spray bottles were found stored in areas where they could contaminate clean ware coming out of the dishwasher. Store chemicals in areas away from foods and food contact items. Corrected by doing such.
Provide a shatterproof light bulb for the basement bar service area.
Service the urinal flush valves so they stay on for a few seconds to adequately flush urinals. Also, you need additional spray arm in one of the urinals.
A bag of unpopped popcorn was found exposed. Once bag is opened, keep in covered container.
There are no qt-10 or chlorine test papers available. Obtain them and use regularly to verify proper sanitizer strengths.
These areas in need of cleaning: some of the liquor storage racks at both bars, some pop dispensing hoses and holders at both bars, floor/wall juncture in a few areas under both bars and interior some waste receptacles in bottle storage room. Please clean now and as often as necessary to maintain in clean condition.
Please keep lids closed when not in use on dumpster and other waste receptacles behind facility

One of the ceiling tiles out of place in the room with ice machine and reach-in-cooler. Please put back in place and consider replacing ceiling tiles with the vinyl-backed tiles that are smooth and easily cleanable
Please re-establish ice bin air gap for upstairs ice bin closest to entrance. The air gap shall be twice the pipe diameter above the flood rim of the drain. You may remove the elbow on the end of the drain line instead of trying to tie it up with brackets as long as this does not cause water build-up onto the floor
(Critical) Several flies in facility in various areas, especially bottle sorting room. The presence of insects, rodents and other pests shall be controlled to minimize their presence on the premises. Not corrected. Please provide a copy of your extermination companies recommendations and invoice. Recommend storing empty bottles and possibly sorting empty bottles in outside storage shed. Combination to lock not known at time of inspection; at follow-up please provide access to storage shed.
The sump pump in the pail used to pump the condensate water not functioning properly. Water level is at the top of pail. Please repair/replace sump pump or change bucket on a more frequent schedule
Some refrigeration door gaskets in poor repair. Please replace to provide good seal and so smooth and easily cleanable
