(Critical) Observed slicer blades and guard area with an accumulation of dried on food debris (appears slicer was not properly disassembled and then washed, rinsed and sanitized after use yesterday). All slicer blades and slicer components must be washed, rinsed and sanitized after every use and must be clean to sight and touch. Pic corrected by disassembling slicer then washing, rinsing and sanitizing all components at time of inspection. Violation corrected. , observed can opener blade in prep area with an accumulation of dried on food debris. All can openers must be washed, rinsed and sanitized every 24 hours and must be clean to sight and touch. Pic corrected by washing, rinsing and sanitizing can opener at time of inspection. (Critical) Observed plate of freshly cooked rte chicken nuggets tightly covered with plastic wrap cooling in prep area ric. Chicken nuggets have been in cooler for 20 minutes per staff and have an internal food temperature of 125-130f. This cooling method is not approved. Need to loosely spread food evenly out on sheet pan and leave uncovered while actively cooling in cooler. Please remember that all phf must be cooled from 135f to 70f within 2 hours and from 70f to 41f or below within a total of 6 hours. May need to utilize ice baths as needed. Whatever cooling method you employ, staff need to be monitoring food as it passes through the temperature danger zone. Pic corrected by uncovering and spreading out chicken nuggets on pan in cooler. Observed rte foods stored under chipped/flaking paint in wic (painted vent cover in wic is flaking off). Foods were loosely covered with deli paper and no signs of paint were observed in food at time of inspection. All food must be stored in a clean, dry area and not stored under potential sources of contamination. To correct, do not store any food under the chipped paint until vent cover is repainted

Observed a heavy accumulation of mold and debris in wic floor drain (for condensate line discharge). To correct, please clean now and on a more frequent schedule.

(Critical) Observed pan of rte meatloaf and whole meatloaf in cres-cor hot holding unit in prep area with internal food temps of 105-110f. All potentially hazardous rte foods must be held at 135f or more at all times. Pic corrected by reheating meatloaf to 165f at time of inspection. Violation corrected

(Critical) Observed 12 cartons of chocolate milk in milk cooler on main serve line with expired quality dates (expired on 4/8/13). All milk must be used or discarded by date on container. To correct, need to rotate stock more often and discard all expired milk as needed. Pic corrected by discarding all expired milk at time of inspection. Violation corrected

(Critical) Observed cooked rte meatloaf in steam table not reheated to 165f as required. When i first arrived for inspection, meatloaf was being cooled on prep counter (pic was not sure if they were going to use it or not) and internal food temp was 105f (meatloaf was on counter for approximately 35 minutes per pic). Meatloaf was then placed in hot holding unit within 5 minutes of my arrival. After 20 minutes, observed another staff member take meatloaf out of hot box and take internal food temp with a food thermometer. Same employee then placed meatloaf in steam table on serve line. When i inquired what meatloaf temperature was, staff member stated food was at 124f. I verified internal food temp of 110-120f. All staff in kitchen knew proper 165f reheating temperature requirement, but they did not follow these procedures today. I attempted to verify meatloaf cooking temperatures from this morning, but temp logs were not completed as required by chartwell. Pic corrected by reheating meatloaf to 165f in oven at time of inspection. Violation corrected.

All slicer components must be washed, rinsed and sanitized after every use and must be clean to sight and touch. All raw animal foods must be stored below and away from all rte foods at all times to protect foods from possible cross contamination. Observed single service spoons and forks in condiment station area in dining room stored with food contact portion of utensil pointing up. All utensils must be stored so the food contact portion of utensil is pointing down (and handles are up) to protect utensils from possible contamination when handling (from customer hands)
