(Critical) Observed small flies (likely fruit flies) in the dish washing area of the kitchen, at the bar and in the basement below the bar. Also, different type of flies were observed in the slime on the floor tile in front of the walk in freezer. Take necessary action using approved methods/products to eliminate these pests. Have your pest control contractor stop by asap for an additional service visit (provide me verification of their visit) utilize a floor drying fan at the dish washing area during the over-night hours to dry up the out of the way areas.
(Critical) After an employee ran the dish washing machine, i tested the rinse water for the residual sanitizer concentration. No chlorine was detected. The chlorine in the dispensing tube had siphoned back into the bulk container over night. The system was primed and 50 to 75 ppm chlorine was achieved in the rinse water. Have your contractor repair this system so the chlorine sanitizer delivery system stays primed
(Critical) A thin tipped, digital metal stem food thermometer is required to verify the cook off temperatures for thin, raw animal foods for your kitchen staff
(Critical) The walk in freezer drain line is discharging onto the floor in the mechanical room causing water damage to wallboard and wall structure and allowing mold to grow. Re-plumb this drain line so it terminates with the required air gap (vertical air gap = 2x the discharge pipe diameter). The drain line may need to be re-routed to exit the freezer at a higher elevation to achieve the height needed for an air gapped discharge

(Critical) 1) provide an air gap (2 inches high) for the two flexible drain lines from the basement ice machine above the 3" receiving drain. 2) provide an air gap (1. 5 inches high) for the combined condensate drain line serving the basement walk in cooler and the cooler for canned beer
(Critical) A nearly full, covered 1/3 pan of cream of broccoli soup was placed in the three door reach in cooler at the end of last night. This thick soup must be cooled using different methods (multiple containers to reduce the volume and thickness, in an ice bath, uncovered, etc. ). Ensure all staff are made aware of and are following rapid cooling techniques to cool your phfs
(Critical) 1) the interior surfaces of the ice making machine were observed to have slime growth on them. Have this unit professionally cleaned and sanitized to eliminate this build-up. 2) the ice tea dispenser spout was observed with a lot of slime build-up on the interior surfaces. Staff have not been disassembling this spout when the dispenser is washed. This spout was taken apart for cleaning
(Critical) Cream of broccoli soup and a small container of cheese sauce removed from hot holding last night were found at 49f and at 48f, respectively in the three door upright cooler this morning. These foods were discarded
Observed the outside trash dumpster with a cover open and the side doors open. Ensure these covers/doors are maintained closed
The bar cooler for canned beer has excessive moisture build-up in the bottom of the unit. Unclog this drain line to allow this water to drain away
Observed the trash dumpster to be over flowing with garbage bags. This restaurant along with a neighboring restaurant are now using a new trash disposal area. Apparently the pick-up frequency is not adequate enough to serve all of the users. Either increase the frequency or have additional/larger dumpsters provided to handle the volume
1) the floor drain located in front of the dish washing machine is leaking into the basement. Repair this floor/drain as needed. 2) remove the loose floor tile located in front of the walk in freezer. Moisture and debris are migrating under these tile. This area is likely a fly breeding area
Observed deteriorated pipe insulation for a refrigeration line in the basement leading to either the walk in freezer or the walk in cooler. A lot of condensation is dripping onto the floor because of this condition. Remove the existing, damaged insulation and provide new insulation to eliminate this moisture source
Multiple areas in the facility (the basements and the small back kitchen room where the electric slicer is located) have old, broken/un-used equipment accumulating. Please remove these unnecessary items to free up additional space and to make cleaning these areas easier
Observed mold growth on the bar matting and the shelves below the glass ware above the ice bin at the bar. Thoroughly clean these surfaces now and regularly in the future to prevent this condition
The floor mats located in front of the walk in freezer have excessive food/debris build-up on them with some areas that are wet creating a food source and/or breeding area for pests. Asap clean and dry these mats and the floor underneath
(Critical) Observed the following ready to eat foods in the kitchen refrigerators to be past their 7 day use date: chicken salad (12-30), deli meat for chef salads (1-1), hot dogs (1-1). Corrected by discarding these foods
Secure to the wall and floor, as needed, the white pvc drain pipe for the walk in cooler and freezer located in the mechanical room to the waste-water is routed to the floor drain. The current air gaps must be maintained.
(Critical) Observed excessive old food residue on multiple surfaces of the electric food slicer (back side of the blade, the push block, and on surfaces under the blade guard, etc. This slicer was unplugged, and disassembled for cleaning. ,
(Critical) Observed the following foods in two steam tables that were not at operating temperature. The foods (black beans, chili, cheese sauce, soup) were found to be at 98 to 129f. Corrected by reheating these foods to above 165f and by turning up the hot holding units
(Critical) Observed approx. 1/2 gallon of beef barley soup removed from hot holding last night that was placed in a covered plastic container in the three door upright cooler to still be at 52f. Corrected by discarding
Observed the following items to be in need of repair: two door gaskets on the three door upright kitchen cooler to be torn, the condensation drip pan for the basement walk in produce cooler (leaks water), the lennox furnace in the bar basement--condensation from running the a/c unit is leaking onto the floor. Repair the above items as needed
Observed gaps to still be present under the two entry doors located along michigan avenue. Provide replacement door sweeps/weather-stripping and/or build-up the door sills as needed to eliminate the existing gaps
The interior surfaces of the basement ice making machine need to be cleaned now and more frequently in the future
The floors in the following locations are due for cleaning now and more frequently in the future: the floor / wall junction under the dish washing machine, slime build-up on the bar basement floor near the furnace, standing water on the basement floor in the boiler room, a couple small areas of the bar floor, and misc. Wood, bottles, etc. , under the rack/shelves in the bar basement for the soda syrup boxes
Observed the two sanitizer test kits in the kitchen (chlorine and quat) to be old and discolored. Kitchen staff could not locate any other test papers at the time of the inspection. Either locate or purchase additional test papers for daily use
1) complete the extension of the basement walk in cooler drain line (located in the boiler room). 2) bar basement: re-establish the drain line air gaps (the drain line air gap = 2 x the discharge pipe diameter) for the ice machine and the ice bin above the flood rim of the receiving basin. 3) provide pipe insulation on the bar ice bin drain line to eliminate the condensation that is forming on the exterior of this drain line that is dripping onto the floor
(Critical) Observed small flies in six, partially consumed liquor bottles (grand marnier, sweet and dry vermouth, tropical pineapple, apple and island blue schnapps). This liquor was discarded. Ensure your food and beverage is protected from pests
(Critical) Observed the following previously washed kitchen items to be unclean: multiple pair of tongs, scoops, spoons, ladles, spatulas, the bar blender, iced tea dispensing spout, several plastic and metal kitchen food pans, the electric can opener blade, etc. Ensure staff are thoroughly inspecting these items after they have air dried, prior to placing in storage
(Critical) Observed several house flies in the main kitchen and numerous small flies (fruit flies?) at the main bar and in the basement under the bar. Take necessary action to eliminate fly breeding areas (debris build-up that is moist) on a regular basis
Observed gaps under the two entry doors along michigan avenue. Replace/adjust the door sweeps to eliminate the gaps that can allow pest entry
Observed multiple surfaces of the draft beer dispensing unit (custom made metal covering on this unit) that have open seams with moist debris build-up, and some rotting wood. Repair/replace this unit as needed to provide smooth and easily cleanable surfaces
Observed numerous areas of the facility with debris build-up and/or excessive moisture, such as: a couple depressions in the floor at the main bar, the floor at the wall and around pipes at the kitchen dish washing machine, the interior surfaces of several floor drain pipes (main floor and basement), standing water in the sw corner of the basement mechanical room, the basement utility sink, several drains and drain pipes in the basement below the bar. These areas are in need of better sanitation to eliminate odors and fly breeding areas.
No thermometers were located in the following refrigerators: the walk in produce cooler, the slide top bottled beer cooler at the bar, and the pepsi sliding door cooler in the basement. Provide thermometers so staff can monitor temperatures
Observed an excessive amount of what appears to be unneeded/old/broken equipment in the basement areas. Take the opportunity to sell/recycle/discard items that are no longer needed.
Observed a damp folded cloth towel left on the counter at the main bar over the night. Only store wet wiping towels in a container of sanitizer to prevent bacterial growth. At the end of the business day these towels should be wrung out and allowed to air dry by hanging them up in a suitable location
Observed damaged door gaskets on the upright three door kitchen cooler. Replace as needed to provide a cleanable surface and to seal in the cold air
The basement walk in produce cooler and the ceiling fixture located above the basement ice machine
1) observed water draining slowly from the two men's bathroom hand washing sinks. Unclog as needed. 2) re-establish the drain line air gap for the basement ice bin drain line and the produce walk in cooler drain line
Observed non functional light bulbs in the following locations: the three door upright kitchen cooler and kitchen ceiling fixtures above the three comp. Sink and the dish washing area. Provide replacement light bulbs/repair as needed
Observed two white plastic buckets used for transferring ice to the bar to be stored upright, uncovered on the basement floor. The interior of these buckets needs to be protected and allowed to drain out. Store these buckets inverted on a clean and sanitized surface
The sliding two door beer cooler at the bar has two thermometers that are inaccurate. One was reading 4f and the other 26f. Product in the cooler had a temperature in the upper 30's. Provide a replacement thermometer
(Critical) Observed cooked chicken at a temperature of 46 degrees f. At 11:30 am on 6/20/2011. According to pic person in charge the item would have been cooked on evening of 6/19/2011 and then placed into a covered pan into the preparation area reach-in refrigerator. To correct provide proper cooling for the chicken to meet the cooling requirements as required above. This item was corrected by the pic discardinig the chicken item found at improper food temperature
Observed some plumbing items located in the back kitchen areas to need repairs as follows: found floor drain slow draining water, to correct should clean out drain line. Observed handle missing on mop sink faucet, should replace handle. Found leaking faucet assembly at the spray rinse arm located beside the dishwashing machine.
Observed the light fixture located in the walk-in produce cooler of the basement with an exposed light bulb, and the light fixture located above the ice machine was found with end cap missing at tube shielding. To correct please shield the lighting in these locations to protect against breakage of light bulbs.
Observed the interior of the up right reach-in freezer with a frost build up. To correct schedule defrosting the reach-in freezer for cleanable surfaces
Observed the bar area pop gun holder with layer of syrup indicated a need for more frequent cleaning. Pic corrected this item by cleaning the pop gun holder

Observed the vent cover missing for the prep refrigerator. To correct should replace vent cover to divert a spill or splash
Observed the back kitchen screen door with an open gap where the screen is pulling loose from the door. To correct schedule repairs to the screen to prevent potential entry of insects to the kitchen

These areas in need of cleaning: pop gun holders at bar, interior bottom section 3-door upright reach-in-cooler in kitchen and interior salad reach-in-cooler, including top interior ledge. Please clean now and place on routine cleaning schedule

Please remove the spray nozzle on the hose in basement when not in use; existing atmospheric vaccum breaker attached to hose bibb. Corrected at time of inspection
(1) some refrigeration door gaskets in poor repair. Please replace to provide good seal and so smooth and easily cleanable. (2) please provide handle for reach-in-cooler door across from cook line. (3) please defrost upright freezer rear kitchen
(1) please re-establish the air gap, currently air break, for the ice machine drain line in basement; new upper section of existing ice machine recently installed. (2) please re-establish air gaps, currently air break, for ice bin drain lines at bar; receiving receptacle in corner same section of basement as ice machine. Please consider cutting the drain lines instead of tying them up. The air gap shall be twice the pipe diameter above the flood rim of the drain.
These areas in need of cleaning: floors under plastic shelving unit in kitchen, floors, walls and top of pipes under dishmachine, floors in rear kitchen with upright freezer, especially under table with slicer, vents to ceiling exhaust fan in middle of kitchen and floors throughout in both basements. Please clean now and as often as necessary to maintain in clean condition
