(Critical) Your facility was found with small flies on the walls and by the brooms and gargage cans. You need to contract with a licensed pest control operator to eliminate your store of these flies. I will be back in 3 weeks to verify they are gone.
The mobile food cart in the walk in cooler was observed dirty. Please be sure to clean this cart more frequently.

I did not detect any sanitizer at the chicken prep sani bucket. Need to keep at a minimum of 200ppm. Change out every hour at this station!
There is frost build up on the compressor in the front reach in cooler (slaw cooler). Repair asap. Also, the doors to the walk in cooler and freezer need to be serviced so they close tightly without any gaps.
Provide a small waste basket at the front handsink.
You folks need to change out your sanitizer water more often to keep it clean and at 200ppm quat.
Repeat violation 2 times in a row. Once again, the vacuum breaker cover is missing on the mop sink. Maintenance crew was in store and had a new one on truck to install.
There is a leak in the plumbing under the handsink near the 3 comp sink. Also, the faucet is leaking at the 3 comp sink. Repair the leaks asap. Also, the vacuum breaker on the mop sink is missing it's cover. Provide a cover asap. Unclog the checken prep handsink.
There are several wiping cloths and scrubs in one of the mop sink basins. Keep the basins cleaned.
Keep waste basket at each handsink. There is not one at the chicken prep area handsink.
Scoop for cinnamon sugar has no handle. Provide on that does and keep handle out of contact with sugar.
You need to adjust the metered foot pedal faucets at the handsinks so they are not excessively hot.

Critical corrected: at time of inspection (1) employee purchased atmospheric hose bib vacuum breaker and installed on end of y connector at chicken station mop sink. (2) re-installed atmospheric hose bib vacuum breaker on end of y connector at mop sink near restrooms.
(Critical) Some dented cans observed in storage area near prep sink. Critical corrected: at time of inspection set dented green bean cans aside for return to supplier. Provided and reviewed dented can handout. Pic will review with all staff
Currently chicken strainer and pans are rinsed in the mop sink. To prevent potential contamination either a separate rinse sink will need to be installed near the chicken station or one mop sink will need to be designated for rinsing raw chicken strainer and pans. Discussed with pic, have agreed to designate the mop sink closest to the chicken station for rinsing raw chicken strainer and pans in-between breading raw chicken. This mop sink will need to be kept clean and no mop water will be dumped into this sink. The existing chemicals shall be removed. The other mop sink will be used for chemical storage and dispensing and filling and dumping mop buckets. As discussed a short hose with a spray nozzle may be attached to the chicken rinsing mop sink if the spray nozzle is removed between uses. This will prevent the water build-up currently on the floor in front of the rinsing mop sink
Leak observed at mop sink closest to chicken station and at mop sink near restrooms. Please repair and maintain in good repair. In addition do not remove the atmospheric hose bib vacuum breakers that were installed
These areas in need of cleaning: interior ledges some hot holding cabinets and underside of wedge hot holding unit. Please clean now and place on routine cleaning schedule
Some wet dishes observed on clean dish storage rack. Please provide addtional shelving units for this storage rack to be able to spread clean dishes out for air drying prior to stacking
Floor grout missing in some areas of rear kitchen, especially near dishwash area. Please replace grout to prevent water build-up between tiles ( and potential insect attraction and harborage conditions)
Please secure conduit and other piping under make line so it does not lay on the floor obstructing the employees ability to clean the floors properly. In addition please provide cover for floor drain that is no longer used under make line
These areas in need of cleaning: floors under make line near conduit, interior mop sink near chicken station, floors near same mop sink, interior drain under pop dispensing machine and end of drain lines under pop dispensing machine. Please clean now and as often as necessary to maintain in clean condition
Some sticker residue exterior of long hot holding pans. Please provide dissolvable stickers or soak pans to remove old stickers
Please do not overfill the cup dispensers at drive thru to prevent potential contamination of the food-contact end of the cup when retrieving
