(Critical) Observed the mop sink with a chemical tower and sprayer attached to the mop sink faucet which is equipped with an atmospheric vacuum breaker. A plumbing system shall be installed to preclude back flow of a solid, liquid, or gas contaminant into the water supply system at each point of use at the food establishment, including on a hose bib if a hose is attached or on a hose bib if a hose is not attached and back flow prevention is required by law, by: (a) providing an air gap as specified under # 5-202. 13 (p); or (b) installing an approved back flow prevention device as specified under # 5-202. 14. (p) to correct provide a vented t also known as a side kick to the noted faucet.

(Critical) Observed the dishwashing machine with sanitizer less than 10ppm. A chemical sanitizer used in a sanitizing solution for a manual or mechanical operation at contact times shall meet the criteria specified under # 7-204. 11 sanitizers, criteria, shall be used in accordance with the epa-registered label use instructions. To correct discontinue use of dish machine and repair the noted dishwashing machine to properly sanitize the dishes. ,

(Critical) Observed a squeeze bottle with a blue liquid inside and no label near the dump sink. Working containers used for storing poisonous or toxic materials such as cleaners and sanitizers taken from bulk supplies shall be clearly and individually identified with the common name of the material. (pf) to correct provide the common name to the chemical being stored with in. Corrected: person in charge provided a lable for the noted item

Observed both gaskets torn on the coolers directly below the espresso machine. Equipment shall be maintained in a state of repair and condition to correct repair or replace the noted gaskets.

Observed the floors under equipment such as coolers and the ice machine to have a debris and dust build up. Physical facilities shall be cleaned as often as necessary to keep them clean. To correct clean the noted floors and maintain them clean

Observed the plastic unwrapped single service knives being stored with the food contact portions up and handles down. Knives, forks, and spoons that are not pre wrapped shall be presented so that only the handles are touched by employees and by consumers if consumer self-service is provided. To correct wash hands, put gloves on and rearrange the knives so that the handles are up and the food contact portion is down. Corrected: person in charge washed hands, put on gloves and placed the knives with the food contact portions down.

(Critical) Used a chlorine test kit and found the dishwashing machine not chlorine sanitizing dishes. Employee states she normally checks the dishwashing machine at the 10:00 at the time i was checking dishwashing machine. To correct call for repair and use the 3 compartment sink to wash, rinse and sanitize dishes. ,

Found the front door and door edges soiled with splash and accumulation. To correct clean door and edges and clean on a more frequent schedule.

Found heavy dust build up on cooler coils of display cooler. To correct clean coils and clean as needed.

Found a bag of water softener salt stored on the floor under shelf. To correct store salt on shelf at least 6 inches above the floor.

Found wrapped desserts in the front counter area without labels. To correct provide labels for wrapped desserts.

Asked employee how ice tea dispenser is cleaned, employee state ice tea dispenser is washed in the washing machine, and the beverage spout is flushed. To correct follow manufacturer directions and properly clean ice tea dispenser, that is, take apart spout and wash, rinse and sanitize all parts of the spout.

Observed the presence drain flies in the floor drain area of the 3 compartment sink. To correct keep areas clean and dry, in good repair, and continue fly control measures.

(Critical) Found time/temperature control for safety (tcs) food, turkey wrap 45°F, chicken salad 44°F, tuna wrap 45°F, stromboli 47°F, and quiche 45°F on the bottom, front area of the front counter display cooler. Pic corrected by placing these foods in a box marked do not sell, and states she will record these items as waste and discard them. Pic has been instructed not to use the bottom front portion of the display cooler until repaired/replaced to hold tcs food at 41°F, and to monitor and ensure tcs food is held at 41°F or lower. Note: this is not a repeated cold hold violation as situation and nature are not the same.

Note: a long plate was left on top of the air vent which blocked air flow in this cooler. ,

Discussed with staff the process they use to cool soup at the end of the day, and found staff placing soup to be cooled in plastic storage containers. To correct use shallow metal pans to rapidly cool soups from 135°F to 70°F in 2 hours, and then from 70°F to 41°F in 4 hours.

Found the floor drain cover missing in the basement storage area. To correct replace missing floor drain cover.

Observed an opened bag of water softener salt and a case of food supplies on the floor of the basement storage area. Pic corrected by placing items on plastic pallets.

(Critical) Found time/temperature control for safety (tcs) food, 2 trays of spinach and feta pockets 46°F to 52°F, in the display cooler. Pic states these pockets were just delivered. Pic corrected by removing all spinach and feta pockets from cooler, placed in a bag marked return to supplier. Pic has been instructed to take temperatures of incoming tcs food, and to reject any cold tcs food delivered above 41°F.

(Critical) Used a test kit and found no sanitizer residual in the sanitizing bucket in the front counter area. Pic corrected by having employee remake sanitizing bucket to proper concentration (verified with test kit). Pic has been instructed to use test kit and verify employees are making up proper concentration of sanitizer.

Found the spray nozzles of the dishwashing machine clogged with debris. To correct clean as soon as possible, and clean on a more frequent schedule to preclude accumulation.
