Observed sanitizer bucket at wrong concentration, 150ppm of quat ammonia. Cloths in-use for wiping food spills from tableware and carry-out containers that occur as food is being served shall be: (1) maintained dry; and (2) used for no other purpose. (b) cloths in-use for wiping counters and other equipment surfaces shall be: (1) held between uses in a chemical sanitizer solution at a concentration specified under # 4-501. 114; and (2) laundered daily as specified under # 4-802. 11(d). (c) cloths in-use for wiping surfaces in contact with raw animal foods shall be kept separate from cloths used for other purposes. (d) dry wiping cloths and the chemical sanitizing solutions specified in subparagraph (b)(1) of this section in which wet wiping cloths are held between uses shall be free of food debris and visible soil. (Critical) Used a quat test kit and found the quat sanitizing solutions in sanitizing buckets and solution from the quat dispenser below manufacturer's recommended range of 150 to 400 ppm. To correct call for repair and use test kit to manually mix sanitizing solution. ,

(Critical) Found the water supply line of the espresso machine without back flow preventer. To correct provide a back flow preventer with intermediate vent meeting asse 1012 or 1022. ,

Found the spray nozzles of the dishwashing machine clogged with debris. To correct follow manufacturer's directions and clean spray nozzles.

Found the cleaned pitcher of blender with water on bottom. To correct allow pitcher to drain after washing before placing in blender base.

Found the floor, under equipment, edges and corners, in the back kitchen soiled with accumulation of debris. To correct thoroughly clean floor and clean on a more frequent schedule.

(Critical) Used a chlorine test kit and found the chlorine sanitizer in the 3 compartment sink and towel bucket for the espresso machine at very high levels. Pic corrected by re-making sanitizing solutions to manufacturers specification of 50 to 200 ppm. Pic has been instructed to monitor and ensure staff are properly mixing sanitizing solutions.

Found the ceiling in the back kitchen prep area soiled with splash. To correct clean ceiling and clean on a more frequent schedule.

Pic corrected by providing 7 day ( day prepared + 6 days) datemarks on soy chai and coffee creme base, and has been instructed to monitor and ensure staff are properly datemarking items. ,

(Critical) Used a chlorine test kit and found the dishwashing machine not chlorine sanitizing dishes. To correct call for repair, and use the 3 compartment sink to wash, rinse, and sanitize dishes. ,

(Critical) Found time/temperature control for safety (tcs) food, an opened carton of half and half 45°F, cream cheese cups 49°F, and an unopened gallon of milk 51°F in the right side of the 3 door cooler in the front counter area. Pic corrected by discarding tcs food, and has been instructed not to use this cooler to hold tcs food, until repaired/replaced to hold tcs food at 41°F. ,

Observed the presence of flies in the back kitchen prep area and in the front counter area. To correct keep areas clean and dry, in good repair, and continue fly control measures.

Found the 1 door reach in freezer in the side front counter area without thermometer. To correct provide internal hanging thermometer.

(Critical) Used a chlorine test kit and found the sanitizing bucket used to wipe the steaming wand of the espresso machine clearly above manufacturers specification of 100 ppm. Employee corrected by remaking chlorine sanitizing solution to manufacturer's specification of 100 ppm. Pic has been instructed to monitor and ensure sanitizing solutions are made to manufacturers specification of 100 ppm.

Found the drain line of the water softener/regeneration system of the espresso machine no longer securely strapped to the sink drain line and without proper air gap. Pic corrected by re-securing drain line to sink drain line providing adequate air gap.

Found the door gaskets, and bottoms of coolers soiled with accumulation. Found the interior top and edges of the ice cream counter top freezer soiled. To correct clean as soon as possible, and clean on a more frequent schedule.

Observed the presence of flies in the front counter 3 compartment sink floor drain area. To correct keep areas clean and dry, in good repair, and continue fly treatments.

Found the top lid door trim of the counter top ice cream freezer lifted and a gap between trim and counter top. Found the counter top back splash/trim around the left counter top handsink loose and not secured. To correct repair/replace door trim of counter top ice cream freezer and trim around handsink and seal gaps.

Found the water discharge drain line of the water softener draining into floor drain receptacle without air gap. Pic corrected by re-securing drain line to other drain line pipes re-establishing air gap. Pic has been reminded to check and verify that drain lines are air gapped.

Found a bag of water softener salt on the floor of the mop closet. To correct store water softener salt (food) on shelf at least 6 inches off the floor.

Found a few nozzles of the dishwashing machine clogged with debris. To correct clean nozzles, and clean as needed.

Found the bottom and door gaskets of coolers and freezers soiled with accumulation. Found the inner top lid of the ice cream chest freezer on the back counter top soiled with accumulation. To correct clean as soon as possible and clean on a more frequent schedule.
