In the rear dry storage area, observed a cfl lamp without proper protection. Correct by installing end caps on each of the light bulbs

Observed refrigeration units without ambient air thermometers: 1. In dry storage area, small white refrigerator 2. In the sushi make area, the fish display case near the cash register. 3. In the rear storage area, the white two door upright unit pic corrected #2 during the inspection by installing an approved thermometer. Correct the other two by installing an approved thermometer at each location

In the kitchen, observed two light fixtures with four fluorescent light bulbs without proper protection. The bulbs have plastic shields but do not have end caps. Correct by installing end caps on each of the light bulbs

In the kitchen at the true upright refrigerator observed both doors to have ripped door seals. Correct by replacing door seals. Pic indicates the door seals have been ordered and will be replaced very soon. This is a repeat violation

In the storage area between the bathroom and the dining room observed boxes of single service items stored on the floor. Correct by installing shelving so these items are at least six inches off of the floor

(Critical) Observed no air gap for either the ice maker drain or the ice bin drain at the ice machine in the rear unapproved prep area. Correct by providing an air gap of two pipe diameters of the drain line of the equipment above the drain the equipment drains into

(Critical) Observed no consumer menu advisory on the menu mounted on the wall in the ordering area or on the wall in the kitchen. Correct by providing a proper menu advisory at each area

(Critical) At the unapproved rear prep area observed house pickled onion and jalepeno peppers. Pic indicates this food is for personal use. It appears this food is not being used in the restaurant food preparation. However, this type of food cannot be made or stored in a food establishment. Correct by removing this food from the premises.

(Critical) Observed no sanitizing of dishware. Correct by proper dishwashing including washing, rinsing, and sanitizing

Observed no use of dishwashing detergent during the wet washing of dirty dishware. Correct by proper warewashing including prerinsing, washing with approved detergent and sanitizing with proper sanitizing solution

Observed a table top fryer in the kitchen and two hot plates in th unapproved rear prep area without proper ventilation systems. Equipment should not be used until proper ventilation systems are installed with prior approval from health department and ann arbor building department.

Observed handwashing to be blocked with a trash can. Correct by not placing a trash can or other items in front of the handwashing sink

Observed the following pieces of food prep equipment of residential construction: 1. Toaster oven in the kitchen 2. Toaster oven in the unapproved prep area. 3. Blender in the unapproved prep area. 4. Two hot plates in the unapproved prep area. Correct by submitting manufacturers specification for replacement equipment before purchasing equipment.

Observed the only food probe thermometer in the establishment to have a temperature range for measuring 50 degrees f to 500 degrees f. Correct by obtaining a thermometer with a proper temperature range

In the basement observed empty soda syrup boxes, empty cardboard boxes, and other trash needing to be discarded. Correct by discarding these items. In several storage areas observed unused and extra equipment. This equipment is cluttering the storage area and may prevent proper cleaning of the area. Correct by removing these items from the premises and organizing these areas

Observed rear door not tight fitting. Correct by repairing or replacing

Observed an unprotected light bulb in the food storage room off the hallway to the restroom. Correct by installing an approved shielded bulb/fixture.

Observed open boxes of single service items, containers of cooking oil, and other food items stored on the floor. Correct by moving items to proper storage areas

Observed no antichoking poster in the restaurant. Correct by obtaining and displaying an anti choking poster

Observed the overall surfaces and lack of handwashing facilities in the rear unapproved prep area to be not acceptable for your present use. This area was not approved for these activities during your original plan review and approval to operate. It was indicated that this area is used for personal use only. However, based on the amount of equipment, size of food containers etc. It appears that this area is used to process and store food used in this food operation. To receive approval of this area for food preparation a plan review must be submitted, reviewed and approved prior to any remodeling being completed. Use of this area for food activities must be discontinued until approval is obtained
