(Critical) Observed all 8 pop nozzles on pop machine in dining room with an accumulation of mold and debris. All pop nozzles and pop machine components must be washed, rinsed and sanitized every 24 hours and must be clean to sight and touch. Pic corrected by washing, rinsing and sanitizing all pop nozzles at time of inspection. Violation corrected

(Critical) Staff need to be verifying sanitizer concentration with test strips in 3 compt sink and wiping cloth buckets more often and as needed. Corrected by educating all staff on proper sanitizer concentrations and test strip usage (including test strip location) at time of inspection. Violation corrected

(Critical) Observed facility not sanitizing pans and utensils properly as i measured 0 ppm of quat in 3 compt sink (auto quat dispenser appears to be broken). Need to sanitize as per manufacturers directions on chemical bottle (in this case 150-400ppm). Pic temporarily corrected violation by manually mixing sanitizer compt to 400ppm of quat at time of inspection. Please educate and train staff to manually mix sanitizer (and verify with test strips) until quat dispenser is repaired and then inspected by the health dept. I will check this item at follow up inspection. Correct within 10 days. ,

Observed 0 ppm of quat in wiping cloth bucket on make line. Need to store all wet wiping cloths in approved sanitizer concentration in between uses to help prevent the accumulation of bacteria on wiping cloths

Observed box of rte italian dressing packets stored under handsink sewer line in front prep area. All food and s/s items must be stored in a clean, dry area and not exposed to potential sources of contamination. To correct, please relocate food to approved location as soon as possible

Observed iced tea container next to pop machine in dining room not covered. All food must be stored in a protected manner. To correct, please cover all food as needed.

Observed wiping cloths stored in empty bucket in front prep area. All wiping cloths must be stored completely submerged in a properly mixed wiping cloth bucket when not in use. To correct, please mix wiping cloth bucket to 200ppm of quat as soon as possible

Observed no employee handwashing sign at handsink in ladies restroom. To correct, please provide as soon as possible

Observed single service plastic bowls and lids on shelf under sandwich make line stored with food contact portion of bowl/lid up and exposed. All s/s items must be stored in original packaging or stored inverted to protect s/s item from potential sources of contamination.

Observed no thermometer in 2 door ric in front prep area. To correct, please provide as soon as possible

Bread forms found drying on plexi-glass barrier between 3-comp sink and mop sink. Please discontinue drying bread forms on top of mop sink barrier. Discussed cleaning out closet in office, which will provide shelving for chemical storage. This will then free up a shelving unit across from manual warewashing for additional drying space

Some clean dishes found stacked wet. Please provide adequate shelving to allow clean dishes to drain/air dry prior to stacking

Handwash reminder sign missing from men's employee/public restroom. Please provide sign notifying food employees to wash their hands
