(Critical) Observed low water temperature (70f) in wash compt of three compt sink. Bread forms and utensils observed in wash compt at time of inspection. The temperature of the wash solution in the 3 compt sink shall be maintained at not less than 110f (120f per subway standards). To correct, please change 3 compt sink wash water more often and as needed. Pic corrected by changing and refilling wash compt to 125f at time of inspection. Violation corrected

Observed clear water leaking from the corner of one of the upright bread ovens in front prep area (water is from excess condensation inside of oven). To correct, please repair bread oven as soon as possible so all water/condensation is maintained inside of equipment

Observed wet mop stored in mop sink in back kitchen area. All mops must be hung up after use to allow for proper air drying

Observed end of pop machine drain line and drain receptacle under pop machine in dining room soiled with mold and debris. To correct, please clean now and on a more frequent schedule. , observed floors under upright bread ovens in front prep area with an accumulation of thick dust and food debris. (Critical) Observed all pop nozzle barrels on pop machine in dining room with a heavy accumulation of mold and debris. All pop nozzles and pop machine components must be washed, rinsed and sanitized every 24 hours and must be clean to sight and touch. Pic corrected by washing, rinsing and sanitizing all pop nozzles and pop nozzle barrels at time of inspection. Violation corrected

(Critical) Observed bowl of partially frozen/partially heated rte pastrami deli meat on prep table in back kitchen area with an internal food temperature of 25-75f. Upon further investigation, discovered that staff member tried thawing pastrami in microwave oven 30 minutes ago and left on prep table (stored at room temperature) when they got busy. All phf must be cold held at 41f at all times. Do not thaw foods in microwave oven and do not store phf at room temperature. Since rte pastrami has only been in the temperature danger zone for 30 minutes, pic corrected by portioning pastrami immediately and placing in wic to rapidly cool. Violation corrected

Observed 2 packages of rte pastrami thawing at room temperature on prep table in back kitchen area. Also, staff are partially thawing frozen rte food in microwave oven. Both of these practices are not approved. All phf must be thawed under refrigeration or under cool running water to prevent the accumulation of bacteria. To correct, please educate and train all employees on proper thawing procedures as soon as possible.

Observed pop machine drain line and pop machine drain receptacle in dining room soiled with mold and debris. To correct, please clean now and on a more frequent schedule

(Critical) Quaternary ammonium test kit not being used to monitor sanitizer concentration at manual warewashing operation and wiping cloth solutions. Corrected at time of inspection by pic using her quat test kit to confirm proper quat concentration in manual warewashing operation

(Critical) No sanitizer present in sanitizer compartment at manual warewashing operation. Sanitizer shall be provided per manufacturer's label use instructions. Corrected at time of inspection by emptying sink and setting up again at proper quat sanitizer concentration per manufacturer's label use instructions (states 150- 400 ppm quaternary ammonium). Per pic the quat dispenser is cracked on the top that attaches to the bottle; must push down on the top for quat to dispense properly. Quat dispenser section that is cracked to be replaced

No sanitizer present in wet wiping cloth buckets front counter and dining area. Please provide sanitizer at proper concentration and monitor throughout the day with your quat test kit to assure proper concentration

Mop stored improperly in mop sink. Please hang mop to dry between uses

These areas in need of cleaning: underside of pop dispenser housing, pop levers, interior drain under pop dispensing machine and end of drain lines under pop dispensing machine. Please clean now and as often as necessary to maintain in clean condition

No wet wiping cloth solution set up at front counter. Please set up wet wiping cloth solution daily for storage of cloths once wet and monitor quat concentration with your test kit

(Critical) Observed packaged food item thawing in rear handwash sink. Handwash sinks shall be accessible at all times for handwashing and used for handwashing only. Corrected at time of inspection by removing packaged meat from handwash sink and placing in empty compartment of 3-comp sink to properly thaw submerged with running water.

(Critical) Food not maintaining at proper temperature on sandwich make line. Temporarily corrected at time of inspection by discarding all potentially hazardous food on top of sandwich make line found at 47°F - 53°F. Facility temporarily closed until owner arrived with several bags of ice; set up sandwich make line with insert pans on bottom of unit with ice level above food level. Repair person has been notified. Please continue to purchase and add ice as needed to keep the ice level above food level. Use your thermometer to monitor food temperatures to assure food maintaining at 41°F or below at all times. Food is removed from make line at close and unit shut off; unit must be at proper temperature before any food is placed in unit. ,

Packaged pre-cooked meat not thawing properly in handwash sink rear kitchen. Potentially hazardous food shall thaw under refrigeration maintaining at 41°F or below, or completely submerged under cool running water or as part of the cooking process

Sandwich make line not maintaining food at proper temperature. Repair person has been notified; temporarily using ice until unit repaired (see 3-501. 16)

Some single-service cups opened from wrong end and some coffee filters found without cover in rear kitchen. Please open cups from bottom end to prevent potential contamination of food-contact end and keep coffee filters covered

Soap dispenser front counter handwash sink not attached to wall properly. Please reattach
