(Critical) Moldy bag of limes observed in single-service storage area ( inside refrigeration unit that is not operating in rear kitchen ). Food shall be safe, unadulterated and honestly presented. (Critical) No hot water available in facility. Hot water shall be sufficient to meet the peak hot water demands throughout the facility. Facility open thurs, fri, sat, sun only. Not open this evening (wednesday). Plumber scheduled to be on site later today. Facility serves no food; uses single-service cups and does have hand sanitizer available at all handwash sinks. Please call as soon as determination made regarding hot water heater; will discuss with supervisor for opening tomorrow. Not corrected. ,

(Critical) Provide air gap, currently air break on ice bin #4 at bar. Remove pvc from end of drain line and consider installing rigid pipe to provide proper air gap, a minimum twice the pipe diameter above the flood rim of the drian. Not corrected. ,

(Critical) Small leaks observed under first and third compartments of 3-compartment sink. Repair and maintain in good repair. Since the 3-compartment sink is the only warewashing in facility these sinks must be repaired/replaced and function at all times for manual warewashing. Not corrected. ,

Water-stained ceiling in women's public restroom. Repair leak and monitor; repair damaged ceiling

These areas in need of cleaning: walls and plumbing under front bar, floors in bulk pop storage room, floors in martini bar, ice machine floor drain, end of ice machine drain lines, ceiling vent and surrounding ceilings in pop storage room front bar area, some refirgeration door gaskets and handle area some refrigeration units (used for single-service storage, linen storage, etc). Please clean now and as often as necessary to maintain in clean condition

Please provide cover for drain at bar and patch any holes in floor tile to prevent standing water. Please repair stall walls in men's restroom. In process of being repaired; maintain in good repair. Please secure pop lines to wall at bar to make it easier to clean
