(Critical) Again found the thermometer sleeves dirty with an accumulation of food debris inside them. Thermometers are not being cleaned properly before they are stored. To correct - clean sleeves now and each time they are used. This is the second time in a row that this violation has been cited. As such a risk control plan is required to be completed and submitted to this office within 10 (nov. 16th) days for review. ,

(Critical) Found commercially made veggie dip, hummus in the 2 door reach-in unit opened without a date. Also found a ball of some kind of dough in plastic wrap without a label or date. Corrected - pic discarded these food items. In the future, all potentially hazardous foods and house made foods must be dated for a 7 day discard and if house made, food item must also be labeled with contents. If food items in this unit are for staff use, a designated area or shelf must be assigned and labeled as such

(Critical) Found the kitchen to not be equipped with the required needle tip thermometer suitable for thin masses. To correct - please obtain this required thermometer and have it available at the time of the follow-up inspection. Information on where to get this required thermometer can be found at the end of this report

(Critical) Found the can opener blade and the thermometer sleeves to have an excessive accumulation of food debris on them. To correct - thoroughly clean these items now and keep on a much more frequent cleaning schedule. A follow-up inspection will be performed on/about june 5, 2013 to verify correction

Found single service items stored without original wrapping or other protective cover in cabinet l-4, and in the dry storage room. Also found boxes of single service items stored directly on the floor in the dry storage room. To correct- place all opened single service items; paper plates, bowls, napkins in a covered durable, cleanable container if the original wrapper is not available; can also use zip-type plastic bags for small quantities

Found the bottom of cabinet l-19 to have debris collected there. To correct - clean now and keep on a more frequent cleaning schedule

(Critical) Found raw shell eggs stored on the top shelf of the reach-in refrigerator, above ready to eat food items. Corrected - raw shell eggs were relocated to bottom shelf in reach-in refrigerator.

Found an old baby bath tub stored on top of a box of single service cups/containers in the dry storage area. Corrected - baby tub was removed from the shelf, placed next to the door on the floor and will be relocated away from kitchen/food storage area asap

Found kitchen to be without any kind of sanitizer test strips for testing the concentration of the wiping cloth buckets used during kitchen service. To correct - provide appropriate test strips for use during kitchen service. It appears that the sanitizer used is bleach so the required test strips would be for chlorine

Found clean knives stored in a cardboard box in cupboard # u-14. The cardboard box is not an a pproved container as it is absorbant and cannot be cleaned. To correct - provide a container that is durable, non-absorbant and cleanable

Found restrooms without required handwashing signage inside the restroom. To correct - provide signage inside each restroom. An adequate location for the signs would be on the mirrors above the hand sinks in the restrooms. Signs were provided at the time of this inspection

Found restroom off the kitchen and the main women's restroom to be without the required covered waste receptacle. To correct - provide a covered waste receptacle for these restrooms

The black spray hose at the kitchen 2c sink is not approved and must be removed. Correct by removing this hose immediately. Continuation of non compliance may result in enforcement action. You may replace with a commercial spray rinse arm similar to the one at your dishmachine that is airgapped above the sink when hanging freely.

Domestic stoves in facility are not approved. Replace with commercial equipment when in need of repair.
