(Critical) Found time/temperature control for safety (tcs) food, 2 covered and filled containers of chili cooling in a thin ice bath in the walk in cooler. Pic corrected by having employee divide chili and place containers in ice baths, uncovered in the walk in cooler for rapid cooling. Pic has been instructed to monitor and ensure tcs food is cooled rapidly that is uncovered, from 135°F to 70°F in 2 hours, and then from 70°F to 41°F in 4 hours.

(Critical) Found a large pan of raw beef patties stored next to ready to eat food ( red peppers, cooked bacon and pasteurized shell eggs) in the top portion of the 2 door prep top cooler. Pic corrected by relocating ready to eat food and placed empty containers by raw beef patties. Found a container of raw sausage patties stored next to ready to eat food ( cooked bacon and hot dogs) in the bottom portion of the 2 door prep top cooler. Employee corrected by relocating raw sausage to a location below and away from ready to eat food. Note: pic states store manager states it is company policy to setup and display raw meats in cited arrangement in cooler. Please provide written company policy of arrangement as this policy violates cross contamination of the current food code.

Found the side edges of cooking hood soiled with grease and build up. Found the bottom of drawers and shelf edges soiled with accumulation of debris. Found the interior covers/lids and door gaskets of coolers and freezers soiled. Found the interior of microwaves soiled with splash and accumulation. To correct clean all noted areas and keep clean.

Found numerous containers of shake toppings without labels. To correct provide labels.

Found 2 bags of water softener salt stored on the floor next to the water softener. To correct store water softener salt on shelf at least 6 inches off the floor.

Found no thermometer in the 1 door reach in freezer. To correct provide internal hanging themometer.

Found the tube of the bulk milk/shake not cut to milk dispenser manufacturer's directions which is to cut tube to be flush with bottom valve. To correct follow manufacturer's directions and cut tube to be flush with bottom valve.

Found the lids of the dumpster in open position. To correct close dumpster lids.

Observed the presence of flies in the back kitchen area. To correct keep facility clean, dry, in good repair, and continue fly control measures.

(Critical) Found time/temperature control for safety (tcs) food, sour cream 48°F, cream cups 48°F, hamburger patties 47°F, and hot dogs 48°F in the walk in cooler. Pic states- walk in cooler was checked earlier and holding at 40°F, employees left door opened and the walk in cooler appeared to be on defrost. Pic temporarily corrected by placing items in the walk in freezer to quick chill back to 41°F. Pic has been instructed to monitor and ensure tcs food is held at 41°F or lower and repair walk in cooler if needed, as later walk in cooler appeared to be out of defrost mode and colder.

Found the walk in cooler not holding tcs food at 41°F. To correct repair/replace cooler to hold tcs food at 41°F or lower. Found the upper spray arm assembly of the dishwashing machine missing. To correct repair dishwashing machine to function properly, and use the 3 compartment sink to wash, rinse, and sanitize dishes.

Found the lid of dumpster in open position and several large buckets of used grease in dumpster area without cover. To correct keep dumpster and used oil containers covered.

Found the edges of the cooking hood and back wall soiled with greasy accumulation. Found the sides of the fryer, hood area, heat lamp, and cooler sides soiled with greasy accumulation. Found the interior of the microwave soiled. Found the bottom of the shake cooler soiled with accumulation of debris. To correct clean noted areas and clean on a schedule to preclude accumulation.

Found no handwashing signs in the ladies and mens public restroom. To correct provide signs reminding employees to wash their hands.

Found the floor in the cooking and prep area and under the drive thru area soiled with accumulation of debris. Found the ceiling air vent covers in the kitchen soiled with dusty accumulation. To correct thoroughly clean floors, air vent covers, and keep clean.

(Critical) Found hot time/temperature control for safety (tcs) food, bag of liquid cheese 120°F in the hot holding cheese dispenser. Pic corrected by discarding cheese and has been instructed not to use this cheese dispenser until repaired/replaced to hold hot tcs food at 135°F and above. This violation has repeated and a risk control plan has been issued.

Pic corrected by discarding chili and has been instructed to monitor and ensure employees are cooling chili from 135°F to 70°F in 2 hours and then from 70°F to 41°F in 4 hours.

(Critical) Found a y type shut off valve attached to the mop sink faucet which is downstream of the atmospheric vacuum breaker. To correct install a hose bibb vacuum breaker on the y type shutoff valve, and disconnect the hose after each use. ,

(Critical) Found time/temperature control for safety (tcs) food, a container of cream cups 46 to 48°F in the walk in cooler, which was previously left in an ice bath in the wait station. Pic corrected by discarding cream cups, and has been instructed to store cream cups in cooler at 41°F or use cream cups that do not require refrigeration. This is a repeated cold hold violation and a risk control plan has been issued. Found tcs food, liquid cheese 121°F in the hot holding cheese dispenser. Pic corrected by discarding liquid cheese, and has been instructed to verify that hot holding cheese dispenser is working properly, and repair cheese dispenser to hold cheese at 135°F and above if necessary. ,

Found tcs food, a container of cream cups 64°F stored in a container of melted ice, in the dining room wait station. Pic corrected by discarding cream cups and has been instructed to keep cream cups in cooler at 41°F. ,

(Critical) Found the front shield and edges surrounding the front shield of the ice machine soiled with moldy build up. Found the pop nozzles, internal diffusers and pop spout areas of the drive thru pop machine soiled with accumulation. To correct follow manufacturers directions and clean ice machine and pop nozzles as directed. ,

(Critical) Found facility using regular raw shell eggs for eggs dishes, which may be cooked any style on breakfast menu, and doesnot have proper consumer advisory. Please note pic states they use pasteurized raw shell eggs which would not require consumer advisory and found an opened case of pasteurized shell eggs in the walk in cooler, but eggs on the cooking line cooler were not pasteurized raw shell eggs. To correct either discontinue using regular shell eggs and use pasteurized raw shell eggs, or provide proper consumer advisory (disclosure and reminder) for eggs on menu. ,

Found a towel wrapped around the y type valve on the mop sink faucet and hose leading to the chemical dispenser. To correct repair leak.

To correct evaluate cleaning schedule, ensure all noted items are cleaned and cleaned on a regular schedule.

(Critical) Used a chlorine test kit and found no chlorine sanitizer residual in the dishwashing machine. To correct either provide a sanitizing step after washing in the dishwashing machine or use the 3 compartment sink to wash, rinse, and sanitize dishes until the dishwashing machine is repaired. , used a chlorine test kit and found no chlorine sanitizer residual in the dishwashing machine. Asked employee how is ice tea dispenser cleaned. Employee replied spout is removed and tea dispenser and spout (not taken apart) is placed in the dishwashing machine. To correct follow manufacturers directions and properly clean ice tea dispenser, that is spout needs to be taken apart and thoroughly cleaned.
