(Critical) Found time/temperature control for safety (tcs) food, opened gallons of 2% milk 57°F, whole milk 49°F, and skim milk 49°F on the front counter above cooler. Pic corrected by discarding milk and has been instructed to keep milk in cooler at 41°F or have a written time policy in place.
Found the floor under equipment, edges and corners soiled with accumulation and build up. To correct thoroughly clean floor and keep clean.
Found numerous cases of single use utensils stored on floor just outside of the storage room. To correct store utensils on shelf at least 6 inches above the floor.
Found ice stored in a covered storage container on the left drain board of the 3 compartment sink. To correct discontinue storing ice in a storage container, as ice should not be stored in a container that is not self draining.
Found wiping cloth stored on the side of the red sanitizing bucket and wiping cloth not submerged under sanitizer in front counter by the espresso machine. To correct store wiping cloths under sanitizer.
Found a bag of water softener salt stored on the floor under the back counter and cabinet area. To correct store food supplies on shelf at least 6 inches off the floor.
Found water pooling on the floor under the espresso machine area. To correct locate source of leak, repair leak and any damaged surfaces.
Found wiping cloths in sanitizing pan not completely submerged under sanitizer. To correct store wiping cloths completely submerged under sanitizer.
(Critical) Again observed the interior surfaces of the whipped cream dispensing spouts to be unclean after washing them. Corrected by disassembling them and cleaning them in the three compartment sink. Ensure staff are taking these spouts apart for the cleaning process
Two door cooler under the espresso station: observed the right hand door gasket to be torn. Provide a replacement gasket to be smooth and easily cleanable
A pail of sanitizer for wiping cloths was found to be too weak. Corrected by providing fresh sanitizer at 300 ppm
The following areas are in need of cleaning: the area behind the water heater and the floor sink for the ice machine drains
(Critical) Again, observed the interior surfaces of the whipped cream dispenser spouts to be unclean after washing. These items were on the clean dish drain board of the dish washing sink. Corrected by discarding these spouts and providing new spouts. Ensure staff are disassembling these spouts and using the available bottle brush to thoroughly clean these interior food contact surfaces
Observed excessive dust accumulation on the ice machine compressor coil. Have this dust removed to ensure efficient operation of this unit
Nearly all out of the way floor areas (including drains) of this facility (main kiosk and the back dry goods storage area) have debris build-up. Some areas are retaining debris and moisture that can lead to a fly breeding area. Thoroughly clean these areas and maintain them clean on a more regular basis
(Critical) Observed both of the recently cleaned whipped cream dispensing spouts to be unclean (old cream residue present). Ensure staff fully disassemble these devices, soak and thoroughly wash this equipment (small bottle brushes are likely needed for this task)
(Critical) A rinse-o-matic blender rinser was added to the sink located between the espresso machine station and the ice bin. The water lines do not contain the required back-flow preventer. Provide an asse 1024 back-flow preventer (double check valve) on the water lines leading to the blender rinser. At the end of each night, this rinse-o-matic unit needs to be disconnected and the pressure release from the faucet where it connects
The floor sink located under the ice making machine is overdue for cleaning. This basin is located in a difficult to reach area. When the ice machine is removed for service this drain basin needs to be fully cleaned. Investigate the possibility of cutting in a door to the back side of the kiosk (right, rear side of the ice machine when facing the ice machine) to gain better access to the floor sink for regular cleaning
The ice machine compressor coil is visible through the louvered vent located adjacent to the pump syrups at the back counter. This coil has significant dust accumulation on it. Remove dust to facilitate better air flow for this unit. You may wish to also check the cleanliness of the refrigerator compressor coils at the same time this work is performed
Observed multiple boxes of single service food containers stored on the floor in the back store room. Provide an additional dunnage rack or storage shelf as needed to keep all items off of the floor
Observed the clear, braided hose from the water treatment system located above the new ice machine to not be air gapped above the floor sink discharge location. Provide a way to secure this drain line to provide a minimum of a 1" air gap. Correct w/in 20 days
(Critical) Observed a number of small birds (appear to be sparrows) present in the center of the mall where this shop is located. Take necessary steps to eliminate these pests
(Critical) Observed three drain lines (one black one from the water softener and two from water filtration systems (one clear hose and the other is a white braided hose) that do not terminate with an air gap above the floor sinks they drain into). Trim and secure these drain lines as needed to provide the required air gap
Observed the right hand door gasket on the two door upright cooler in the back storage room to be torn. Provide a replacement door gasket
A pump for a chocolate syrup. Corrected by sending the pump to be washed
The following areas are in need of cleaning at this time and more frequently in the future: the floor under the three compartment sink, and the floor sink serving the ice making machine (this floor sink is difficult to get to as the two door low-boy cooler must be removed to gain access to it).
Observed the wet mop head to be resting on the bottom of the mop sink in the back storage room. Locate your mop handle and hang up the wet mop head above the mop sink to facilitate faster drying
Observed a bag of napkins stored on the floor in a cabinet at the customer self service station (access from the outside of the shop). Provided a way to elevate all single service items off of the floor for protection

Observed gaps where the floor meets the wall in multiple locations (opening for the ice bin, under the three compartment sink, etc. ), as well as some poorly sealed seams on the counters/back splash areas. After a thoroughly cleaning and drying these joints need to be sealed to prevent moisture and debris build-up.
Observed debris build-up (some of it moist) in the following areas that need regular cleaning: the floor sink located in the base cabinet below the dish washing basins, the floor mats, and the floor drain in the middle of the kiosk
Again observed a bird(s) in the center of the mall where your food service facility and tables/chairs for your customers are located. Take any/all necessary means to eliminate and prevent future access of birds to the interior of the facility
Observed the ice bin drain lines (flexible hose) to not slope from the bin outlet to the discharge point. Provide rigid drain pipe for this drain (must maintain required air gap (equal to 2 x the discharge pipe diameter)). This may necessitate adding spacers to elevate the ice bins to a higher elevation and/or removing one section of the tile base coving so a hole can be bored through the adjacent cabinet at a lower elevation allowing proper slope to the drain pipe
Observed colored tape wrapped around the handles of the metal spoons for the steaming pitchers. This tape is not a smooth and easily cleanable surface--eliminate this tape
Observed small gaps between the panels of glass on the new pastry case (where the front panel mates with the side panels). Please seal this gap with a silicone sealant
Observed several birds in the center court of the mall. Take necessary steps to eliminate these pests in order to protect exposed food and equipment
