Pic states cooler has been repaired and working properly. Violation corrected. On follow up inspection found covered carafes of- half and half 39°F, nonfat milk 39°F, and 2% milk 39°F on shelf in the 2 door reach in cooler. Pic states they chill carafes prior to filling carafes, and store them in cooler until needed. Violation corrected.
(Critical) Found time/temperature control for safety (tcs) food, an opened carton of half and half 50°F, and whipping cream 51°F in the 2 door under the counter cooler, in the front counter area. Pic states cooler was just cleaned, but tcs food should not have risen 10 degrees in a short period of time. Pic temporarily corrected by discarding half and half and whipping cream. To correct monitor and ensure tcs food is held at 41°F, do not use cooler to hold tcs food if cooler can not keep food at 41°F, and repair/replace cooler to hold tcs food at 41°F if needed. Found tcs food, 2 covered carafes of nonfat milk 47°F and 51°F, in the back kitchen 2 door reach in cooler. Note: other items in this cooler was at 41°F and lower. Pic states carafes were filled this morning. Pic temporarily corrected by discarding nonfat milk in carafes, has been instructed to monitor and ensure tcs food is held at 41°F, and to review carafe filling procedures. ,

(Critical) Found time/temperature control for safety (tcs) food, carafes of half and half and skim milk on a time control policy, without time control marks or timer to indicate time tcs food was removed from temperature control. To correct have written time control policy available, follow time control policy and either have time control marks or use timer not to exceed 4 hours. Pic temporarily corrected by placing carafes on a timer set for 2 hours. This is a repeated violation and a risk control plan must be submitted. ,
Found a gap along the floor and dining room side door. To correct eliminate gap.
Found several bags of water softener salt stored on the floor by the water softener. To correct store salt on shelf at least 6 inches off the floor.
Found no waste basket by the handsink in the back kitchen area. To correct provide waste basket.
Observed the presence of flies in and around the floor drain in the front counter area by the espresso machines. To correct keep areas clean and dry, in good repair, and continue to work with pest control company to control flies.
Found no antichoking poster in the kitchen wall. To correct provide and post antichoking poster on kitchen wall.
(Critical) The dairy products out on the counter for customer self service were not provided with discard time labels, nor was a timer available for reminding staff to switch out these carafes and dispose remaining dairy product at intervals not to exceed four hours. Corrected by providing discard time labels.
(Critical) 1) observed a 1/2 gallon of milk being stored on the back counter upon arrival. An employee relocated this milk to the two door under counter cooler along the back wall. A short while later the temperature was taken of this milk. The milk was found to be at 53 to 54f. Corrected by discarding. Ensure staff keep phfs stored inside the coolers. 2) the back counter two door cooler was only maintaining foods at 45 to 47f. Corrected by discarding any phfs. Maintain phfs at 41f or below
Observed two wiping cloth pails stored on the floor at each end of the front counter. Please provide a low stand with legs or utilize a bottom shelf to keep these pails off of the floor
No trash can was provided at the back kitchen hand washing station. Corrected by relocating one to this area that was down at the office
The two door back counter cooler was only maintaining foods at 45 to 47f. The temperature control dial was moved from a setting of 5 to 6. The gm placed a call for service of this unit. Ensure it holds below 41f prior to using this unit for phfs
(Critical) The air thermometer in the espresso machine station two door cooler is damaged--the glass tube was separated from the background containing the temperature scale. Corrected by providing a replacement thermometer in this cooler
Mop sink faucet: the hot water valve is still leaking when the water is turned on. Repair as needed to eliminate the leak
A gap at the wall is present on the right side of the mop sink basin. Clean and dry this area and provide more sealant to eliminate this gap
The interior of the wheeled trash cart is in need of cleaning now and regularly in the future to eliminate odors and debris/liquid build-up
The low boy two door cooler at the espresso station was operating at a slightly elevated temperature. Ensure partners are not leaving the door open when dispensing milk or other liquids. The temperature adjustment dial was turned to a slightly colder setting (a higher number). Ensure this adjustment is dropping the temperature. If the unit cannot maintain 41f or below, have the unit adjusted further and/or call for service on this unit
The floor under the following equipment is in need of cleaning now and more frequently in the future: under the two adjacent two door upright refrigerators in the back room and under the ice machine
The frame containing the food service license was laying on the counter behind the display case. Relocate this frame to somewhere it is visible to the public
(Critical) A second rinse-o-matic blender rinser was added to the sink located near the espresso machine station. The water line does not contain the required back-flow preventer. Provide an asse 1024 back-flow preventer (double check valve) on the water line leading to the blender rinser. At the end of each night, this rinse-o-matic unit needs to be disconnected and the pressure release from the faucet where it connects
The following items are in need of repair to eliminate water leaks: 1) the hot water valve on the mop sink faucet (rebuild/replace valve as needed); 2) the pvc drain lines for the ice machine and bin in the back kitchen have couplers that have not been solvent welded and come apart too easily--repair/replace these drain lines to be water-tight (be sure to maintain the required air gaps at their termination above the flood rim of the floor sink)
Pastry case: observed frosting stuck to the interior upper frame and some spilled food/crumbs. Please clean the interior of this case within one day

Observed what appears to be a small leak at the drain line connection on the barista beverage station ice holding bin (located inside the base cabinet)
Observed the wiping cloth sanitizer solution at the barista station to be weak. Corrected by providing a fresh solution. The manager indicated there was a safety issue with a recently installed water softener system so the system was by-passed currently. This may be leading to reduced quat sanitizer concentration from the dispenser. The manager said this system will be replaced/upgraded soon so it could be put back in use
Observed some lingering moisture on the floor and floor to wall juncture in the area of the ice making machine and the three compartment sink. There may be several sources for this moisture. Please monitor this area to determine the source(s) of this moisture and find a way to eliminate/control it so this floor area doesn't become a fly breeding area.
The interior of a white chocolate syrup pump did not get pre-rinsed/flushed prior to washing it. Corrected by disassembling this pump and thoroughly flushing the interior surfaces prior to washing again.
Replace or repair the black trim strips on the top edge of the large back counter ice holding bin. These trim pieces are loose and currently do not prevent moisture from seeping underneath them. Re-install this trim (if needed) using a silicone sealant to hold it in place and to prevent moisture from seeping under it
1) the hvac ceiling vents (especially the one located above the beverage making station) are due for dust removal. 2) the floor and lower wall section in front of and directly adjacent to the back room mop sink are in need of a thorough cleaning. Clean these areas now and place on a regular cleaning interval to prevent excessive build-up
After cleaning and drying provide a properly installed replacement base cove tile at the end of the wall adjacent to the left corner of the mop sink
Provide a small trash can at the back dish washing room hand washing station for the disposal of paper towel, etc
Observed a serving tray of bite size pastries for customers to sample at the customer counter at the cash register without any protection. In the future provide a display cover/dome or sneeze guard, individually wrap the food items or locate them such that customers cannot contaminate them. Corrected by eliminating these foods from this location
