Found tiled coving under the front counter with a missing tiled coving piece which allows cables to run through, but leaves a large gap along the floor, collecting debris. To correct fill tiled coving minimizing hole and accumulation.
Found a large opened box under cooler fan essentially blocking all air flow in the 1 door reach in cooler. Pic corrected by removing box, allowing air to flow in the 1 door reach in cooler.
Found the tiled floor in the back kitchen storage area chipped and stained. To correct repair tile floor and remove stains. Found a large hole where cables are running through the coving under the espresso machine. To correct repair hole to minimize hole.

Observed a few small flies (fruit/vinegar) on the walls near the drive through station. Ensure staff are monitoring the facility on a regular basis for the presence of pests. The most recent pest contractor visit was five days prior. Their report indicated no pest activity.
A few out of the way areas (floors, drain pipes and floor sinks) were observed with some moist debris build-up. Ensure these areas are maintained clean and dry to prevent fly breeding areas
(Critical) A second rinse-o-matic blender rinser was added to the sink located near the espresso machine station. The water line does not contain the required back-flow preventer. Provide an asse 1024 back-flow preventer (double check valve) on the water line leading to the blender rinser. At the end of each night, this rinse-o-matic unit needs to be disconnected and the pressure release from the faucet where it connects
Observed some starbucks garbage on the ground around the shared trash dumpster. Corrected by an employee cleaning up the starbucks garbage
Observed one of the sliding doors to be missing on the trash dumpster. Contact the solid waste hauling company or the property manager to have a replacement sliding door provided
A new general manager is in charge of this store and her previous certification was from the city of chicago. The certificate provided does not indicate it is from an accredited, nationally recognized manager level food safety program. The gm is scheduled to take a serv-safe manager food safety course within the next couple weeks. Please provide me with a copy of your certificate once you receive it in the mail
The wash spray arms (top and bottom) of the mechanical dishwashing machine were plugged with debris. Ensure these wash arms are removed daily for back-flushing to remove debris

Observed some areas of deteriorated silicone sealant (around the back counter ice bin, adjacent sink basin, a joint between the counter top pieces and the junction of the counter top to the back splash). Remove all deteriorated sealant, thoroughly clean and dry the area and then apply new sealant to provide a water-proof barrier
Observed some out of the way areas (under the front beverage making counter and where the floor meets the wall in the back counter base cabinets). Clean these areas at this time and more frequently in the future to eliminate this debris build-up

Re-establish the drain line air gap for the discharge pipe of the back counter ice holding bin (vertical air gap = 2x the discharge pipe diameter from the flood rim of the receiving basin to the bottom of the discharge pipe).
(Critical) Observed an unapproved black water hose connected to the potable water faucet at the three compartment sink (the hose is attached via a threaded hose connection while a barbed connection is used at the chemical dispenser). This hose supplies water to the chemical dispenser. To correct this issue install a potable water hose meeting nsf standard 61, or equivalent
Observed three wastewater drain lines (water softener, brine tank and filtration system) that have slipped down at their mounting point above the mop sink. Re-establish the drain line air gap above the flood rim of the mop sink (vertical air gap distance must be at least 2 x the discharge pipe diameter)
Observed the mop head to be stored in the bottom of the mop sink. Please hang up your mops to facilitate faster drying
The mop hanger (a rubber coated pinching device) is worn and no longer holds up a wet mop. Please repair/replace this mop hanger so your wet mop heads can dry faster.
