(Critical) Used a heat strip and found the dishwashing machine not hot water sanitizing dishes. To correct call for repair and use the 3 compartment sink to wash, rinse and sanitize dishes. ,
(Critical) Used a quat test kit and found the sanitizing solution flowing from the quat dispenser at least 500 ppm above manufacturer's recommended range of 150 to 400 ppm. To correct call for repair, and manually mix sanitizer per manufacturer's directions, until quat dispenser is repaired. ,

Found a storage container of sugar on the storage room floor. To correct store sugar on shelf.

Observed the drain line of the dishwashing machine splashing off the side of the drain receptacle onto the floor. To correct adjust drain line to drain into floor drain receptacle.
(Critical) A rinse-o-matic pitcher rinser was added to the sink located at the far east end of the front counter. The water line does not contain the required back-flow preventer. Provide an asse 1024 back-flow preventer (double check valve) on the water line leading to the pitcher rinser. At the end of each night, this rinse-o-matic unit needs to be disconnected so the pressure is released from the faucet where it connects
(Critical) 1) the two, green insulated cambro bulk beverage dispensers were observed to be unclean on the interior surfaces, including the dispensing spouts. These items were cleaned at the three compartment sink--corrected. 2) observed a stainless steel syrup pump that was cleaned and placed on the drying rack above the drain board for clean dishes. This pump is oozing a white syrup from two locations on the bottom of the pump. Ensure staff fully disassemble these pumps for a thorough cleaning. Corrected by re-washing
The drain outlet for the three compartment sink (rinse and sanitize basins) is in need of cleaning now and on a regular basis in the future
1) re-establish the required air gap for the overhead spray nozzle above the flood rim of the three compartment sink. 2) re-establish the drain line air gap for the water softener discharge line above the mop sink it drains into in the back kitchen area. 3) provide a replacement bleed-off tip for the end of the eco-lab side-kick that is installed on the mop sink faucet
Observed several round openings in the counter tops that need covers to prevent debris from falling into the base cabinets below. Provide covers as needed
(Critical) The three drain lines (two black and one clear flexible hoses) associated with the water filtration system installed since the last routine inspection are no air gapped above the mop sink. Provide brackets/strapping as needed to provide an air gap (1" minimum) for these drain lines above the flood rim of the mop sink basin. ,

(Critical) Observed some small flies (likely fruit/vinegar flies) at the front counter beverage making area and a lot of these flies in the basement trash/recycling storage area. Take necessary steps (only chemicals approved for use in a restaurant may be used according to label direction) to maintain your facility clean, dry and in good repair. Contact your pest control company, as needed, for additional service visits. The basement trash and recycling cart storage room had the most pests. Physically prevent access to the materials being recycled, the trash and maintain the carts clean and closed. Utilize commercial floor drying fans to expedite the drying of these floor areas after nightly mopping
Observed most all of the trash/recycling carts to be soiled with debris and liquid build-up in the bottom of them. Thoroughly clean these carts to remove this build-up
The dispensing spouts for the two green insulated cambro beverage dispensers (for coffee/tea) were not disassembled or cleaned after their last use. Ensure these food contact surfaces are removed from the container, disassembled and washed, rinsed & sanitized after use
Observed water damaged wood and base cove tile below the leaking pvc drain pipes at the espresso machine beverage station. Repair/replace as needed to eliminate the damaged wood and provide a water-proof joint where the wall meets the floor
Observed a water leak in the pvc drain piping at the espresso machine beverage station. Repair/replace the damaged section of the drain pipe to be water-tight
Observed one of the garbage trash carts in the basement storage room to have a large crack in the bottom of it. Evaluate all of your trash/recycling carts the next time they are empty and replace any that are not leak-proof
Observed debris plugging some of the water outlets for the dish washing machine arms. These top and bottom arms were removed and sprayed out to removed debris. Ensure staff are reminded to remove and flush these arms out nightly
Observed a few out of the way areas (primarily in the beverage making area where the walls meet the floor and around legs of equipment) to be in need of cleaning now and on a regular basis to remove debris build-up

(Critical) Observed sandwich utensils stored in a painted metal container that is not easily cleanable due to its construction, and that has some paint peeling. These divided containers usually have removable plastic inserts that are smooth and can be washed frequently. Provide replacement inserts
Observed the interiors of the trash and recycling wheeled carts to have spilled food and liquids in the bottom of them. Maintain these carts clean and dry to prevent the attraction/breeding of flies
1) observed the following areas to be in need of sealing to prevent moisture intrusion: floor to the base cove tile by the two door low-boy cooler on the back counter of the beverage station, the below the patched in base cove tile located under the back dish washing room hand sink, the counter around the perimeter of the back counter ice holding bin, and a seam in the back counter top. Maintaining a good seal in these areas will prevent moisture and debris from becoming a problem. 2) observed two ceiling tiles to be missing above some of the refrigeration units in the back room. Provide ceiling tiles
Observed several wiping towel containers to be too weak (<150 ppm). Corrected by providing fresh sanitizer at 200 ppm
Observed several out of the way areas that are in need of cleaning at this time and more frequently in the future: most of the floor to wall junctures by the main beverage station; the area around the mop sink (behind the water softener, and around the insulated water supply pipes), and at the floor to wall junction under the back room hand washing sink. Maintain these locations clean and dry to prevent a pest breeding/food source area
The following items are not being cleaned daily to eliminate debris build-up: the dispensing spouts on the coffee urns, green cambro beverage dispensers, and the dish washing machine washing arms. Please maintain these areas clean
Observed the mop head to be resting in the bottom of the mop sink. Please hang up your mop to facilitate quicker drying
The female threads (plastic) for the upper wash/rinse arms for the dish washing machine were previously damaged/cross-threaded. The upper arms could not be removed by hand--pliers were needed to remove these arms. Have your equipment repair contractor order and install this threaded plastic part so staff can remove the arms for periodic cleaning.
Observed one of the light bulbs in the pastry case (left side from employee side) to be scorched and as a result the plastic coating is damaged. Install a replacement bulb with the shatter-proof coating intact to prevent glass shards from contaminating food or the pastry cabinet
Observed the water spout at the hand washing sink at the back counter to be leaking water. Also, observed a water supply leak from the toilet flush plumbing in the basement. Repair/replace these items as needed to eliminate the leaks.
Observed the door gaskets to be split on the two door low-boy cooler located at the blending station. Please order and install new gaskets
Observed excessive dust accumulation located below the bottom louvered vent of the grab and go customer cooler. Please clean this area now and on a more frequent schedule in the future
Observed old hand soap residue from a previously leaky soap dispenser in the base cabinet for the hand sink near the ovens. Please clean the plumbing pipes and the under side of the counter to remove this soap
