(Critical) The dish washing machine was not sanitizing dishes with chlorine. It was determined that the chlorine source had become too weak. Corrected by opening a fresh container of dish washing machine sanitizer, and priming the system to obtain 100 ppm chlorine. In the future, do not purchase your dish washing machine sanitizer in a five gallon pail. Switch to either a 2. 5 gallon or one gallon container so it gets used up faster before it goes too weak

(Critical) Ensure staff are testing the sanitizer residual in the dish washing machine rinse water at least once per day (consider one time per shift) to ensure the machine is working properly. Corrected by discussing this requirement with the manager and by verifying the sanitizer level with test papers

(Critical) The one door under counter refrigerator did not have an air thermometer inside of it. A calibrated probe thermometer was placed inside for an immediate correction. Purchase and install an air thermometer in this unit

(Critical) Observed the following potentially hazardous foods to be cooling in three refrigerators in large quantities and two of them were covered: cooked falafel, rice, sliced chicken, and falafel mix. Corrected by uncovering these foods and placing them into at least two containers

(Critical) Observed an employee not wash his hands upon beginning his work day prior to putting on food service gloves. Corrected by having the employee remove the gloves and wash his hands

The wet mop head was found resting in the bottom of the mop bucket. Provide a way to hang your wet mop head above the mop bucket or mop sink to allow for faster drying of the mop

Observed a few cardboard boxes being used to store cups and lids under the front counter. These boxes are becoming damaged and soiled. Eliminate the use of cardboard for this purpose as it is not cleanable, absorbs moisture and can be a hiding area for pests. Provide a container or divider that is rigid, smooth, easily cleanable and doesn't absorb moisture

Office/dry storage room: observed multiple aluminum foil food trays stored upright. Either invert these trays or store them in a covered container or in a bag for protection

(Critical) Upon arrival, a kitchen knife was stored in the basin of the cooking area hand washing sink. Corrected by taking the knife to the dish washing sink

(Critical) The three door prep cooler was operating at slightly elevated temperatures. Have this unit repaired/adjusted as needed to maintain foods at 41f and below

(Critical) Observed a one dozen of personal raw, in-shell eggs stored on the top shelf of the two door upright refrigerator. Corrected by relocating to another refrigerator on top of containers with raw chicken

(Critical) 1) observed the small portion of gyro meat in the hot holding unit to be at 109 to 114f. Corrected by discarding. 2) observed the following foods in the sandwich prep cooler to be at elevated temperatures: cooked potatoes (52f), hummus (50f), sliced tomatoes and feta cheese (44f). Cool these food items down to 41f asap.

(Critical) Observed the following items to be unclean: a metal soup pot, two deep metal 1/2 pans, a plastic colander, and the interior surfaces of the dispensing spout for the lemonade container. Corrected by washing these containers

Observed the three door prep cooler to be operating at an elevated temperature. It appears the temperature dial needed adjustment to maintain foods at 41f and below. This dial was turned to a colder setting and the compressor turned on to bring down the temperature. Closely monitor this unit and adjust/repair as needed

Observed the pails of sanitizer for the wiping cloths to be too weak. Corrected by providing fresh sanitizer at 100 ppm chlorine

1) observed the handle on the kitchen aid food processor to be damaged. Provide a replacement blending base. 2) observed an empty bulk drink vessel (purchased used, but has not been placed into service at this store) located on the back kitchen storage shelf to have some damage to the base and spout connection area. Discard this container and provide a replacement unit if you desire

Observed the drain in the base cabinet under the espresso machine to be in need of cleaning now and more frequently in the future

(Critical) Observed sliced chicken (cooked the previous day) in the hot holding unit that was just removed from the stove for reheating to be at 132 to 138f. Corrected by reheating to 165f+.

Observed some gaps at the bottom and lower latching side of the back delivery door. Please seal these gaps to prevent pest entry and heat loss

Observed two of the three wiping cloth pails at the front kitchen area to be registering no chlorine. Corrected by providing a fresh chlorine solution (+/- 75 ppm) in these two container

The door gaskets on the two door upright cooler are split and in need of replacement to keep your cold air in the unit and to provide a smooth & cleanable surface

(Critical) Observed the falafel making tool to be unclean. Please ensure this tool is thoroughly washed, rinsed and sanitized at least every four hours when in use during the day. Corrected by cleaning and sanitizing this tool

Back kitchen area: observed the following areas to be unclean--the area under the dish washing machine and the area behind the floor sink that receives water from the hand sink and the three compartment sink. Please clean these areas today and maintain them clean

Observed no paper towel in the dispenser at the back kitchen hand washing sink. Temporarily corrected by relocating a small amount of paper towel from another dispenser. The owner indicated cintas is to bring more towels today

Observed several water outlet ports in the top and bottom wash arms of the dish washing machine to be plugged with debris. Corrected by removing these arms and unplugging/cleaning them
