(Critical) Follow up site visit 6/15/15: facility has installed a new salad bar cooler in dining room. Unit is nsf approved (vollrath model# r38960) and has approved sneeze guards installed. Unit was cold holding at 34f at time of site visit. Drain line is self contained with built in hose/drain valve installed in back of unit. Ok to put new salad bar cooler unit into service
(Critical) Follow up site visit 5/21/15: pic stated that salad bar unit was not functioning correctly so management has decided to take this unit of service and not replace it at this time. All salads are now prepped in kitchen, no salad bar in dining room at this time. Pic stated that facility may get a new salad bar unit in future. Please call me before placing any new units into service. Violation corrected
(Critical) Observed insert pans of rte lettuce, spinach, shredded cheese and hard boiled eggs in dining room salad bar with internal food temperatures of 53-59f. Unit appears to be working correctly, but all insert pans were double panned at time of inspection which might be preventing food from reaching proper temperatures. Pic temporarily corrected violation by discarding all above mentioned food items at time of inspection. Pic also called heating/cooling tech at time of inspection to evaluate unit and see if the unit needs repaired or if the double panning is the cause of the problem. Do not use this unit to store phf until repaired and then inspected by the washtenaw county health dept. I will follow up on this violation to ensure compliance. Correct within 10 days. ,
Observed vent filters above char grill in cooking area with an accumulation of grease and debris (grease starting to pool and drip in hood). To correct, please clean now and on a more frequent schedule
Observed microwave oven door handle at end of cooking line in poor repair (door handle is missing and staff have made a handle out of duct tape). Duct tape is not an approved repair surface as it can accumulate bacteria. To correct, please repair or replace microwave oven door handle as soon as possible so all surfaces are smooth and cleanable
Observed floor in dry storage area in poor repair (several floor tiles are chipped and/or broken). To correct. Please replace all damaged floor tiles in dry storage area as soon as possible. , observed mop sink wall in poor repair (wall is buckled and does not provide a good seal to mop sink. As a result, water can accumulate in wall next to mop sink. To correct, please repair or replace wall in mop sink area as soon as possible
Observed the following areas in need of cleaning: *floors under fryers and prep units in cooking area (accumulation of food debris). *floor and floor drain behind pop syrup containers adjacent to keg cooler (accumulation of debris). *walls behind char grill and near edge of hood filters in cooking area (accumulation of grease and debris). To correct, please clean all these areas now and on a more frequent schedule
Observed open trash and grease dumpster lids in back of building. All dumpster lids and doors must be shut after use to prevent entry of unwanted pests, insects and birds
Observed wet mop stored in mop sink in back kitchen area. All mops must be hung up to dry after use to allow for proper air drying

Observed prep top unit lid in cooking area in poor repair (lid repaired with duct tape). Cannot use duct tape as a repair surface as it can accumulate bacteria. Observed several clean forks in utensil bin at server station in dining room stored with food contact portion of utensil pointing up. All clean tableware must be stored so the food contact portion of utensil is pointing down (and handles are up) to help prevent contamination of utensil (from employee hands)
Observed missing light shield and end caps on overhead light above food containers in dining room salad bar. To correct, please provide as soon as possible

Observed ceiling in back kitchen area (near vent hood) in poor repair (paint is cracked/chipped/flaking). No food located under chipped paint. (Critical) Observed gasoline container laying on its side on top of a bucket in utility room 3-4°Feet from hot water heater. Also observed container of lighter fluid on shelf +/- 1°Foot from hot water heater creating a possible safety/fire hazard. All chemicals must be used as per manufacturers directions and not stored near an open heat source. To correct, need to relocate gasoline and lighter fluid away from hot water heater immediately. (Critical) Observed several flats of raw shell eggs stored above packages of rte shredded cheeses in wic. All raw animal products must be stored below and away from rte foods at all times to help prevent potential cross contamination. Observed old equipment, empty buckets, cleaning supplies, tools and general clutter on floor in mop sink room. To correct, please organize mop sink room and remove all unnecessary items as soon as possible.
Observed contractor drilling and installing new lights above salad prep unit cooler in kitchen without removing food items from cooler. Lid on top portion of cooler was shut and minimal shavings were observed on equipment, but food cannot be stored in any area which may be exposed to potential contamination. No visible contamination observed in food insert pans in cooler. Pic corrected by covering prep unit cooler with vinyl tarp at time of inspection.
(Critical) Observed facility not sanitizing glasses properly as i measured 0 ppm of chlorine in bar dishmachine. To correct, need to be sanitizing at 50-100ppm at all times. Pic corrected by replacing sanitizer bucket and priming dishmachine at time of inspection. Violation corrected.
