(Critical) There was no hot water available to the store when i arrived. The pilot light was out on the hot water tank. Spoke with the chef, herb, and he said they have had this issue for about a week now. You must have the hot water tank repaired by a licensed technician and have the tech submit a written report detailing the problem and what has been done to repair the problem. This must be faxed over by this friday, april 8, 2011. Pilot has been ignited and hot water is up to 158*f coming out of faucets now. Ok to operate, after a brief shutdown. The bar handsink does not have soap or paper towels stocked at it, making safe handwashing impossible. You must always have handsink stocked. I will be back to verify this.

(Critical) Repeat violation 2 times. Complete and fax in the risk control plan within 10 days. I found 3 unlabelled chemical spray bottles. Pic's must assure staff are labelling with common name of product. I will be back in 2 weeks to verify you are in compliance with this.

(Critical) A raw steak was just set on a rack in the line reach in cooler right next to cheese creating a cross contamination risk. Keep foods in covered containers and raw foods on bottom shelf. Corrected by discarding the steak, sanitizing the rack.

(Critical) Date marking is not in compliance. The following items were not date marked: 3 opened creams at the bar, opened whipped cream at the bar, deli meats on the line in the reach in cooler, cole slaw, staff must be held accountable to properly date mark. I will be back in 2 weeks to verify date marking system is in compliance. Product was discarded.

(Critical) The following food contact items were observed dirty: the can opener blade, the meat slicer, the mixers, a produce peeler, and the bowls under the hot well on the line. You must be properly washing, rinsing and sanitizing food contact surfaces after uses. Corrected by doing such while i was present.

You must be covering all foods in sealed containers. Obtain food grade storage containers with lids and begin to use them. I will be back in 2 weeks to verify this is being done.

Put up a soap and paper towel dispenser in the male restroom.

Mop was left in dirty mop water overnight. Staff need to empty mop water, rinse out buckets and mop heads and hang up mops after uses to faciltate rapid drying.

Observed several chipped spatulas. Discard damaged utensils. Corrected.

Replace the damaged microwave oven on the line. Plastic is melting inside the unit.

Repair the leak inside the front reach in cooler, the pepsi cooler on the expo line, and in the walk in freezer.

You have lots of "clutter" stored on the floor and in the back hallways and unused equipment. You must be getting rid of all the stuff that is not a part of your current business. All stuff needs to be up and off the floors and thoroughly organize things. I will be back in 2 weeks to verify this has been done.

Move the soap dispenser so it's not over the exposed tortilla chips on the line.

This store is not being kept cleaned and organized on a daily basis. The following areas were observed dirty: the main line coolers are dirty on the outside, inside and on the cold well, the storage shelves are all dirty, the cooking equipment on the line is very dirty, the floors are dirty under all the equipment and shelves and in the walk in cooler/freezer, the exhaust hoods, upright freezer, dirty cloth towels were left all over, the utensil storage bins are dirty, floors in hard to reach areas at the bar. You need to develop and enforce a daily cleaning schedule of your store. I will be back to verify this is being done daily.

Found original cans used to store food in numerous coolers. To correct once cans are opened. Store unused food in commercial grade storage containers.

Found employee working in the cooking/prep area without hat/hair restraint. To correct provide hat/hair restraint, and remind employees to wear them.

Observed the presence of flies in the back kitchen by the ice machine, and in the bar area ice bin area. To correct keep areas clean and dry, in good repair, and continue pest control treatments.

Found the trim around the door handle of the 2 door reach in freezer broken. Found the blade of the can opener worn. Found the door handle of the back door broken, and door misaligned. To correct repair/replace noted items.

Found the shelves in the pizza prep area soiled with flour and accumulation. Found the drip trays of range on the cooking line soiled with accumulation and drips. Found the interior bottom of coolers and freezers soiled with accumulation. Found the interior walls of the walk in cooler soiled. Found broken glass and debris on the bottom of the glass chiller. To correct clean as soon as possible, and clean on a more frequent schedule.

Found the drain line of the steam table broken and in pieces on the shelf of the steam table. To correct repair drain line and ensure the drain line has a 1 inch air gap above the rim of the floor drain.

Found in use cooking utensils stored between equipment on the cooking line. To correct store in use utensils in a more protected manner. Found ice scoop handle laying on top of ice. To correct store ice scoop with handle stored up.

Found no handwashing sign in the mens and ladies public restrooms, and in the bar handsink area. To correct provide signs reminding employees to wash their hands at all locations where they may wash their hands.

Found a gap along the back door and floor. To correct repair door sweep and eliminate gap.

Found various old and broken equipment, and other items in the back hallway storage area. Found old food (dry food products) passed it's best by date in the back storage room. Pic states these items are not used in food operations. To correct clean areas and remove/discard items no longer needed or used in operations.

Found several wiping cloths stored on counter tops in the cooking/prep area. To correct store wiping cloths submerged under sanitizer in sanitizing buckets.

Found the floor (under equipment, edges and corners) on the cooking/prep area soiled with accumulation and build up. To correct throughly clean floors and clean on a more frequent schedule.

Found facility not throughly cleaning the beer tower dispenser nozzles. To correct follow manufacturers directions and clean beer tower dispensers and nozzles on a schedule to preclude accumulation.

Found the door of the mens public restroom propped open. To correct remove door stopper, and keep door closed.

(Critical) Found the common shared drain line of the beer cooler in the bar without air gap. Pic corrected by discarding sausage and beef, and has been instructed not to use this side of the cooler, until repaired to hold tcs at 41°F. Noted at time of inspection that fan on left side of cooler was not working. Found the hood filters and edges of cooking hood in the kitchen soiled with greasy build up. Found the areas surrounding the battering shelf in the kitchen soiled with flour and bread crumbs. Found the interior of the microwave in the kitchen soiled with accumulation and build up. Found all coolers and freezers in the kitchen , (lids and doors, door gaskets and door handles, interiors surfaces) soiled with food debris, build up, and accumulation. Found the bottom of the mug chiller in the bar soiled with broken glass and debris. To correct set up cleaning schedule and ensure that all items are cleaned on a regular schedule to preclude accumulation.

Found the bottom portion, left side of the 3 door prep top cooler on the cooking line not holdin tcs at 41°F. To correct repair/replace cooler to hold tcs at 41°F. Note: found fan on left side of the cooler not working. Found the bottom edge of the beer keg cooler damaged and warped. To correct repair/replace edge of beer keg cooler. Found the door handle of the back door missing. To correct replace door handle. Found the paper towel dispenser through out facility not working, but with rolls of paper towel on top of dispensers. To correct replace paper towel dispensers as soon as possible.

Found in use cooking utensils stored in between cooking equipment, and tongs stored on oven handles. To correct store in use utensils in a more protected manner. Found glass mugs in orignal cases stored in the walk in freezer. Pic states glass mugs have been washed, placed in orignal cases, and placed in the walk in freezer to used as back up glasses. To correct store cleaned mugs in a more protected manner such as in food grade storage containers or covered dish racks.

Observed employee working in the cooking/prep area not wearing hat/hair restraint. To correct provide hat/hair restraint, and remind employees to wear them.

Found particle board used as shelving in the back bottle return storage area, and food a wood framed screen use to secure liquor in the walk in cooler. To correct either replace particle board and wood frame with a material that is smooth and easy to clean or seal/paint wood. Found food left in cans for storage after opening in the 2 door prep top cooler. To correct after opening cans, store left over food in commercial grade food containers.

Found the floor in the entire faciltity (kitchen, bar, wait station, and storage areas) soiled with accumulation of debris. To correct throughly clean as soon as possible, and clean on a more frequent schedule.

Found single use "bomb glasses" being washed in the glass washer and being re-used. To correct discontinue washing, and re-using "bomb glasses", and discard after each use.

Found cooked corned beef in original plastic wrapping with edges cut open in the walk in cooler shelf. Pic states they cook corned beef in it's packaging. To correct pic has been instructed not to cook corned beef in it's original packaging unless manufacturers states that corned beef may be cooked in this packaging.

Found the floor under the new beer keg cooler missing tile and not smooth. To correct repair/replace floor to be smooth and easy to clean. Found the floor surrounding the bar entrance with uneven surfaces. To correct repair/replace floor to be smooth and easy to clean. Found ceiling tile missing in the mens public restroom. Found the back storage area and the side storage areas with numerous old and unused utensils and equipment. To correct clean and organize storage areas and remove unnecessary items

Found bunches of lettuce stored on the bottom of the 2 door prep top cooler. To correct store lettuce in a more protected manner, such as in a food container.

Found the ground around the used grease collection dumpster soiled with grease. To correct clean greasy areas and work to keep areas clean.

Found several fruit garnish containers stored in the ice bins in the bar. To correct discontinue storing fruit garnish containers in ice bin where ice is used for customers drinks.
