(Critical) Observed bags of cut rte lettuce and spinach on counter in prep area with internal food temperatures of 59-61f. Lettuce and spinach have been on counter for less than 30 minutes per pic. All phf must be stored at 41f or below at all times. To correct, please put lettuce and spinach back in wic after initial food prep has finished. Observed all single service knives stored all jumbled together in self serve area in cafeteria (spoons and forks stored properly). All utensils must be stored so the handles all face the same direction and the food contact portion of utensil is protected from potential sources of contamination (like kids hands)

(Critical) Observed all pre-portioned housemade chicken salads in self serve coolers on serve line with no datemarking labels. All potentially hazardous housemade rte foods (including deli meats and housemade salads) must have a 7 day consume by datemark label on container (day 1 is prep/slice date). (Critical) Observed freshly made rte housemade salads (pasta, tuna, chicken) in cold tables on serve line with internal food temps of 55-78f. Insert pans are being displayed at an angle and are not in contact with cold holding surfaces in unit. All phf must be cold held at 41f or below at all times. Will need to keep pans stored completely in cold holding unit or supplement well with ice to maintain proper temps. Salads were prepped less than an hour ago per pic, so pic corrected by rapidly chilling all salads in wif at time of inspection to 41f and filling cold wells with ice at time of inspection. Critical corrected.

(Critical) Observed all rte hamburgers, chicken patties and burritos in hot holding unit on serve line with an internal food temp of 99-115f. Unit is working properly, but appears foods were sitting out too long after cooking earlier in morning. All potentially hazardous rte foods must be held at 135f or more at all times. Pic indicated that facility may explore time control policy for burgers and chicken patties. Facility not able to properly wash, rinse and sanitize in automatic dishmachine as i observed no soap in automatic soap dispenser (no backup soap containers available in entire facility). This item must corrected immediately. Pic stated that soap is scheduled to come in tomorrow and will be temporarily washing dishes in 3 compt sink with liquid dish soap until chemical shipment arrives tomorrow. I will follow up on this violation to ensure compliance.

(Critical) Dented can found in can storage rack. Correct by discarding or removing from storage rack for return. Store return can in separate area which is labeled for return dented cans. Corrected, dented cans were removed from storage rack

Reach-in-cooler at 60 degrees f. . No phf stored in cooler at time of inspection , but is used to store phf sometimes. Correct by repairing refrigeration unit to maintain foods at 41 degrees f or below. Johnson control was onsite at time of inspection and was looking into the problem
