Observed the floor under the main cook line in the kitchen a heavy accumulation of food debris and grease. Correct by cleaning now and on a more frequent basis to prevent the accumulation of food debris and grease. Improvement has been made since the last inspection in cleaning the following areas: 1. Under the upright freezer near the pass through window 2. Behind the prep top coolers on the main make line
(Critical) Observed dual use of handsinks at the following locations: 1. At the handsink behind the front service counter observed a coffee pot and a bucket used for wiping cloths stored in the sink 2. At the handsink in the main kitchen next to the upright freezer observed a toothbrush and toothpaste stored on the paper towel dispenser above the sink. Also, observed a bottle of mouthwash stored on the adjacent shelf. Corrected during the inspection by reminding the pic to only use handsinks in the facility for washing hands
(Critical) In the walk in cooler, observed the following raw animal food items cooked using a non continuous cooking process not properly labeled or segregated from rte foods: 1. Breakfast sausage 2. Chicken breast 3. Hamburger patties pic corrected during the inspection by properly labeling the food items and segregating them from rte foods
(Critical) At the main prep line observed four cardboard cartons (30 eggs per carton x four cartons=120 eggs) of raw shell eggs stored out of refrigeration. The internal temperature of one of the eggs in the top carton was 62 degrees f. Other eggs on different cartons were approximately 45 degrees f. The pic indicated the eggs were out for the lunch rush. However, the eggs were found at 2:30 pm, well after the end of the lunch rush. Item corrected during inspection by returning the 45 degree f. Eggs to the walk in cooler. The 62 degree f. Eggs were discarded.
Observed several water stained ceiling tiles throughout the facility. No obvious water was observed leaking from the ceiling. Correct by identifying the source of the water staining and repairing the leak. Replace the water stained tiles. This is a repeat violation. Also, several broken, missing, and badly worn floor tiles were observed in various areas of the facility. Correct replacing the affected floor tiles to provide a smooth and easily cleanable surface

Observed the can opener to have a gouge in the blade. Correct by replacing the blade. This is a repeat violation. The pic indicates the blade has been changed since the last routine inspection. The blade needs to be changed more frequently so it cuts the cans properly
At rear entry door observed gap between bottom of door and threshold. Correct by repairing or replacing door sweep to prevent entry of pests. This is a repeat item. Pic is actively trying to repair the gap. A professional door repair contractor may be needed
In the walk in cooler on the shelves observed uncovered containers of food including fish, hash brown style potatoes, shrimp, cut vegetables, and soup, and three hotel pans of bacon. Also, on the floor of the walk in cooler observed two uncovered hotel pans of fish. Food not in the active cooling process shall be covered at all times to protect it from contamination. Pic corrected during inspection by covering above mentioned items. This is a repeat violation from previous inspections

In the kitchen at the three compartment sink observed a garden hose draining into the rinse compartment. The pic indicates the hose drains ice melt from the roof. This hose must be redirected to an approved receptacle such as the floor sink under the three compartment sink. The pic stated he would correct this item today. The three compartment sink must be properly washed, rinsed, and sanitized before food preparation or warewashing

Found the 3 compartment sink without stoppers for the sink compartments, (note this facility uses a automatic dishwashing machine for daily dishwashing as required) to correct provide stoppers for each sink compartment as required for proper use of the sink for washing, rinsing, and sanitizing solutions. Found the access cover of the mechanical portion of the ice machine removed from the machine. To correct should reinstall the cover on the ice machine mechanical portion at the top section of the unit
Observed missing or stained ceiling tiles located at various locations of the kitchen areas, and public rest room. To correct should schedule repairs to ceiling tiles to provide a cleanable ceiling surface as required

Found the lower shelf of the wait area to be less than six inches above the floor. To correct should adjust the height of the lower shelf to be atleast six inches above the floor to provide access to clean the floor under the shelf

Gravy found cooling improperly in walk-in-cooler. Cooling potentially hazardous food shall be accomplished within 2 hours from 135°F to 70°F and within 4 hours from 70°F to 41°F, using one or more of the following methods listed. Corrected at time of inspection by dividing gravy into two metal containers with approximately 3 inches of gravy in each container uncovered; gravy was found >150°F in plastic container approximately 10 - 11 inches thick. Pic will stir gravy and monitor to assure cooling is accomplished within stated criteria above
