Observed the floors under and the walls behind the main cook line with a heavy accumulation of grease and grime. Correct by cleaning these areas now and on a more frequent basis to prevent the accumulation of heavy grime

(Critical) Observed an employee donning and doffing the same pair of single service gloves several times during the inspection. This activity was occurring in two different locations within the kitchen. Corrected during the inspection by discarding the used pair of gloves and educating the employee with the help of the pic. At the dishwashing machine observed the dishracks to have an accumulation of grime in the bottom of the racks. Correct by degreasing/degriming now and on a more frequent basis to prevent the accumulation of grime.

At the main ice machine for the restaurant observed the ice scoop stored with the handle in the ice. Correct by storing the scoop on a purpose designed holder inside the machine or on a clean protected designated surface outside of the machine

(Critical) Two food processor/choppers: the plastic housings that hold the chopping blades were cracked and allow food debris and moisture to accumulate. Discard these damaged parts and provide replacement units. ,

(Critical) The following ready to eat, potentially hazardous foods were found to be expired in the walk in cooler: cooked tofu (5-21 to 5-27) and white potatoes from a can (5-15 to 5-21). Corrected by discarding these foods. Perform daily checks of your food supplies and purge expired foods

The interior of the ice making machine is in need of cleaning and sanitizing to remove some bacterial slime build-up. Clean this unit now and on a regular basis in the future to maintain it clean (2-4 times per year as needed)

The food safety certification for the owner's son has just expired this month. He recently took the serve safe course and exam, but did not pass. He is currently studying further and will take a re-examination within two months

A non-coated light bulb was installed in the white, one door refrigerator near the cooking area. Corrected by removing this bulb and providing a shatter-proof bulb. Also, in the food transfer window with the heating elements there are two uncoated light bulbs that the glass has come disconnected from the metal base. After unplugging the unit from the power source, use a tool to remove the metal base from bulb socket. Staff do not use the light bulbs any more

Observed a case of chicken stored on the floor of the walk in cooler. Corrected by relocating the chicken to a bottom shelf in the unit used for raw animal foods

The ceiling air vent near the cooking area has grease build-up on it. Maintain this vent clean

(Critical) Walk in cooler: observed several instances of raw animal foods (beef, poultry, seafood, etc. ) stored above / adjacent to ready to eat foods (raw vegetables, tofu, cooked sauces, etc. ). Corrected by relocating the raw foods to shelves lower than the ready to eat foods

(Critical) Multiple potentially hazardous, ready to eat foods (such as cooked sauces, dairy products, tofu, etc. ) were observed without date marking. Ensure these ready to eat foods are provided with dates when prepared/opened

(Critical) Observed a prep employee cutting up green pepper to be used for a salad with bare hands. Corrected by the employee discarding the green pepper and providing food service gloves prior to resuming pepper cutting

The grease trap leaks when water is drained through the wash compartment sink. The owners indicated the company that cleans the exhaust hoods also cleans out the grease trap. A provided sticker indicates they were to be back on 11-5-13. Since they are one week late, call them asap to request a service visit to clean this grease trap. The grease trap is quite old, if the cover needs a new seal have them provide one, otherwise this unit may need to be abandoned and a new grease trap installed

Multiple cardboard boxes are being used for longer term storage in the walk in cooler. Eliminate these cardboard boxes and utilize metal or plastic containers that can be washed when needed

(Critical) Observed three containers of food in the walk in cooler to be expired. Two containers of pork in chili paste that expired on 9-12 and 9-19. Also some fried rice that expired on 9-22. Corrected by discarding these foods

Observed end caps to be missing from the light shield tubes in the dry storage room. Provide end caps to keep glass contained should a bulb break

(Critical) Observed the following foods to be expired: two batches of fried rice (expiring on 3-8 and 3-15), and tapioca pudding that expired on march 3. Corrected by discarding. In the future, make smaller batches of these foods to ensure you sell out of them prior to the end of seven days

(Critical) Observed a +/- 3 inch deep pan of left over brown rice covered in plastic wrap from the previous night to still be at 45 to 46f. Corrected by discarding. In the future, place this amount of rice in two pans so the rice is thinner and leave the rice uncovered or well vented on a top shelf to allow the heat to escape

Observed the cover (inside plastic portion) of the ice machine bin to be cracked. Provide a replacement cover that is smooth and easily cleanable

Observed the walk in cooler to be operating at a slightly elevated temperature. The evaporator coil was recently cleaned. Adjust the thermostat as needed to drop the temperature to 41f and below

(Critical) The process for setting up the three compartment sink is not correct. Currently, staff are adding soap and bleach to the first compartment and then clear water in the middle and last compartment for rinsing. Ensure staff are only adding hot water with soap/detergent in the first basin, only clear rinse water in the middle basin, and only chlorine sanitizer in the last basin. When mixed according to the label directions the dishes must remain in the chlorine sanitizer for two minutes and then be air dried (no further rinsing). Corrected by properly setting up the three compartment sink.

(Critical) Observed a bag of shrimp that an employee was cleaning/prepping earlier to still be setting out on the counter. The employee got involved in some cooking activities and did not put the shrimp back in to the cooler. The shrimp was at 70-72f. Corrected by discarding

Observed multiple cleaned plastic food containers stacked on shelves in the dish room area that were not fully dry prior to storage.

The interior of the dish washing machine and the waste water drain pan and housing for the dish washing machine has old food build-up on it. Thoroughly clean this unit now and as a daily closing chore

Observed the evaporator coil in the walk in cooler to be in need of cleaning so air flow is improved. Also, your contractor should check the coil on the ice making machine

Observed a cardboard box (from coffee) being used in the walk in cooler to thaw small packages of food. The moisture on the outside of the plastic food packages is allowing mold to grow on the box. Corrected by discarding the box. Staff will use rigid, re-useable plastic tub or a metal pan in the future

Observed two cardboard boxes taped together and used as a shelving unit. Eliminate this cardboard and any additional excess cardboard as it is not cleanable and can harbor pests

Dry storage room: observed cases of to-go food containers and some equipment stored on the floor. Provide enough shelving to store all items off of the floor

Observed three containers of lard and two cases of chicken on the floor in the walk in cooler. Re-organize the cooler so all foods are at least 6" above the floor to prevent contamination

Observed the floor in the dry storage room to be over-due for cleaning. Remove all items stored items from this room (including the dunnage racks) so the entire room can be cleaned

Observed some interior surfaces of the ice machine to be due for cleaning at this time. Schedule your contractor to clean this ice machine. All ice must be discarded from the bin prior to cleaning.
