(Critical) Found the overhead spray rinse nozzle in the dishwashing area below the flood rim of the dishwashing counter. To correct provide an air gap of at least 1 inch from the spray rinse nozzle and flood rim of the dishwashing counter.

(Critical) Observed numerous small flies on the post and wall at the end of the bar, and on the wall by the ice bin. Observed small flies in the dining room waitstation wall by the pop machine. Observed small flies in the kitchen beverage waitstation counter and wall, numberous flies on the drain lines and floor drain receptacle area. To correct clean all floor drains and surrounding areas in noted areas, keep facility in good repair, that is eliminate cracks, gaps, and areas where flies may breed, and have pest control company target flies.

(Critical) Found time/temperature control for safety (tcs) food, chicken breast 48°F, chicken wings 46°F, on the bottom shelf of the 2 door sectional reach in cooler by the fryer on the cooking line. Found tcs food, raw shell eggs 49°F, liquid egg whites 47°F, and pooled eggs 49°F, on the bottom shelf of the 2 door sectional reach in cooler by the range on the cooking line. Note: tcs food on top and middle shelves of both coolers were at 41°F, and both coolers had poor air flow as cooler shelves were stuffed with food. Pic temporarily corrected by discarding tcs food above 41°F in both coolers. Pic has been instructed to monitor and ensure tcs food is held at 41°F, and coolers are stocked properly allowing all tcs food in coolers to be held at 41°F or lower. Found tcs food, sausage 51°F, various pasta 46°F, in a cold hold well, in the cook to order station, with ice that did not completely surround containers of tcs food. Pic temporarily corrected by discarding tcs food above 41°F, and has been instructed to completely surround food containers with ice.

Found the floor under equipment, edges and corners in the kitchen and bar soiled with accumulation of debris and build up. To correct thoroughly clean floor and clean on a more frequent schedule.

Found several trays of covered pasta cooling in the walk in cooler. Pic corrected by partially removing plastic wrap allowing heat to escape, and rapidly cool.

Found numerous wet containers on the clean utensil storage shelf. To correct allow containers to air dry, prior to storing and stacking.

Found spoons with handles laying on top of food, in numerous storage containers in kitchen coolers (hummus), and on top of counter tops (granola). To correct store utensils in food, with handles up to minimize contamination. Found in use knife and tongs stored in a bucket of sanitizer on the cooking and prep line. To correct discontinue storing in use utensils in sanitizer, and store on a clean surface or protected location. Found the bottom of ice scoop holder besides the ice machine with pooling water. To correct clean ice scoop holder, as ice scoop should be stored in a clean and dry environment.

Found the hood filters in the back kitchen cooking area soiled with dusty build up. Found the shelves in the produce walk in cooler soiled with build up. To correct clean hood filters and shelves.

Found recently constructed bare wood shelves in the liquor room. To correct paint/seal wool shelves to be nonabsorbent.

Found the floor in the hallway leading to beer keg walk in cooler damaged with uneven surface. To correct repair floor to be durable, smooth and cleanable.

To correct add disclosure to all animal-based foods on menus that can be ordered undercooked, that is asterisk (*) undercooked foods to a footnote stating the items are served raw or undercooked, or contain (or may contain) raw or undercooked ingredients. Example: menu item: *tasty basic burger on the menu: *contains raw or undercooked ingredients. Consuming raw or undercooked meats, poultry, seafood, shellfish, or eggs may increase your risk of foodborne illness. ,

(Critical) Found time/temperature control for safety (tcs) food, milk 48°F, and liquid eggs 52°F in the 1 door under counter cooler in the dining area cooking station. Pic temporarily corrected by discarding tcs food above 41°F and has been instructed not to use this cooler to hold tcs food, until repaired/replaced to hold tcs food at 41°F. ,

(Critical) Found ready to eat (rte), time/temperature control for safety (tcs) food, several opened boxes of desserts, cheese cake, cakes, opened salt cured products (sopressata, abruzzes) without 7 day (day opened + 6 days) datemarks in the 4 door sectional reach in cooler. Found an opened carton of half and half which was passed use by date, and without 7 day datemark in the bar cooler. Pic corrected by providing datemark for desserts, and discarded salt cured products and half and half. Pic has been instructed to monitor and ensure appropriate rte, tcs foods are datemarked. ,

(Critical) Found several spray bottles of cleaning chemicals stored on utensil storage shelves with clean utensils. Found several spray bottles of cleaning chemicals stored on top shelf in the dining room waitstation above tea packets and utensils. Pic corrected by relocating chemical spray bottles to areas below and away from food utensils and food. ,

(Critical) Observed dishwasher handle soiled dishes and then handle clean dishes without washing hands. Pic corrected by having employee wash hands prior to handling clean dishes. Pic has been instructed to monitor and ensure dishwashers are washing soiled hands before handling clean utensils. ,

Found the floor under equipment, edges and corners in the entire kitchen and bar soiled with accumulation and build up. To correct thoroughly clean floor and clean on a more frequent schedule.

Observed the presence of flies in the bar drain areas, and in the kitchen prep areas. To correct keep facility clean and dry, in good repair, and continue fly control measures.

Found the pop corn kettle soiled and bottom of pop corn machine cabinet soiled with old popcorn. To correct follow manufacturer's directions and clean entire pop corn machine.

Found some pop nozzles of the pop machine in the dining room and kitchen waitstations soiled with light accumulation. Found the pop gun nozzles in the bar soiled with light accumulation. Found the areas surrounding the interior front shield of the ice machine soiled with light accumulation. To correct follow manufacturer's directions and clean pop nozzles and ice machine.

Found the 1 door under the counter cooler in the dining area cooking station not holding tcs food at 41°F. To correct repair cooler to hold tcs food at 41°F. Found heavy ice build up in the 1 door cooler in the dining area waitstation, and in the right wine cooler in the bar. To correct defrost coolers and defrost on a more frequent schedule. Found the soap dispenser in the employee ladies restroom not mounted to wall but sitting on edge of sink. To correct reinstall soap dispenser to wall or replace if necessary. Found the secondary dishwashing machine (not in use) not hot water sanitzing dishes as indicated by heat strip. To correct repair dishwashing machine to hot water sanitize dishes prior to use.

Found no handwashing sign in the bar handsink area. To correct provide sign reminding employees to wash hands.

Found ice cream scoop stored on top of lid/cover of ice cream containers. To correct either store ice cream scoop in a dipper well, or provide numerous ice cream scoops and wash after each use. Found ice scoop stored on top of the right ice machine in the back kitchen area. To correct store ice cream scoop in a holder or clean container.

Found the bottom of the glass chiller in the bar soiled with broken glass and debris. Found the door gaskets and exteriors of coolers in the kitchen soiled with accumulation. To correct clean coolers and keep clean.

Found no thermometer available in several coolers and freezer through out entire facility. To correct provide internal hanging thermometers in all coolers.

(Critical) At the bar, observed the dishwashing machine with a final rinse sanitizer level of 0 ppm chlorine. Machine was placed out of service and repair was called. Use the dishwashing machine in the main kitchen until repairs are completed on the bar dishwashing machine. ****also, this violation has been cited on 3 of the last 5 routine inspections. Pursuant to the washtenaw county procedures for enforcement of the michigan food law you are required to attend an administrative conference at the health department office. An administrative conference notice was provided to randy garver during this inspection. A risk control plan needs to be developed and submitted to the health department for approval. A copy of risk control form was provided to randy garver during the inspection.

(Critical) Observed the following potential cross contamination of rte foods: observed 17 sheet trays of raw sausage and bacon stored next to two large bowls (service bowls covered with plastic wrap) of cut melon. Item corrected during the inspection by carefully inspecting the bowls for contamination, rewrapping the bowls, and relocating the bowls to a different approved location. Raw animal proteins shall be stored separate from rte foods

(Critical) At the four drawer cooler under the flat top grill on the main cook line observed the following improper cold holding potentially hazardous food/temperature combinations: 52 to 54 degrees f. #1-6 previously heat treated 1. Several portion bags of pastas 2. Fingerling potatoes 3. Potatoes 4. Asparagus 5. Corned beef 6. Several types of sausage 7. Several types of sliced cheese these items had been in the cooler overnight. The air temp thermometer inside the unit registered 50 degrees f. The above mentioned phf items were discarded. The unit was put out of order and a repair service was called. Please call for a followup inspection. Item is temporarily corrected.

(Critical) At the bar observed the dish machine in use with too low of sanitizer concentration. A test kit must be used to verify proper sanitizer concentration in the dishmachine.

On the main cook line observed the four drawer cooler under the flat top grill unable to maintain phf at the proper temperature. The unit is designed to hold at the proper temperature and has the proper capacity. The unit has been placed out of order and service has been called

Observed the area around the dumpster not clean. A large bag of trash and much debris was observed on the ground. Correct by cleaning now and on a more frequent basis to prevent the accumulation of food debris.

(Critical) At the bar, observed the dishwashing machine with a final rinse sanitizer level of 0 ppm chlorine. Machine was placed out of service and repair was called. Use the dishwashing machine in the main kitchen until repairs are completed on the bar dishwashing machine

(Critical) At the bar observed the dish machine in use with too low of sanitizer concentration. A test kit must be used to verify proper sanitizer concentration in the dishmachine.

At the rear prep area cook line observed the top of vulcan oven and the top of the snorkel oven with a heavy accumulation of dust and grease. Correct by cleaning now and on a more frequent basis to prevent the accumulation of dust and grease

In the kitchen walk in cooler #9 --the door fits poorly in the opening and rubs on the floor when opened. Also, the door threshold is cracked and not easily cleanable. Correct by repairing or replacing door and threshold. This item has been observed on previous inspections

Observed the following floor and wall areas in need of repair 1. Missing base cove molding on wall by handsink across from employee bathroom area. 2. Missing base cove molding on the wall by the main cook line. 3. Missing grout on the floor on the main cook line also, observed other tiled areas which need to be monitored and repaired now to prevent additional damage. Correct by identifying and scheduling these areas for repair or replacement. Several previously observed repairs of floor tile have been completed since the last inspection.

At the main dishwashing machine, observed a towel and plastic bag used to plug a broken cap on the upper spray rinse arm. The dishwasher functions and sanitizes properly. However, this spray rinse arm shall be repaired by a qualified person to manufacturer's specification. Correct by repairing

Observed missing coving on wall by handsink across from employee bathroom area. Also observed a few other areas which need to be monitored and repaired now to prevent additional damage. Correct by identifying and scheduling these areas for repair or replacement. Several previously observed repairs of floor tile have been completed since the last inspection.

At the rear cook line observed the floor with a heavy accumulation of food debris and grease. Correct by cleaning now and on a more frequent basis to prevent the accumulation of food debris and grease

At the main dishwashing area observed carts of clean dishes stored exposed with a deck brush, squeegee, broom, and long handle dust pan stored leaning against the cart. Correct by storing the brush, squeegee, broom, and long handle dust pan in a proper area such as a mop storage area or broom closet.

In the kitchen walk in cooler #9 --the door fits poorly in the opening and rubs on the floor when opened. Also, the door threshold is cracked and not easily cleanable. Correct by repairing or replacing door and threshold. This item has been observed on previous inspections.

The dining manager was instructed to use the dishwashing machine in the main kitchen until repairs are completed on the bar dishwashing machine

Correct by repairing or replacing door and threshold.

Found the rear outside entry door to the rear kitchen prep/ banquet area with a torn weather seal on the bottom. Light is visible under the door. Correct by repairing weather stripping to help prevent the entry of pests.

In the kitchen on the main make line, observed the floors under the grill top and fryers with an accumulation of grease and food debris. Correct by cleaning now and on a more frequent basis to prevent the accumulation of grease and food debris

Found the drain lines of some equpment un secured drain plumbing with items like a stone or other pipes used to maintain air gaps above the drain the equipment drains into. To correct should properly secure drain line plumbing of kitchen equipment to maintain air gaps above the drain the equipment drains into. Should secure plumbing with straps or other approved device.

Found clean pans stacked wet located at clean utensil storage shelving. To correct should allow pans to air dry prior to stacking pans at the storage area shelving

Found the outside garbage storage unit with access door in open position. To correct keep the door to the garbage storage unit in a closed position to reduce attraction of insects or rodents to the area

Observed the sides of the cooking area equipment with accumulation of debris. To correct should schedule more frequent cleaning to maintain the non food contact surfaces of the equipment at the cooking area clean as required

Found the door to the walk-in freezer with ice accumulation around perimeter of the door. To correct should schedule service to unit as needed to provide proper seal and easily cleanable surfaces
