(Critical) I observed slime mold growing on the interior of the ice machine. You need to be cleaning and sanitizing this unit on a monthly basis to prevent this bacteria from growing. I will be back in 2 weeks to verify it has been cleaned. Remove all ice before cleaning.
Your kitchen is not being kept clean enough. The following areas are observed dirty: the floors under all equipment, particularly the cooking equipment, the cooking equipment, shelves under prep tables, the walls and floors in the walk in cooler have mold growing on them (remove moldy beverage bottles). Also, clean the exhaust filters more often. I am going to come back in 2 weeks to verify that all of the above has been thoroughly cleaned.
By the correction date, have the condensation leak in the 2 door upright cooler repaired. There can be no water leaking into the interior of this unit.

(Critical) Your sanitizer is way too strong in your wiping cloth bucket at the front serve area. Use your test papers to assure it is made up at 50-100 ppm. Corrected by adjusting to proper strength.
(Critical) Once again, you are not safely re-heating your leftover product. You must be re-heating on the stove or in the oven and get foods to 165*f or greater before stocking on front steam well or in hot hold unit in the rear kitchen area. This is the 2nd time i've observed this unsafe heating method in 3 inspections. I am going to come back in 3 weeks to see that proper methods are being done.
