(Critical) Observed broken air dryer at handsink in ladies restroom. All handsinks must be supplied with soap and paper towels/heated air dryer at all times for proper employee handwashing. (Critical) Observed facility not sanitizing glassware properly as i measured 0 ppm of iodine in dishmachine (container empty). Need to sanitize between 12. 5-25ppm at all times. Please ensure all soap and sanitizer levels are full before washing glassware. Pic corrected by replacing empty sanitizer container at time of inspection. Rechecked, now sanitizing at 12. 5 ppm of iodine. Violation corrected.

(Critical) Observed several (10/11) unlabeled chemical spray bottles in facility. As stated in past, all chemicals/toxics must be labeled with correct common name at all times for proper identification. I will check this item at follow up inspection. Correct within 10 days. Please note: this violation has been cited at 2 consecutive routine inspection. As a result, will need to complete a risk control plan to help control this recurring violation. ,

(Critical) Observed a heavy accumulation of fruit/drain flies (cloud of flies) in upstairs bar area (near orange/pineapple boxes). Also, observed several drain flies in main bar area (near floor drains and in coffee station area). To correct, need to clean all these areas thoroughly and call a licensed pest control company for immediate treatment. I will check this item at follow up inspection. Correct within 10 days. ,

(Critical) Observed facility not sanitizing dishware and utensils properly as i observed 0 ppm of chlorine in dishmachine (sanitizer container empty). All dishmachines must wash, rinse and sanitize as per manufacturers directions (in this case 50-100ppm). Temporarily corrected by violation by refilling sanitizer bucket. Retested, now sanitizing at 50ppm. I will check this item at follow up inspection to ensure compliance. Please note: this violation has been cited at 3/5 routine inspections. As a result, you are required to attend an administrative conference at the washtenaw county health department for enforcement actions. Will need to call carl walczesky at 734 222-3872 within 72 hours to schedule. Please bring completed risk control plan to admin conference. , observed facility not sanitizing glassware properly as i observed 0ppm of iodine in glassware dishmachine (sanitizer container empty). Need to sanitize as per manufacturers directions (in this case 12. 5-25ppm of iodine). Replaced iodine container at time of inspection. Retested at 12. 5 ppm, but machine not functioning correctly (wash cycle not working). Will need to repair as soon as possible. Do not use this glassware machine until properly repaired and inspected by the washtenaw county health department. All glasses must be washed in kitchen dishmachine until repaired. I will check this item at follow up inspection. Correct within 10 days. ,

All fcs of equipment must be washed, rinsed and sanitized as per manufacturers directions and must be clean to sight and touch. (Critical) Observed facility not sanitizing dishware and utensils properly as i measured 0 ppm of chlorine in prep area dishmachine. Ecolab called at time of inspection. Need to sanitize as per manufacturers directions on chemical (in this case 50-100 ppm). To correct, please verify proper dishmachine operations and sanitizer concentrations more often and service as needed. Ecolab arrived and repaired dishmachine at time of inspection. Dishmachine now sanitizing at 50ppm. Critical corrected.

Will need to call a licensed pest control company to help eliminate all rodents. Also, staff needs to clean floors in dry storage area and eliminate any possible harborage conditions as soon as possible. A follow up inspection is required. Health Report - 08/25/2010 (2 critical incident(s))

(Critical) The dish machine is not providing (50) parts per million chlorine sanitizer as required. This is because the container of sanitizer attached to the machine is empty. Dishes, equipment and utensils must be properly sanitized every time they are washed in the dish machine. Failure to properly sanitize these items can leave harmful bacteria on them. This can cause food borne illness. Management must always ensure that adequate dishwashing supplies are on hand. Additionally, kitchen staff must routinely check, using chlorine test strips to ensure that the dish machine is functioning correctly and providing (50) parts per million sanitizer as required. Immediately, obtain sanitizer for the dish machine. Until that is available, as discussed use the three compartment sink, for washing, rinsing and sanitizing utensils.

(Critical) I observed three containers of home style raid at the bar. As discussed at the time of inspection, only insecticides designed and approved for use in a food service operation, maybe on the premises. Corrected at the time of inspection. The products have been removed from the restaurant.

I found a pair of flip-flops sitting on a counter top. These should not be stored there. Store personal belongings only in designated locations.

Replace the refrigerator thermometer in the bottom of the prep unit.

I found that 2-3 liquor bottles are stored in the drink ice. As discussed, ice is a food, as such it must be protected from sources of contamination. Do not use ice for drinks as a cooling medium.
