(Critical) Found spray and squeeze bottles used for chemicals, not labeled with contents. Corrected - pic labeled all bottles with contents

(Critical) Found soft serv mix in the machine holding @ 67* f. Corrected - pic discarded soft serv mix, cleaned the machine and took it out of service. Do not use this machine until it has been repaired and can hold mix at 41* f or less

Found no food thermometer in establishment. To correct - provide thermometer and use it. Soft serv ice cream and general refrigerator malfunction may have been prevented if thermometer was available and used

Found refregerator holding at 49* f. Corrected - pic removed and discarded all phf and took unit out of service. Do not use this unit until it is repaired. This is the second consecutive routine inspection that this violation has been cited. A risk control plan is required to be completed and faxed to me @ 734-222-3930 within 10 days (july 19th, 2012)

Found kitchen without correct test strips. Quat test strips instead of chlorine strips. To correct - provide correct test strips for chlorine (bleach)

Found staff not wearing required hair restraints. Corrected - staff put hats on

Found gasket at snow cone ice machine to have an accumulation of syrupy deposits and sprinkles. To correct - clean now and keep on a more frequent cleaning schedule

(Critical) Observed no chlorine residual in the dish washing sink. Corrected by adding additional chlorine to achieve 100 to 150 ppm

(Critical) Observed only household bleach provided in the facility (directions only for cleaning bathrooms and doing laundry). Provide food service chlorine sanitizer (proper sanitizing use directions and epa registration)

Observed the below counter one door refrigerator to be operating at a slightly elevated temperature. Adjust or repair as needed to maintain foods at 41f or below

Re-establish the drain line air gap for the ice cream dipper well located by the shake blending station. The vertical air gap distance is 2 x the discharge pipe diameter

Observed significant gaps along the side and bottom of the back door. Repair and/or provide additional weather stripping as need to eliminate the gaps

Domestic blender in facility. Replace with commercial equipment prior to 2011 season. And or provide spec sheet info.

Front dor ajar at time of inspection due to ac not working. Keep door closed once ac is repaired to minimize potential for insects to get into the facilty. Repair ac so door can stay closed or provide a screen.

Excess condensation in 4 door freezer which is used to store whipped cream adn in reach inc ooler which has condensation dripping and accumlating n bottom of unit. Eliminate condensation accumulation and repair so it does not continue.

(Critical) Raw eggs found stored in cooler with ready to eat foods. These eggs are for personal use as per owner. Remove from facility or store in tub in rear cooler covered on bottom shelf and label personal food.

(Critical) Water brought from home well water in reused containers. This is prohibited. Discontinue this process immediately. Remove water or discard. Your water supply at the facility is approved for use in the food service establishment.

Assorted domestic equipment including lapavoni coffee grinder, which is in porr repair and no longer cleanable where broken, microwave, canopeners, chest freezer and other domestic equipment in rear storage that is not in use. Replace with nsf approved commercial equipment. Provide cut sheets to the healt department for review and approval prior to purchase and installation. All menu changes must be approved by the health department prior to implementation including equipment changes.

Light bulb in the pretzel unit is not plastic coated. Replace with a plastic coated bulb to minimize potential for glass fragments to get onto foods if bulb should shatter.
