Observed interior of reach-in freezer located in the basement with accumulation of debris on the bottom compartment of the unit. To correct schedule more frequent cleaning of this unit as required. Found the catch tray of the range with debris to correct schedule more frequent cleaning.

(Critical) Kitchen two door refrigerator: observed a pan of raw, pork sausage to be stored on top of packages of deli salami on the bottom shelf. Corrected by moving the salami to the second shelf up from the bottom with other ready to eat foods. Dining room frigidaire one door cooler: observed eight, one dozen cartons of raw, shell eggs to be stored on a middle shelf above lettuce, carrots, tomatoes and juice cartons. Corrected by relocating the eggs to the bottom shelf away from other ready to eat foods

Observed a couple plastic containers on a kitchen storage shelf stored upright with a little debris in them. Corrected by re-washing them and storing them up-side-down for protection

One of the two light bulbs in the basement food storage room (with the freezers) does not have a shatter-proof coating. Provide a replacement light bulb that has a protective coating on it to prevent glass shards from contaminating food or single use items should the bulb break.

(Critical) Observed a spray bottle in the base cabinet below the hand washing sink to be lacking a label. Corrected by staff labeling the bottle as "degreaser".

(Critical) Observed an expired 1/3 pan of ready to eat pasta in the double door refrigerator that was expired as of 12-9. Corrected by discarding this food

(Critical) Observed two hotel pans of pork chops with sauce made earlier today to be covered and double stacked in the refrigerator. Corrected by separating these two food pans on different shelves and rotating the covers slightly to allow venting during the remainder of the cooling process

Basement dry goods storage pantry: the floor under the storage shelves is due for cleaning at this time

Observed cardboard being used on a couple wire shelves in the kitchen near the hand washing sink. Replace this cardboard with a smooth, easily cleanable surface that doesn't absorb moisture (plexi-glass, formica coated piece of wood, etc. )

The solenoid valve for the garbage disposal water line appears to be stuck in the open position. Repair/replace this valve or electrical controller as needed so the water is turned on and off when the disposal is used

Observed the water distribution bar of the ice making machine to be in need of cleaning at this time

Basement dry storage room for canned goods: the cinder block wall in the room that is an exterior foundation wall shows evidence of moisture intrusion (flaking paint and salt crystals). Thoroughly scrub and clean this wall. After allowing the wall to dry, apply an appropriate sealer/primer and then paint to prevent the migration of moisture/salts into this space

(Critical) The food service table is lacking protection for most of the food containers. Provide sneeze guards, individual pan/tray covers, etc. To protect foods during the self service period

(Critical) Observed a spray bottle of chlorine sanitizer mixed way too strong (bleached out the test paper). Corrected by diluting the solution to provide 100 ppm +/-. Ensure staff check these solutions with the appropriate test papers prior to using them

(Critical) Observed multiple potentially hazardous foods (potato salad 54f, fruit salad with melon 59f, chocolate milk 48f, salad lettuce 70f, and meat & cheese sandwiches 56-58f) to be out for service without any attempt to maintain them at 41f or below. These foods were discarded. At future meals ensure your phfs are maintained at proper temperature (cold foods <41f, hot food >135) by using ice baths, plug-in cold holding units, etc

(Critical) Observed a container of head lettuces that were moldy in the locked, one door upright refrigerator in the dining room, and a swollen, un-opened container of dairy product with a mfg. Date of 5-30. Corrected by discarding these foods. Ensure staff are making daily checks of your refrigerators to purge any expired/moldy foods

(Critical) The mechanical dish washing machine was not providing a detectable level of chlorine with test papers after the rinse cycle. It was determined that the one gallon source bottle of chlorine was weak. Corrected by providing a fresh bottle of chlorine sanitizer, priming the system and verifying 50-75 ppm chlorine

(Critical) Many of the interior surfaces of the ice making machine have a slime build-up on them. Turn off this unit, discard ice, disassemble piping/parts as needed to thoroughly wash rinse and sanitize. Place this unit on a regular cleaning schedule to prevent this build-up in the future

(Critical) Two spray bottles of chemicals stored at the three compartment sink were not labeled as to their contents. Corrected by labeling. Ensure these chemical bottles are labeled properly to prevent mix-ups with their usage

(Critical) Observed the end of the over-head spray rinse nozzle to be extended down into sink basin. Provide a replacement hose spring to maintain the end of the nozzle above the flood rim of the sink basin

Observed a couple metal pans stacked under the kitchen prep table while still wet. Ensure these items air dry prior to stacking them in storage

Observed a full pan of frozen food thawing on a counter top at room temperature and a large frozen piece of beef round thawing in the three compartment sink in standing water. Corrected by relocating the pan of food to the reach-in cooler and by turning on the cold water in the sink basin

(Critical) Observed previously opened containers of cream cheese and cottage cheese without the last day of use marked on them. Staff did not know when the cream cheese was opened, so it was discarded. The cottage cheese was opened last week friday, so is was marked with the date corresponding to thursday of this week. Corrected

Observed some paper plates in a storage cabinet adjacent to the kitchen to be stored upright. Please either invert these plates or place them in a clean bag for protection

Observed a large container of frying oil to be stored on the floor. Please find a location where this container can be stored at least 6" off of the floor

(Critical) Observed two glass jars of a home canned relish/salsa in the one door upright refrigerator in the dining room. Corrected by discarding. No home canned foods are allowed due to botulism concerns and home prepared foods are not an approved food source

(Critical) Observed a med/large bowl of mashed sweet potatoes (leftovers from dinner the previous day) to still be at 44f in the two door kitchen upright cooler (other foods were found to be at 39-40f in this unit). Corrected by discarding this food. In the future, a dense food like this needs to be spread out in a thinner layer to facilitate quick cooling

Observed two wet mop heads not hung. Please hang up your wet mops to facilitate faster drying

(Critical) Observed a dish washing machine detergent containing a chlorine additive being used to fill the sanitizer spray bottle labeled "quat sanitizer". Corrected by dumping out the detergent and refilling it with a 200 ppm quat sanitizer solution

Observed several aluminum foil pans on a bottom shelf in the dry storage room in the basement to be stored upright. Keep these pans stored upside down to protect the food contact surfaces from misc. Debris accumulation

Observed plastic knives in a drawer in the dining room with the handles in both directions. Please have some one with clean hands organize the knives with the handles extended toward the front of the drawer

Observed leftover hot foods placed in plastic containers and covered tightly with plastic wrap. Utilize uncovered metal food containers to rapidly cool your foods in a thin layer on the top shelf of a refrigerator

The floor in the dry storage room is due for a cleaning to remove spilled food & misc. Debris, especially under the shelving

Observed two third pans that have a damaged non-stick coating on them. These pans were discarded. In the future, please only use a non-scratching utensil (silicone, plastic, etc. ) in contact with the non-stick surface
