Reusable container with milk, unknown opening date. Potentially hazardous food (time/temperature control for safety food) prepared and held in a food establishment for more than 24 hours shall be clearly marked to indicate the date or day by which the food shall be consumed on the premises, sold, or discarded when held at a temperature of 5�c (41�f) or less for a maximum of 7 days. (Critical) Used a temperature sensitive tape and found the dishwashing machine not hot water santizing dishes. To correct call for repair and use the 3 compartment sink to wash, rinse and sanitize dishes. ,
(Critical) Found a bag of toasted almonds and cereal covered with beetles in the basement pantry shelf. Employee corrected by discarding bags of food covered with beetles and was instructed to clean shelves and surrounding area. ,
Found the interior bottom of the 2 door reach in freezer soiled with accumulation. Found the edges and sides of the hood gutter soiled with drips. To correct clean freezer and hood and keep clean.
Found the drain line under the left sink of the 3 comparment sink leaking. Found the edges of the cover of the grease trap eroding. To correct repair drain line and repair/replace cover of grease trap.
Observed the presence of mouse dropping in the basement pantry floor. To correct keep facility clean, in good repair, and continue pest control measures.

(Critical) Cheeses sticks were found at 46*f in the old true glass door cooler. This cooler cannot hold 41*f or colder so you need to take it out of use. Work on getting a new cooler. Corrected by discarding and will use the 3 door cooler to store all potentially hazardous foods in from now on, until a new cooler is brought in.
(Critical) I observed a large pan of pasta that was served yesterday at 53*f in the upright cooler. You need to follow safe cooling procedures and verify cooling in the proper times and temps. You need to break down large portions and cool in shallow pans and not be covering foods while cooling. I need you to rapidly cool hot foods in your walk in freezer from now on. Corrected by discarding and following the above procedures.
The old true glass door cooler is not capable of maintaining foods at 41*f or colder, therefore you need to remove it from your premises.
Garbage was not emptied from day before in your main storage building and small flies were present. You need to take out garbage after use to keep flies out of your buildings.
Found lid of dumpster in open position. To correct keep dumpsters fully closed.
Observed the presence of ants on the floor under the dishwashing machine. Pic states they have regular pest control service (observed glue boards with dates of service on them) and will have pest control company target ants. (Critical) I found an opened package of sliced turkey deli meats in the 3 door upright cooler not date marked. The package expiration date was december, 2012. Workers need to be sure to date mark opened, ready to eat foods. Corrected by discarding and educating workers.
Found a thermometer stored in a cup of water on the kitchen counter. To correct store clean and sanitized thermometer in it's holder in a clean and protected container or drawer.
Found a container of single use knives on the kitchen counter stored mixed. To correct store all single use knives with handles up.

(Critical) Found re-used egg cartons filled with eggs with a farm's name on the shelves in the reach in cooler. Pic states a farmer has donated eggs to facility and they may be given to people. Pic has been informed that all food that is given/donated and distributed to the public must come from an approved supplier(this farm doesnot appear to be licensed). Pic corrected by agreeing not to give/donate these eggs to the public, and has removed eggs from facility.
(Critical) Found a spray bottle of chlorine sanitizer which is used to wipe prep tables and equipment at a very high concentration. Pic corrected by emptying spray bottle of chlorine sanitizer, and remixed chlorine sanitizing solution to about 100 ppm, range minimum 50, and 200 ppm maximum.

One door true glass upright reach-in-cooler in dining area maintaining food at 49°F. All refrigeration shall be capable of maintaining food at 41°F or below. At time of inspection adjusted refrigeration unit to colder setting. At end of inspection refrigeration unit maintaining at proper temperature. Meat fozen or partially frozen, chipolte donated food recently placed in unit still at proper temperature and remaining food not potentially hazardous (bagged cabbage, carrots, etc. ) no food was discarded

(Critical) A bowl of potato salad is improperly dated. It is dated 4/24, whick is well over 7 days from todays date. Must be sure you are properly dating product. Also, there is a ham that has a package stamp use by date of 2/11/11. And there is some moldy cheese and produce that is spoiling. Audit yor foods daily for quality control. Corrected by discarding all of these foods.
Repeat violation 2 times now. Once again, the lights in the kitchen are not protected by a shield. Have them protected within 2 weeks and take a picture and send to my e-mail by 4-20-2011.
