(Critical) Found the food contact surfaces of the slicer soiled with dried food debris. Pic corrected by breaking down slicer and washed, rinsed, and sanitized food contact surfaces of slicer. Pic has been instructed to monitor that equipment is properly cleaned after use.

(Critical) Found food (yogurt and tofu) passed it's use by date in the kitchen 2 door sliding glass cooler. Found food ( tofu, milk) passed it's use by date in the back building walk in cooler. Pic corrected by discarding food passed its use by date, and has been instructed to monitor that items are used by their use by date.

(Critical) Found facility using a household fly insecticide (unapproved). Pic corrected by discarding unapproved insecticide and has been instructed to use only insecticide that states can be used in a food establishment.

Found excess ice build up in the 2 door sliding chest freezer. To correct defrost freezer and defrost on a more frequent schedule. Found the upper sleeve of the mixing bowl of the large mixer with rusty surface. Pic states this mixer is not used. To correct either refinish or replace mixing bowl sleeve.

Found the front and back door propped open. To correct keep doors closed.

Found unsealed shelves in the storage room. To correct either seal wood or replace with surfaces that are durable and nonabsorbent.

Found numerous boxes and a tire on the outside back area sink and floor. To correct remove recycleables and properly dispose of other items.

Observed the presence of flies in the kitchen area. To correct keep doors closed, and continue using fly control measures.

Found the back building's walls and ceiling areas with open areas. Found the tiled floor in the front entrance area with chipped and missing tile. To correct properly repair/replace noted areas as soon as possible.

Found various equipment and utensils stored on the floor of the storage room. To correct store equipment and utensils at least 6 inches off the floor. Found a stack of unwrapped coffee filters stored in a box under the prep table. To correct either store in it's original bag or store in a covered box.

(Critical) Observed employee peel/handle cooked potatoes for potato salad with bare hands. Pic corrected by having employee discard peeled potatoes, wash hands, wear gloves and continue peeling potatoes. Pic has been instructed to monitor that employees are not handling ready to eat foods with bare hands.

(Critical) Observed employee working in the dishwashing area handle soiled utensils, rinsed hands in the 3 compartment sink, and then use a towel to dry clean utensils. Pic corrected by having employee wash hands in handsink, before handling clean utensils.

(Critical) Found the hose bibbs in the back door area without backflow protection. Pic corrected by providing and installing hose bibb vacuum breakers.

Found employee working in the cooking/prep areas not wearing hat or hair restraint. To correct provide hats, and remind employees to wear them.

Found the screening of the interior screen door leading to the kitchen loose and not secured to door. To correct re-secure screening to door.

Found the entire kitchen floor soiled with build up. To correct throughly sweep, scrub and mop floor, and clean on a more frequent schedule.

Found the interior of the microwave soiled with accumulation and build up. To correct clean interior of microwave.

Found the walls in the storage area peeling. Found the ceiling above the prep area with open seams and peeling paint. Found the surface of the floor in the kitchen cooking area uneven and worn. Found holes in the cooking line wall, corner wall by water heater, and in the restroom improperly covered with various items (plastic, towels, insulation, and trays). Found the tiles floor in the front counter area cracked, chipped and uneven. To correct properly repair all noted areas.

Found single use plastic carry out containers being re-used to handle food. To correct discontinue using single use containers and use commercial grade utensils.

Found roasted whole chickens cooling covered in walk in cooler. To correct cool chicken uncovered, and consider cutting chicken into smaller pieces. Pic has been reminded that tcs food, must be cooled from 135 to 70°F within 2 hours, and from 70°F to 41°F within 4 hours.

Found no handwashing sign in the employee restroom handsink area. To correct provide sign reminding employees to wash their hands.

Found bread covered with trash liners. To correct discontinue using trash liners to cover bread, and use food grade materials.

Found mop left in the mop bucket in the mop sink. To correct hang mop, (allowing it to air dry), when cleaning is completed.

Found no light shield in the inner area of the walk in cooler. To correct provide light shield.
