(Critical) Observed a container of raid ant and roach killer in the kitchen. Poisonous or toxic materials shall be: (a) used according to: (1) law and this code, (2) manufacturer's use directions included in labeling, and, for a pesticide, manufacturer's label instructions that state that use is allowed in a food establishment, (p) (3) the conditions of certification, if certification is required, for use of the pest control materials, (p) and (4) additional conditions that may be established by the regulatory authority to correct discard the noted item into the garbage. Corrected: person in charge discarded the noted item into the garbage.
(Critical) Found an open carton of commercially prepared cole slaw without and "opened on" date. Corrected - cole slaw was discarded. All commercially prepared food items must be datemarked when opened, and, regardless of commercial date stamp, must be discarded within 7 days from opening
(Critical) Found a large quantity of cooked and cooled steak meat, prepared yesterday (according to staff) portioned in the salad/sub prep unit without any prep/discard date. Corrected - portions of steak were dated with yesterday's date and staff was educated in datemarking requirements
(Critical) Found cooked chicken tenders stacked in a 1/4 pan, covered in salad/sub prep fridge. Upon inspection, when lid was removed, condensation rolled down the lid, chicken was holding @ 43*f. When asked, chicken was prepared the previous day, staff stated cooked chicken tenders are removed from fryer and directly stored in 1/4 pan, covered and placed in prep unit. Corrected - staff was educated on the proper cooling method to be used, staff stated that the correct method is used with the steaks used for pizza and subs. This was verified at the time of this inspection. Staff will now use same cooling method for chicken tenders
Found the ceiling over the pizza oven to have an accumulation of greasy, dusty debris hanging from peeling dry wall tape. Also found the floor sink at the 3 comp sink and both hand sink faucets to be in need of cleaning. To correct - clean ceiling, repair taped seam clean floor sink and hand sink faucets thoroughly, and keep all on a more frequent cleaning schedule
Found spice containers to have an accumulation of food debris on them. To correct - replace these worn out, not cleanable containers with new containers. Change these containers more often in the future
Found a section of the ceiling in the corner of the dry storage room to be in need of repair as the dry wall is chipping, peeling and can fall onto the jars and cans stored below. To correct - repair ceiling drywall
Found a disposable cup filled with single service forks and spoons being stored with tines and bowls up. Corrected - forks and spoons were stored in compartmentalized utensil tray with all handles toward end
Found the can opener blade to have jagged edges which can cause metal shavings to occur and fall into food. To correct - replace can blade

Found gaskets on pizza prep unit doors to be badly worn, sagging and cracked - very hard to clean without further cracking and breaking. To correct - replace gaskets on this unit before the next routine inspection
Found mops hanging with the handle down under the mop heads. In this situation, the mop head can drip on the handle contaminating it for the next person that touches the handle. To correct - hang mops such that the handle is up. A new hanger system will likely be needed to accomplish this set-up
Found the side of the fryer and the wic door (around the handle) to have an accumulation of debris on them. To correct - clean these areas now and keep on a more frequent cleaning schedule
Found the bottom of the reach-in cooler under the front window to have an accumulation of food debris. Also found the side of the deep fryer to have an accumulation of grease on the side. Tcorrect - clean now and keep on a more frequent cleaning scedule.
Found all chest freezers to have an accumulation of frost build-up on them. One freezer also has a broken lid component that is allowing the insulation to be exposed. Found a ceiling vent that has a pizza screen that is being used instead of the appropriate vent screen. To correct - defrost freezers to remove frost build-up and allow the gaskets to seal correctly. Also, the broken lid must be repaired/replaced by the next routine inspection. To correct - replace the pizza screen with the appropriate cieling vent screen
Found pizza sauce splattered on the wall of the walk-in cooler. To correct - clean now and keep on a more frequent cleaning schedule. Recommend adding this item to the closing duties so that this is done daily
Found no anti-choking poster found in establishment. To correct - obtain required anit choking poster from the american red cross or on-line and post in the establishment
The wall inside the walk-in refrigerator, needs cleaning. Clean up the tomato sauce on the wall.
