(Critical) Observed 2 pans of rte soup in salad bar/buffet unit in dining room with internal food temperatures of 92-99f. Soups have been in breakfast bar for less than 30 minutes per staff. All phf must be rapidly reheated to 165f or more within 2 hours. Need to reheat on stove, steamer or in microwave oven before placing into hot holding unit. Staff stated correct reheating procedures, but they did not check food temps this morning before placing soups on breakfast bar. (Critical) Observed pan of housemade cheesy broccoli medley salad in wic and 2 different rte pies in dessert station cooler with expired datemark labels. Salad and pies expired on 7/5 & 7/6/11. All ph rte foods must be used or discarded within 7 days (day 1 is prep date). To correct, please rotate stock more often and discard all food with expired use by dates. Observed clean coffee spoons stored in a badly damaged container in server prep area (container is flaking onto clean spoons). All utensils must be stored in a clean, dry location and protected from sources of contamination at all times. To correct, please send all spoons through dishmachine and discard damaged container as soon as possible

Observed frozen pre-cooked chicken breasts stored on top of box in wif (not in pan, not covered). Also observed open containers of food on shelf in wic. All food must be covered at all times to protect from sources of contamination

(Critical) Observed can of whipped cream in sliding door display cooler in dessert station area (near front counter) with an internal food temp of 54f. Cooler is not functioning correctly. All phf must be cold held at 41f or below at all times. Do not use this cooler to store phf until properly repaired and inspected by the wash county health dept. Temporarily corrected by discarding whipped cream. Pic indicated that he will be taking cooler out of service and will not be storing phf in cooler. I will check this item at follow up inspection. Observed employee/contractor coats stored on top of bread racks in side kitchen area. All employee coats and personal belongings must be stored in proper areas which do not contaminate food or equipment

Observed top portion of sliding door display cooler (near front counter) unable to cold hold at 41f or below. Cooler did not get lower than 50f after 10 minutes. To correct, please repair as soon as possible. Pic indicated he may take this cooler out of service. I will check this item at follow up inspection

Observed covered (tightly sealed with plastic wrap) pans of sliced potatoes (home fries) and cinnamon apples cooling on shelf in wic. Foods were removed from hot breakfast bar 2. 5 hours ago (per pic) and had an internal food temp of 64-68f. Although foods were in shallower pans and were within temp guidelines, foods cannot be covered when actively cooling. To correct, please remind and educate all employees on proper cooling methods and be sure to monitor phf as it passes through the temp danger zone.

Observed scooping utensils stored in dipper well near ice cream freezer that is slowly dripping. If storing utensils in dipper well, dipper well must be turned on and flowing properly to prevent the accumulation of bacteria
