(Critical) Observed cockroaches in the kitchen. Enter into a contract with a pest control company to service your facility for these pests. Provide me a copy of your contract and the invoice for an initial service visit asap. Perform a deep cleaning (out of the way areas) and eliminate any plumbing leaks

(Critical) Observed three unapproved pesticide spray cans in the back kitchen area. These insecticides specifically indicate they are for household use only. Remove them from the facility

(Critical) Observed multiple ready to eat, potentially hazardous foods with out use-by dates (flan, cooked beef, cooked chicken, cooked rice, chicken broth, sauces, etc. ). A discussion with staff was held regarding how to date mark foods. Implement the date marking of your rte, potentially hazardous foods--i will verify date marking at the follow-up inspection

(Critical) Observed raw in-shell eggs to be stored on the top shelf of a back kitchen cooler above rte foods. Corrected by relocating the eggs above raw beef and chicken in an other cooler

(Critical) Observed some cooked vegetables with a date of 10-18-2010. Corrected by discarding

A couple refrigerators are lacking thermometers or had damaged/inaccurate thermometers. Provide replacement air thermometers as needed

Observed the mop head resting in the bottom of the mop sink. Please drill a hole in the end of the mop handle to hang the mop up on one of the screws provided

Observed the following areas to be in need of cleaning now and frequently in the future to eliminate spilled food or misc. Debris: the floor sink drain at the front counter area, the floor sink drain at the dish washing area, the floor around the grease trap, the floor under equipment and the wall at the old bread making area

Observed the new wooden partial wall and shelving unit behind the prep cooler at the front counter area to be in need of sealing to be smooth and easily cleanable. Apply a polyurethane or similar water proof coating to all sides of this bare wooden structure

The metal stem thermometer provided was not calibrated accurately. Corrected by calibrating the unit

Only quat sanitizer test papers were provided in the facility. This operation uses chlorine sanitizer. Corrected by obtaining chlorine test papers

Observed many paper and styrofoam food containers stored in a cabinet with the waste water drain for a hand washing sink. Corrected by relocating these single service items.

Observed a significant gap under the back door that can allow pest entry and heat loss. Provide a properly sealing door sweep

Observed styrofoam bowls in the back storage area and some to-go containers at the front counter to be stored upright. Corrected by inverting these items

Observed damaged and unused equipment still present in the back kitchen area. Remove this equipment from the facility to make it easier for cleaning the kitchen and to eliminate a potential pest harborage area

Observed a bucket under the drain pipe for the sanitizing basin of the three compartment sink catching leaking waste water. Repair this drain as needed to eliminate the water leak
