(Critical) Observed the bar dish washing machine not sanitizing properly. A chemical sanitizer used in a sanitizing solution for a manual or mechanical operation at contact times specified under # 4-703. 11(c) shall meet the criteria specified under # 7-204. 11 sanitizers, criteria, shall be used in accordance with the epa-registered label use instructions. (Critical) Observed new england clam chowder was were cooked the previous day 45f and still in the cooling process, located in the walk-in cooler, in a tightly sealed container. Cooling shall be accomplished in accordance with the time and temperature by using one or more of the following methods based on the type of food being cooled: (1) placing the food in shallow pans; pf (2) separating the food into smaller or thinner portions; pf (3) using rapid cooling equipment ; pf (4) stirring the food in a container placed in an ice water bath; pf (5) using containers that facilitate heat transfer; pf (6) adding ice as an ingredient; pf or (7) other effective methods. Pf to correct discard the noted food and start cooling the noted food with one of the above, approved methods. Corrected: person in charge discarded the noted food and stated that the food would be cooled with an ice bath and would leave it loosely covered to allow the cold air to penetrate the noted food

(Critical) Observed a variety of deserts 58-60f in a non-working display cooler for about 1 hour. 1. Cheese cake 58f. 2. Lemon m. Pie 60f. Potentially hazardous food that is to be cold shall be held at 41f or below. To correct move noted food to a working cooler to rapid cool the food to 41f or below and check coolers before putting food into cooler. Observed the following non-working coolers: 1. Two door top loading cooler used by servers with an air temp of 78f. Note: no potentially hazardous food being stored inside. 2. Dessert display cooler in the dinning room with an air temp of 60f. Note: all potentially hazardous food was moved to a working cooler. Equipment shall be maintained in a state of repair and good condition. To correct repair all noted cooler to hold cold food at 41f or below. ,

(Critical) Observed the following food holding above 41f for an unknown amount of time: 1. Two containers of half and half at both wait stations 46-50f. 2. A pan of cut tomato's 50f. 3. Hard boiled, peeled eggs 46f. 4. Cooked red potato's 45f. Potentially hazardous food that is to be cold shall be held at 41f or below. To correct discard the noted food into the garbage. Corrected: person in charge discarded the noted food into the garbage

(Critical) Observed three cans of raid and other branded pesticides in the facility. Poisonous or toxic materials shall be: (a) used according to: (1) law and this code, (2) manufacturer's use directions included in labeling, and, for a pesticide, manufacturer's label instructions that state that use is allowed in a food establishment, (p) (3) the conditions of certification, if certification is required, for use of the pest control materials, (p) and (4) additional conditions that may be established by the regulatory authority to correct discard the noted items into the garbage. Corrected: person in charge discarded the noted items into the garbage.

Observed the following not sealed to the wall: 1. All parts of the hood. 2. Hand sink shield by the prep table, opposite of dish area. 3. Three compartment sink nearest to the office. Equipment that is fixed because it is not easily movable shall be installed so that it is: (1) spaced to allow access for cleaning along the sides, behind, and above the equipment; (2) spaced from adjoining equipment, walls, and ceilings a distance of not more than 1 millimeter or one thirty-second inch; or (3) sealed to adjoining equipment or walls, if the equipment is exposed to spillage or seepage. To correct seal all the noted items to the wall and use heat sensitive silicon where needed.

Observed all cutting boards throughout the facility to be stained and grooved. Surfaces such as cutting blocks and boards that are subject to scratching and scoring shall be resurfaced if they can no longer be effectively cleaned and sanitized, or discarded if they are not capable of being resurfaced. To correct resurface or replace the noted cutting boards.

(Critical) Found time/temperature control for safety (tcs) food, creme brulee 44°F, cheese cake 47°F, whipping cream 45°F, in the display dessert cooler. Pic temporarily corrected by relocating desserts to 1 door reach in cooler and called for repair. Found tcs food, coleslaw 50°F in the 1 door reach in cooler. Pic corrected by discarding coleslaw and called for repair. Pic has been instructed not to hold tcs food in these coolers until repaired/replaced to hold tcs at 41°F or lower.

(Critical) Used a quat test kit and found the sanitizing solution in the three compartment sink below manufacturers range of 150 to 400 ppm. Pic corrected by calling for repair, service tech arrived repaired quat dispenser, and verified proper concentration with a quat test kit. Pic states quat dispenser was checked and tested last week.

(Critical) Found no paper towels in the basement employee men's restroom handsink area. Pic corrected by having employee restock paper towel dispenser. Recommend that all staff help restock paper towel dispenser when empty or notify management.

Found the dessert display cooler and the 1 door reach cooler in the prep area not holding tcs food at 41°F. To correct repair coolers to hold tcs food at 41°F. Found 1 of 2°Fans on the evaporator coil in the basement walk in cooler not running. To correct repair fan.

Found a plastic sheet covering wall and taped to the back splash of the dishwashing counter which prevents water from dripping onto the floor but in to dishwashing counter. To correct adjust/modify backsplash and seal to wall. Found a hole on the top ceiling of the employee mens restroom and damaged walls in the employee ladies restroom. To correct repair walls and ceiling to be smooth, durable and cleanable.

Found the 2 door prep top cooler on the cooking line not working or in use. To correct repair cooler to hold tcs food at 41°F or lower.

Found some grease dripping from cooking hood edge and heavy grease accumulation by the grease collection container on the edge of the hood gutter. To correct thoroughly clean hood and clean on a schedule to preclude accumulation.

Found expired food license posted on bar wall. To correct post current food license in a conspicuous place for public inspection.

Found the rolling door in the front of the bar counter open with a rolling screen door not pulled down to help prevent the entry of pests. To correct when rolling door is open pull down rolling screen door to help prevent the entry of pest.

Found a spray bottle of olive oil and plastic squeeze bottle of wine and water without labels. To correct provide labels.

Found bar window open without screen. To correct either provide a window screen if window is left open or keep window closed.

Found slight accumulation in the pop nozzle of the pop gun in the bar. Pic corrected by having employee wash, rinse, and sanitize pop nozzle.

Found the entire basement floor in poor condition (missing tile, uneven spots, and peeling surface coating). Found the employee restroom walls with cracks and uneven spots. To correct in future remodeling plans address these noted issues.

Found the spray rinse nozzle hose in the 3 compartment sink leaking water from hose. To correct repair coolers to hold tcs food at 41°F, and repair spray rinse nozzle hose.

Found a wicker type basket used to hold spoons used to dispense food. To correct replace wicker type basket with a commercial grade container that is smooth, durable, and easy to clean.

Found the front side door and it's screen door propped open. To correct keep either door or screen door closed.
