(Critical) Observed the underside of the sliver mixer arm soiled with food debris. Equipment food-contact surfaces and utensils shall be cleaned: (1) except as specified in # (b) of this section, before each use with a different type of raw animal food such as beef, fish, lamb, pork, or poultry; (p) (2) each time there is a change from working with raw foods to working with ready-to-eat foods; (p) (3) between uses with raw fruits and vegetables and with potentially hazardous food (time/temperature control for safety food); (p) (4) before using or storing a food temperature measuring device; (p) and (5) at any time during the operation when contamination may have occurred. (Critical) Observed the following in poor repair: 1. Salad spinner has a broken bottom. 2. Cheese grader is bent and has broken edges. 3. Lemon squeezer at the bar is chipping. Multi use food-contact surfaces shall be: (1) smooth; (pf) (2) free of breaks, open seams, cracks, chips, inclusions, pits, and similar imperfections; (pf) (3) free of sharp internal angles, corners, and crevices; (pf) (4) finished to have smooth welds and joints; (pf) to correct discard the noted items into the garbage. (Critical) Observed no paper towel at the handsink in the prep area. Each handwashing sink or group of adjacent handwashing sinks shall be provided with: (a) individual, disposable towels;(pf) (b) a continuous towel system that supplies the user with a clean towel;(pf) or (c) a heated-air hand drying device;(pf) or (d) a hand drying device that employs an air-knife system that delivers high velocity, pressurized air at ambient temperatures. Observed the following not sealed to the wall: 1. Hand sink by the stairwell. 2. Clean side of the dish line drain board. Equipment that is fixed because it is not easily movable shall be installed so that it is: (1) spaced to allow access for cleaning along the sides, behind, and above the equipment; (2) spaced from adjoining equipment, walls, and ceilings a distance of not more than 1 millimeter or one thirty-second inch; or (3) sealed to adjoining equipment or walls, if the equipment is exposed to spillage or seepage. Found the fryer not completely under the cooking hood. To correct move/adjust cooking equipment to fit under hood or provide additional hood.

Found bread stored in garbage bags in the basement walk in cooler. To correct discontinue using garbage bags to store bread and use food grade bags or containers.

Found the floor in the kitchen cooking and prep areas soiled with accumulation and build up. To correct thoroughly clean floor and clean on a more frequent schedule.

Found opened bags of breading and other ingredients on the basement shelves not covered or protected. To correct store opened bags of breading and ingredients in commercial food containers.

Found the 3 door prep top cooler with torn door gaskets and missing handle on top door. To correct repair/replace door gasket and handle on cooler.

Found the cooking hood hood filter soiled with dust and grease build up, and edges of cooking hood with grease. To correct clean hood filters and hood edges, and clean on a more frequent schedule.

Found an employee's open cup of beverage on the prep table. Pic corrected by discarding open cup and was instructed to monitor and ensure employees are using approved cups in prep areas, such as a cup with lid and straw.

(Critical) Found time/temperature control for safety (tcs) food, cooked beans 44°F, tomatoes 44°F, and burgers 44°F in the main level walk in cooler. Pic temporarily corrected by calling for repair and moved tcs food to basement walk in cooler. Pic has been instructed not to use this walk in cooler to hold tcs food until repaired/replaced to hold tcs food at 41°F.

(Critical) Found ready to eat (rte), time/temperature control for safety (tcs) food, egg whites, salami and duck terrine without 7 day datemark in the 2 door dessert/salad prep top cooler. Pic corrected by providing 7 day (day prepared + 6 days) datemarks for all items and has been instructed to monitor and ensure appropriate items are datemarked. This violation has repeated 3 of the last 5 routine inspections and a risk control plan and administrative conference notice has been issued.

(Critical) Found a container of raw shell eggs stored next to ready to eat foods in the top portion of the 2 door dessert/salad prep top cooler. Found a case of raw shell eggs store next to and above ready to eat foods in the basement walk in cooler. Pic corrected by relocating raw shell eggs to a location below and away from ready to eat foods.

Found the fryer on the edge of the hood not completely under the cooking hood. To correct remove and adjust equipment so cooking equipment is under the cooking hood.

Found several storage containers stored wet and inverted in the clean utensil storage rack. To correct allow storage containers to air dry prior to storing.

Found ice scoop stored on top of the ice machine. To correct store ice scoop in a holder or clean container.

Found the drain line of the basement handsink draining and splashing onto the floor. To correct adjust drain line to drain into drain receptacle.

Observed the presence of flies in the bar area. To correct keep facility clean and dry, in good repair, and continue fly control measures.

Found the edges of the cooking hood soiled with grease. Found the interior of the microwave in the dessert and salad area soiled. To correct clean as soon as possible, and clean on a more frequent schedule.

(Critical) Found the bar menu with drinks that contain raw egg white without consumer advisory. To correct provide consumer advisory, that is a disclosure such as, asterisk (*) undercooked foods to a footnote stating the items are raw or undercooked, and reminder footnote "consuming raw or undercooked meats, poultry, seafood, shellfish, or eggs may increase your risk of foodborne illness. Example: menu item: *new york sour on the menu: *contains raw or undercooked ingredients. Consuming raw or undercooked meats, poultry, seafood, shellfish, or eggs may increase your risk of foodborne illness.

(Critical) Found facility using "pink salt" which pic states contains "sodium nitrates, nitrites to "cure" meats hanging in the walk in cooler, without a variance from the health department. To correct either remove meat from facility or discard meat and obtain a variance if curing food is desired.

(Critical) Found time/temperature control for safety (tcs) food, whipped cream 49°F, roasted squash 52°F, cheese 50°F, in the bottom portion of the 2 door prep top cooler. Pic temporarily corrected by discarding tcs food, and has been instructed not to use this cooler to hold tcs food until repaired/replaced to hold tcs food at 41°F.

(Critical) Found 2 blender/pitcher rinsing units on the left and right side of the bar without backflow protection. To correct provide an air gap of at least 1 inch from the drain line(s) and rim of the drain receptacle and a double check backflow device (meeting asse 1024 or 1022) on water line(s).

Found the grill brush stored on the floor by the grill. To correct store grill brush off the floor in a more protected manner.

Observed flies in the bar ice bin area. To correct keep areas clean and dry, in good repair, and continue fly control measures.

Found no certified manager employed at facility. To correct provide a certified manager.

Found a household fryer on the prep area counter top. To correct discontinue using household fryer and use your commercial grade fryer.

Found the 2 door prep top cooler not holding tcs food at 41°F. To correct repair/replace cooler to hold tcs food at 41°F.

(Critical) Found ready to eat (rte), time/temperature control for safety (tcs) food, quiche, creme fraiche, creme brulee, in the salad and dessert 2 door prep top cooler without 7 day (day prepared + 6 days) datemarks. Note: various items in this cooler had prepared dates, and some had discard dates. Please set up a consistent datemarking system. Found rte, tcs food, a container of hummus in the 3 door prep top cooler without 7 day datemark. Found rte, tcs food, a container of rice, boiled eggs and portioned turkey meat without 7 day datemark in the walk in cooler. Pic corrected by providing 7 day datemarks for all noted items, has been instructed to setup a consistent datemarking system, and verify employees are following datemarking policy.

Observed employee clean long cutting board in the dishwashing area, that is employee poured bleach on cutting board, scrub cutting board, and rinse off cutting board. Corrected by instructing and directing employee to wash cutting board with soapy water, rinse cutting board with clean water, and wipe sanitizing solution on cutting board. To permanently correct pic has been instructed to monitor and ensure employees are trained and following proper cleaning procedures.

Found the corner handsink by the stairs blocked with shelf and power cord of table top appliance. To correct relocate items so handsink is easily accessible.

Found no antichoking poster on the kitchen wall. To correct provide and post antichoking poster on the kitchen wall.

Found a stack of coffee fitlers stored on counter top unwrapped and stacks of food containers stored upright. To correct store coffee filters in a more protected manner, and store food containers inverted.

Found styrofoam boxes, which previously held items, being used to hold food containers of condiments and garnishes. To correct discontinue re-using styrofoam boxes, and replace with commercial grade food containers.

Observed the presence of flies in the garbage cans by the dishwashing area. To correct keep areas clean and dry(including garbage cans) , in good repair, and continue fly control measures

Found the back kitchen floor by the dishwashing area with numerous cracked tiles. To correct repair/replace floor to be durable, smooth and easy to clean.

Found the walk in cooler without emergency door release. To correct provide emergency door release.

Found the pop gun nozzle in the front end of the bar counter soiled with accumulation. Employee corrected by cleaning and sanitizing pop gun nozzle, and has been instructed to clean on a more frequent schedule.

Found the edges and sides of the cooking hood soiled with grease. To correct clean as soon as possible, and clean on a more frequent schedule.

(Critical) Found duck eggs in re-used egg cartons in a re-used egg case without documents indicating licensed approved source. To correct, pic has been instructed to remove all duck eggs from facility, and to obtain duck eggs from sources that comply with law. ,

(Critical) Found the underside of the slicer blade edge soiled with build up. Found the interior food contact surfaces of the juicer soiled with food debris. Found the surfaces of the potato cutter soiled with food debris. Found the edge of the interior front shield of the ice machine soiled with accumulation. To correct properly clean food contact surfaces of equipment and utensils, that is wash, rinse, and sanitize. Pic has been instructed to monitor that employees are properly cleaning food contact surfaces of equipment and utensils. This is a repeated critical violation and a risk control plan form has been issued. ,

(Critical) Found time/temperature control for safety (tcs) food, a container of cantoloupe slices 48°F, turkey slices 49°F, on the bottom left portion of the 2 door dessert prep top cooler. Note: stack of plates were in fan area blocking air flow. Pic temporarily corrected by discarding tcs foods, and has been instructed to verify cooler is working properly and repair if needed, relocate plates, and to ensure tcs food is held at 41°F. ,

(Critical) Observed cook use bare hands to sprinkle ready to eat (rte) food pumpkin seeds on salads. Observed cooks use bare hands to plate rte food, fruit on dessert plates. Pic corrected by having cooks wear gloves. Pic has been instructed to retrain cooks on proper glove usage, that is gloves or utensils must be used when handling rte foods, and to change gloves when contaminated. ,

Found no chlorine test kit in the bar dishwashing machine area. To correct provide a chlorine test kit.

Found the tile grout of the floor in the bar and waitstation area worn. To correct repair tile grout to be smooth and without deep grooves.

Found the condensation pan of the evaporator coil in the basement walk in cooler supported by a can and block of wood on shelf. To correct repair evaporator coil housing. Found the condensation drain line of the walk in cooler on the main level kitchen without insulation. To correct provide insulation from the drain line to at least the side of the walk in cooler wall. Found the left side of the bottom portion of the 2 door prep dessert cooler not holding tcs at 41°F, with a stack of plates blocking fan. To correct verify that cooler is working properly, relocate plates, and ensure tcs food is held at 41°F or lower.

Found facility using a household juicer in the basement prep area, and a styrofoam box to hold containers of food in an ice bath on the plating area of the kitchen. To correct replace juicer with a commercial grade juicer, and use a container that is durable, smooth and easy to clean for ice baths.

Found no handwashing sign by the bar handsink. To correct provide sign reminding employees to wash their hands.

Found the ceiling and front of the hood soiled with dusty build up. Found the stairs leading to the basement soiled with accumulation of debris. Found the wall around the dishwashing machine soiled with moldy build up. To correct clean as soon as possible, and clean on a more frequent schedule.

Observed the presence of flies in the main level bar prep area and in the basement prep area floor drain area. To correct keep areas in good repair, clean and dry, and contact pest control company.

Found the main level walk in cooler not working properly with an air temperature of 55°F, and no time/temperature control for safety (tcs) food in walk in cooler. Pic states he moved all tcs food to basement walk in cooler, and has called for repair. Pic has been instructed not to use the walk in cooler to hold tcs food until repaired/replaced to hold tcs food at 41°F or lower. Found the chemical warning lights of the bar glass washer flashing and dishwashing detergent appear low. Pic corrected by instructing staff to refill detergent, and to refill when light warns of low chemicals.

Found no handwashing sign in the bar handsink area. To correct provide sign reminding employees to wash their hands at all locations where they may wash their hands.

Found the spaces of the beer pallets/keg racks in the basement walk in beer keg cooler soiled with beer and mold. Found the edges and areas around the cooking hood soiled with greasy build up. Found the shelves under and around the steam table in kitchen soiled with accumulation of debris. To correct clean as soon as possible, and clean on a more frequent schedule.

Found metal shakers and plastic squeeze bottle of seasonings without labels. To correct provide labels.

Found several opened bags of food ingredients (sugar) stored on shelf. Pic corrected by rolling/folding bags to better protect food, and has been instructed to better protect food by keeping covered or storing food in storage containers.
