Observed cardboard lining chemical storage shelf. Please discontinue using cardboard to line shelves and provide a non-absorbent, smooth, easily cleanable surface. In addition there is a wet box of floor tiles under the coffee dispenser table rear kitchen. Please remove the wet box.

Bottom section of ice machine in rear kitchen not present; facility using cooler to catch and store ice. Please provide a cooler with a drain until the ice machine can be replaced or install a combination ice machine/pop dispenser front counter and eliminate the ice machine in the rear kitchen. Ice shall be stored in a sloped self-draining container to allow for complete draining during storage

Please provide a waste receptacle for dishwash area handwash sink in rear kitchen for proper disposal of soiled paper towels

These areas in need of cleaning: front exterior sandwich prep unit including handle areas and top ledge of doors, refrigeration door gaskets, floors under chemical storage shelf rear kitchen and floor/wall juncture under 3-compartment sink. (Critical) Overstocked wrapped deli meats found at improper temperature on top of 3 door sandwich prep unit. Potentially hazardous food shall be maintained at 41°F or below or 135°F or above. Corrected at time of inspection by discarding overstocked wrapped deli meats found on top as high as 50°F - 60°F. Discarded all deli meats >41°F. All other food items on top of unit maintaining <41°F. It appears 2 layers of wrapped deli meats in a deep pan were capable of maintaining at proper temperature. Please continue to use your thermometer throughout the day to assure proper temperatures of all potentially hazardous food

(Critical) Observed expired apple harvest chicken salad in bottom section 2 door prep unit. Food items requiring date marking shall be consumed or discarded within 6 days from prep date. Corrected at time of inspection by discarding container of apple harvest chicken salad with a prep date of 9/11/14. Per pic this salad item does not sell well; consider eliminating from menu or make to order

Observed some single-service utensils near cash register stored improperly. Please store with all handles facing the employee to prevent potential contamination of the food-contact end

These areas in need of cleaning: interior bottom section 3 door prep unit, exterior microwave oven including handle area, underside of pop dispenser housing, pop levers and backsplash behind pop dispensers in need of cleaning. Please clean now and as often as necessary to maintain in clean condition

Provide proper air gap, currently an air break at ice machine drain line in rear kitchen. The air gap shall be twice the pipe diameter above the flood rim of the drain

Observed some single-service items stored improperly in rear kitchen. Please store coffee filters in original packaging or covered container once opened. Single-service bowls opened from wrong end; please open from bottom to prevent potential contamination of food-contact end

In the rear prep area observed several items not necessary to the daily operation of the establishment including: 1. Two boxes vinyl composition floor tile 2. Three one gallon paint cans 3. Various other items related to the original construction 4. Several pieces of equipment stored on top of the walk in cooler and walk in freezer. These items are preventing thorough cleaning of the floor and need to be removed from the facility. This violation was observed during a previous inspection. Please remove the items

Observed front panel of ice machine missing, thereby leaving the ice production area exposed to ambient air. During this inspection a piece of aluminum foil is being used to cover the ice production area. Correct by replacing the factory cover or reconfiguring the machine so a more permanent replacement cover can be installed. This is item was observed during the last several inspections. As discussed previously ice is a food and must be protected

A lemonade dispenser 4. Three used fluorescent lamps behind the rear exit door 5. Various other items related to the original construction these items are preventing thorough cleaning of the floor and need to be removed from the facility.

At the front sandwich make line prep top observed the reach in portion door with a broken hinge. Door is not operable because hinge pin is sheared in two pieces. During inspection door was observed in the closed position and sealed. Correct by repairing hinge

Critical corrected: at time of inspection interrupted food employee to wash hands after contacting money before placing on gloves. Facility had one employee present a time of inspection; had to make sandwiches and take money. Employee washed hands properly between all remaining customers

Critical corrected: at time of inspection relocated chemicals stored next to beverages on shelving unit rear kitchen, relocated chemicals stored above clean dishes on shelf above 3-compartment sink and relocated chemicals stored next to clean dishes on shelving unit rear kitchen. All chemicals relocated below and away from food, clean equipment, utensils, clean linens and single-service items to a designated chemical storage shelf

Spoon to dispense tuna salad and tongs for lemons stored improperly. Please provide long enough spoon for tuna salad so that handle does not come in contact with tuna salad and provide storage place for tongs so they do not end up in water surronding lemons to prevent potential contamination
