(Critical) Observed covered pans of cooked meatloaf, rte gravy, cooked pasta, and rte nacho cheese in wic with internal food temps of 70-80f. Foods were prepared or removed from hot breakfast buffet almost 2 hours ago per pic. All phf must be rapidly cooled from 135f to 70f within 2 hours and from 70f to 41f or below within a total of 6 hours. Pic stated correct cooling methods: shallow pans, ice baths, cooling in wif, but appears staff did not follow these procedures today. Pic corrected by uncovering all pans of food and rapidly cooling all above mentioned food items in wif at time of inspection. Violation corrected.

(Critical) Observed pan of rte shredded lettuce and pan of cut rte whole leaf lettuce in tabletop cold holding unit on cooking line with internal food temps of 57-60f. Lettuce has been in unit for 1 hour or less per pic. Cut leafy greens are considered potentially hazardous and must be kept at 41f or below at all times. If wanting to keep lettuce in this tabletop unit, will need to ensure all phf is kept at 41f. Pic corrected by adding ice to unit to supplement cooling at time of inspection. Rechecked lettuce after 30 minutes, now cold holding at 41f. Violation corrected. , observed bowls of rte watermelon and rte cantelope on salad buffet in dining room with internal food temps of 55-60f. Bowls are resting on top of shallow layer of ice and ice not cooling foods adequately. Cut melons are considered potentially hazardous and must be kept at 41f or below at all times. Recommend keeping sliced melons in cold holding unit to maintain 41f. If using ice to keep food at 41f, will need to make sure ice surrounds pan to the top so food can maintain proper temperature. Melons have been on buffet for 1. 5 hours, so pic corrected by double panning melons and surrounding containers with ice to top of pan. Sliced melons now cold holding at 41f. Violation corrected.

(Critical) Observed opened container of commercially processed rte potato salad in upright cooler in prep area with a "use by" date of 7/14/13. Datemark label indicated that potato salad was opened on 7/1/13. All potentially hazardous rte foods must by used or consumed by use by date on package. To correct, please rotate stock more often and discard all expired food as needed. Pic corrected by discarding expired potato salad at time of inspection. Violation corrected.

Facility has repaired holes in employee restroom walls with duct tape. Duct tape is not an approved for repairs in the kitchen as it is not cleanable and can accumulate bacteria. To correct, please remove duct tape and repair walls with approved materials as soon as possible

Observed damaged coving at floor to wall juncture in dining room (near side door/high chair storage area). As a result, food debris is collecting in coving. An accumulation of food debris near a door can attract insects and pests. To correct, please clean up food debris and replace coving as soon as possible.

(Critical) Observed piece of rte sugar free apple pie in display cooler in dessert station that did not appear edible (pie was hard, discolored and had other pie remnants littered on crust and pan). Pie does not require datemarking as per 2005┬░Food code, but no one in facility quite sure how long pie has been in cooler. All food must be safe, unadulterated and honestly presented at all times. To correct, please rotate stock more often and discard all desserts if food shows any signs of spoilage or contamination. Pic corrected by discarding pie at time of inspection. Critical corrected. Please note: other potentially hazardous rte pies in same display cooler were datemarked correctly, but staff are not discarding pies as per use by date on container and big boy datemarking requirements. All ph rte pies were within the 7 day datemarking policy (as per 2005┬░Food code, but big boy has a 3-4 day discard policy). No datemarking disposition violations were cited, but please follow big boy datemarking policies as required.

Observed bottom of hot water heater in utility room leaking from bottom of unit. Hot water was available at all fixtures in facility, but leaking water is creating a large amount of standing water on floor. To correct, please repair/replace hot water heater as soon as possible. Please note: if replacing hot water heater, will need a commercial unit and must replace with same (or more) capacity and btu's. Current water heater is 119 gallons. Also, please obtain proper permits as needed. , observed floor drain under dishmachine draining slowly and causing excess water to overflow from floor drain receptacle. Saw no visible signs of blockage in floor drain at time of inspection. To correct, please investigate source of blockage and repair as needed
