(Critical) Proper handwash cleaning procedure could not be followed due to lack of cold water at dishwash area handwash sink. Please provide hot and cold running water at handwash sink. The cold water control knob is missing. Not corrected. Water is very hot at dishwash area handwash sink making it difficult to wash hands without cold water to temper the hot water. ,

(Critical) Observed improper cooling method in place for breakfast bar food items. Hot potentially hazardous food shall cool within 2 hours from 135°F to 70°F and within a total of 6 hours from 135°F to 41°F using one or more of the following methods listed. Corrected at time of inspection by placing pan of scrambled eggs mixed with diced potatoes found at 54°F and pan of gravy found at 76°F into shallow pans uncovered; spatula left in gravy to stir throughout cooling process. Pan of scrambled eggs with potatoes and pan of gravy were covered with plastic wrap on shelf in walk-in-cooler. Facility did have ice surrounding gravy; there was approximately 5 inches of gravy in small pan. Per pic food was pulled off breakfast bar approximatley one hour prior to observing them in wic. Pic will discuss proper cooling methods with all staff. Food should be placed in shallow metal pans uncovered and stirred. You may also use your walk-in-freezer to facilitate the cooling process. In addition observed employee preparing broccoli salad with mayonnaise and bacon; found at 67°F at end of preparation. Placed in walk-in-freezer; employee will monitor with thermometer to assure salad cools within 4 hours to 41°F. Food items prepared with ambient air ingredients shall cool from 70°F to 41°F within 4 hours. Per pic all ingredients for cold potentially hazardous salads are pre-chilled

Corrected at time of inspection by providing paper towels at handwash sink in employee restroom and replacing battery in paper towel dispenser end of cook line. Observed some cracked food containers and lids on clean dish storage shelf. Please replace to provide smooth, easily cleanable surfaces

These areas in need of cleaning: floors under equipment front counter, interior ice scoop container, handle area some refrigeration units on cook line and floors on cook line where cracked tiles are present. Please clean now and as often as necessary to maintain in clean condition

Observed some fruit flies in various areas of kitchen, including near salad bar. Facility has been working with zero pest extermination company; recently called eco-lab for help in eliminating the flies. Eco-lab due on site next week.

Some clean dishes stacked wet on clean dish storage rack. Please air dry/drain clean dishes prior to stacking

Observed excess frost on the floor in the walk-in-freezer. Please repair any leaks and maintain in good repair

Several refrigeration door gaskets in poor repair. Please replace to provide good seal and so smooth and easily cleanable. The right side of the 3 door 6 drawer reach-in-cooler on the cook line holding slightly higher then the left side. The gaskets are split, cracked, some sections of gaskets missing and some not attached to doors and drawers mainly on the right side. Door and drawer gaskets to be replaced asap. No potentially hazardous food items stored on the right side of unit at time of this inpsection. Right side of unit maintaining food at 46°F; left side at 41°F. Per pic all food removed from this unit end of day. Thermometer is located on the colder side of unit; please place thermometers on both sides

Observed gap at bottom of rear entrance door. Please provide door sweep to eliminate any gaps

Please replace cracked floor tiles and re-grout as needed to provide smooth, easily cleanable surface and to eliminate standing water in these areas. (Critical) Observed food cooling improperly in walk-in-cooler. Cooked and cooled potentially hazardous food shall cool within 2 hours from 135°F to 70°F and within a total of 6 hours from 135°F to 41°F. Corrected at time of inspection by discarding gravy found at 86°F and eggs & potatoes found at 68°F that were pulled from the breakfast buffet at approximately 11:00 am per pic; found in wic at approximately 2:30 pm

Temporarily corrected at time of inspection by discarding gravy found at 86°F covered with plastic wrap in a deep food container and discarding eggs & potatoes found at 68°F covered in a deep food container. Discussed cooling method options as noted above and provided pic with a cooling chart to complete including method used. ,

(Critical) Observed some overstocked potentially hazardous food items at salad bar and cut melon on ice at salad bar at improper temperature. Corrected at time of inspection by discarding overstocked potentially hazardous food items including potato salad, macaroni salad, pea salad, etc found as high as 57°F on top and discarding cut melon on ice found at 55°F. In future facility will monitor the food levels and use thermometer to assure all potentially hazardous food on salad bar maintains at 41°F or below. When using ice bath for phf like cut melons the ice level must be above the food level for the melon to maintain at 41°F or below. Facility may shift food items on salad bar, placing non-phf in ice section. New stainless insert for salad bar did not fit properly; the new insert will accommodate deeper pans. Until new insert arrives for deeper pans continue to monitor your food levels to assure proper food temperatures of 41°F or below for all potentially hazardous food items

(Critical) Observed expired refrigerated, ready-to-eat, potentially hazardous food item in the walk-in-cooler. Please consume or discard within the 6 day time period noted on the food container (adding 6 days from prep date or open date). Corrected at time of inspection by discarding sliced ham found with a date sticker of 12/3 - 12/9. Please continue to monitor all food items requiring date marking and discard as needed

(Critical) Observed pans of raw sausage patties, raw chicken, raw cod, rte ham slices and cooked rte spaghetti and pasta in ric and drawer coolers under flat top grill (in middle of cooking line) with internal food temperatures of 45-55f. All other food in reach in coolers on same cooking line with same compressor as coolers mentioned above cold holding at 40f. Appears one reach in cooler door and one of the drawer coolers were left slightly ajar during lunch today as coolers do not have good seal when shut. All phf must be cold held at 41f or below at all times. Corrected by discarding above mentioned food items at time of inspection. Violation corrected.

Also observed pans of raw hamburger and sliced cheeses in drawer coolers on end of prep line (office side) with internal food temps of 46-48f. Other phf items in other reach in coolers on same cooking line cold holding at 41f or below. Appears that main reach in coolers are holding properly, but drawer coolers on end of lines not able to hold 41f or below. All coolers in middle of cooking line (under grill) utilize same compressor. Bottom line. . . 2 compressors in facility operate 29 different reach in, drawer or dessert coolers on the prep, cooking and server lines. All phf must be cold held at 41f or below at all times. Do not use the drawer and ric coolers on the end of the cooking lines until properly repaired and then inspected by the washtenaw county health dept. Also observed pans of housemade rte coleslaw, sliced tomatoes and sour cream in salad prep unit (top and bottom portions of cooler) in server area with internal food temps of 50-53f. Both sets of coolers run on the same compressor. All phf must be cold held at 41f or below at all times. Do not use either of these coolers to store phf until properly repaired and then inspected by the wash. County health dept. Soups were removed from hot holding on buffet last night around midnight, but were not properly cooled before placing in wic. All phf must be rapidly cooled from 135f to 70f within 2 hours and from 70f to 41f or below within a total of 6 hours. Pic stated correct cooling procedures (shallow pans, ice baths/ice wands), but staff did not follow proper procedures last night. Pic corrected by discarding all soups and will be educating and retraining night staff on proper cooling procedures as needed. Critical corrected.

Observed inaccurate metal stem thermometer in ric on cooking line which is used to monitor ambient air temp of cooler. To correct, please calibrate or replace thermometer as soon as possible

Observed leftover pans of soup from last night on cart in wic that were not cooled properly. Staff did not use proper ice baths or ice wands before placing hot pans of soup into wic for the night. Whatever cooling method you employ, staff must be monitoring foods as they pass through the temp danger zone. Please retrain all staff on proper cooling procedures as needed.

(Critical) Observed employees handling rte food with bare hands. There were no plastic gloves in facility when i first arrived for inspection. Pic stated that facility ran out of gloves this morning and was waiting for his morning delivery for more stock. Please note: most foodborne illnesses are transmitted by poor employee hygiene and bare hand contact with rte foods. Staff shall not handle rte foods with bare hands and shall utilize proper utensils and plastic gloves at all times. If wanting to continue using these cold tables for phf, staff will need to be monitoring temps of foods in this unit often and cover with lids. (Critical) Observed several containers of phf (sausage gravy, marinara, chili sauce) in steam table on cooking line with an internal food temp of 65-70f. Cook informed me that he did not reheat foods in commercial soup steamer before placing sauces in steam table 0. 5-1 hour ago. Steam table was on high, but was not heating food rapidly. All phf must be rapidly reheated to 165f within 2 hours before placing in steam tables. Steam tables are designed to hold temps and should not be used to cook foods. Pic turned heater back on, but dishmachine still never reached 160f or more after 30 minutes in operation. Ecoloab called at time of inspection. I will check this item at follow up inspection. Will need to a manually sanitize all dishware and utensils in 3 compt sink until dishmachine properly repaired and inspected by wash county health dept.
