Thermometer is missing from the rear upright cooler. Provide a thermometer for this unit inorder to properly monitor the temperatures for this unit.
The upright freezer in the rear is in need of defrosting as it has excess frost build up. Defrost now and place on a more frequent cleaning schedule.
Test strips for quaternary ammonium were not available at time of inspection. Only chlorine test strips could be found. The facility uses quaternary ammonium for sanitization also. Please provide test strips for quaternary ammonium so that the concentration of sanitizer in the 3c sink can be checked.
(Critical) Whipped butter packets found in container on storage shelf in rear storage area. Whipped butter is potentially hazardous and must be maintained at 41 degrees f or below. Corrected by discarding whipped butter at time of inspection.
Gasket on the door of the reach in cooler has food debris build up. Clean now and palce ona more frequent cleaning schedule.
The handsink in the rear prep area was blocked for access for routine handwashing by chair stored in front of handsink. Make sure handsink and supplies are readily accessible for routine handwashing.
The following non food contact surfaces need cleaning, cabinets and drawers where the coffee and utensils are stored, gasket s on the door fo the reach in cooler. Clean now and place on a more frequent cleaning schedule.
Thermometers for the upright 2 door cooler and the 2 door reach in cooler are missing. Provide thermometers for these units in order to monitor the temperatures of the units. Corrected at time of inspection by providing thermometers.
The cabinet under the trash disposal in the self serve area needs cleaning. Clean now and place on a more frequent cleaning schedule.
Gasket on the door of the reach in cooler is torn. Once torn gasket is no longer cleanable. Replace split gasket.
(Critical) Observed several packages of opened rte deli meats in wic with no datemarking labels. All phf rte foods must be labeled with a 7 day consume by datemark label (day 1 is open/slice date). (Critical) Observed carton of 1/2 & 1/2 left on corner of front counter area at 65f. All phf rte foods must be cold held at 41f or below at all times. Pic stated that in afternoon/evening, creamer is normally left in ric until a customer requests then is returned to ric. Appears staff forgot about creamer being left out. Corrected by discarding creamer. All staff must be aware of cold holding requirements and proper storage procedures. Please remind and retrain all employees as needed.
Observed 2°Fly strips on ceiling in front counter area soiled with dead flies. To correct, please remove all full insect trapping devices now and on a more frequent schedule
Observed working containers of honey and sugar in self-serve area of dining room with no labels. To correct, please label all food containers with correct common name for proper identification

Observed no paper towels at handsinks in front counter area and mens restroom. All handsinks must be stocked with soap and paper towels at all times
