(Critical) Used a test kit and found the sanitizing concentration of the bar 3 compartment sink at a very high concentration. Pic corrected by diluting sanitizing solution to manufacturer specification of 200 ppm. Note: facility was using a pan inside of the sanitizing compartment of the 3 compartment sink. Please ensure that all items sanitized are submerged under sanitizer.

(Critical) Found the critical level of the fill valve in the mens restroom toilet tank not at least 1 inch above the overflow pipe. To correct repair/adjust toilet tank fill valve critical level to be at least 1 inch above the overflow pipe.

Found no manager certification available. To correct provide information indicating some one from facility has signed up for a food safety class and provide a manager certification. Failure to provide a certified manager will result in further enforcement (formal hearing).

Found the kitchen floor and back kitchen floor heavily worn and uneven. Found the walls and ceiling in the entire kitchen with cracked and peeling paint. To correct properly repair floor, wall, and ceiling in the kitchen.

Found the tiled floor in the kitchen and storage room in poor condition. Found the wall above and behind the handsink in the kitchen damaged. Found the tiled coving in the mens public restroom loose. To correct repair/replace noted areas.

(Critical) The interior of the ice machine has lots of slime bacteria growth in it that has turned brown and black. Thoroughly disassemble this machine to properly clean and sanitize to remove this growth.

(Critical) Observed house hold use only and roach spray in the basement on a storage shelf. Remove this unapproved insecticide from the facility. Only purchase and use insecticides in the facility that are approved for use in a restaurant, or alternatively, hire a pest control contractor to deal with pest issues

(Critical) Observed a quart of 1/2 & 1/2 in the bar area beer bottle cooler (top loader by the cash register) at 54f. Corrected by discarding

(Critical) Observed small flies in both the kitchen and bar areas. Multiple steps will be needed to eliminate these pests: 1) maintain these areas clean and dry (remove spilled food from floor, clean the debris accumulations from cracks and the floor/wall junctures), 2) repair the floors to be water-tight (replace damaged flooring, seal the floor to wall junctures and any openings in the floor around piping, etc. With silicone sealant), 3) repair leaky plumbing, 4) re-insulate the coolant line coming from the basement compressor up to the bottled beer cooler located under the bar counter to eliminate the current moisture problem, 5) clean and re-seal the hand washing sink basin flange by the base of the sink faucet, 6) regularly use a drain debris digester to eliminate debris build-up in your drain lines (follow directions on the bottle and remember this product is not compatible with bleach), etc.

Both the microwave oven cavity (top, bottom and sides), and many surfaces of the pizza oven are over-due for cleaning to remove food debris

There is an excessive amount of old unused items stored primarily in the basement, but also in cabinets at the bar, storage areas, etc. Things such as multiple old tv's, old equipment, old promotional items, etc. Take some time in the near future to remove unneeded items so the facility can be organized to allow for easy cleaning and inspection of any pest activity

Ensure air thermometers are provided for all of your coolers--several units were observed without thermometers

Both the kitchen and the bar three compartment sink drain line manifolds are in need of repair to eliminate leaks. Repair/replace necessary parts/connections to make the drain lines water-tight

This facility is required to have a certified food safety manager. Provide documentation that a full time manager has successfully passed an accredited manager level food safety training course within 90 days. Failure to provide a certified food safety manager will result in an administrative office conference being required (the current 2010 admin. Conf. Fee cost is $121)

The doors on the two door sliding door kitchen cooler do not slide properly. Repair these doors as needed. The ceramic plate on the bottom of the microwave oven is cracked in two. Provide a replacement ceramic plate or entire microwave oven

1) many metal surfaces of the ice making machine are rusty. Provide a replacement ice making machine that has smooth surfaces and is easily cleanable. 2) the one door upright freezer in the basement is over-due for defrosting

1) the exhaust hood canopy and the exhaust hood removable filters are over-due for cleaning--oil is dripping from most surfaces. Contact your hood cleaning company asap to clean this system to prevent a hood fire. Please note that your hood canopy and the removable filter sections will require periodic in-house cleaning between the professional cleaning

The top loading beer bottle cooler by the cash register was not maintaining 41f or below. Adjust/repair as needed to maintain at 41f or below

Observed the following areas to be over-due for cleaning: 1) the floor in the kitchen under equipment/shelving units, 2) the drain basin located in the basement under the stairwell, 3) the floor/wall junctures at the bar area. Please thoroughly clean these areas now and on a very regular basis in the future to prevent attracting pests (flies, ants, mice, roaches, etc. )

Observed a significant gap located under the screen door on the west side of the building. Check all doors for proper door sweeps. Provide a replacement door sweep to keep out pests
