(Critical) Found ready to eat (rte), time/temperature control for safety (tcs) food, an opened carton of half and half, and an opened container of whipped cream cheese, without 7 day (day opened + 6 days) datemark in the 1 door shake cooler. Pic corrected by discarding whipped cream cheese, and provided a datemark for the half and half. Pic has been instructed to monitor and ensure employees are datemarking rte, tcs food.

Found handle missing on the top lid of the 2 drawer prep top cooler. To correct repair/replace missing handle.

Found the floor under equipment, edges and corners in the front counter and back kitchen area soiled with accumulation of debris. To correct thoroughly clean floor and keep clean.

Found the exhaust hood ducts above the ovens in the front counter area soiled with dusty build up. To correct clean exhaust hood ducts and clean on a more frequent schedule.

Found tile missing from the wall besides the handsink in the front counter area. Found the basement floor with large gaps and holes. To correct repair wall and floor to be smooth and cleanable.

(Critical) Found the drain line of the ice machine on installed on top of the pop machine draining into a receptacle without air gap. To correct provide an air gap of at least 1 inch from the drain line and the rim of the receptacle. ,

(Critical) Found time/temperture control for safety (tcs) food, several containers of lettuce 53°F in the top and bottom portion of the left 1 door prep top cooler. Found tcs food, feta cheese 50°F, beans 50°F, and artichokes 50°F in the 2 door prep top cooler. Pic temporarily corrected by discarding all tcs food above 41°F in noted coolers and has been instructed not to use coolers to hold tcs food, until repaired/replaced to hold tcs food at 41°F. Found tcs food, cream cups 73°F which require refrigeration in the self serve beverage countertop. Pic corrected by discarding cream cups and was instructed to either keep cream cups at 41°F or use " no refrigeration required" cream cups. ,

Found the lids of numerous storage containers soiled with build up. To correct clean lids.

Found the ducts in the cooking/exhaust hood soiled with dusty build up. To correct clean ducts.

Asked employee how slicer is cleaned, and employee replied, slicer is cleaned of debris, washed with soapy water, and rinsed with sanitizer. Corrected by educating employee that slicer should be precleaned, washed with soapy water, rinsed with clean water, rinsed/wiped with sanitizer and air dried. Pic has been instructed to monitor and ensure employees are properly cleaning slicer.

Found the automatic faucet in the main level restroom turning off in seconds and hard to activate. To correct repair/replace faucet to turn on and stay on for at least 15 seconds when activated.

Found the left 1 door prep top cooler and 2 door prep top cooler not holding tcs food at 41°F. To correct repair/replace coolers to hold tcs food at 41°F or lower.

(Critical) Observed no backflow prevention device on the recently installed blender rinser at the front service area. Correct by installing an asse 1024 or 1022 type device on the supply lines serving the washer.

At the shake making area, observed no air gap on the drain line serving the blender washer sink. It appears the drain line has slipped down into the receiving drain. Correct by reestablishing the air gap.

(Critical) Observed no paper towel in the dispenser at the handwashing sink adjacent to the dishwashing area. Pic corrected during the inspection by installing paper towel in the dispenser. Item corrected.

At the dishwashing area observed the grease trap to leak water on the floor when the three compartment sink is drained. Staff was observed using a squeegee to direct the water to a floor sink. The water was not observed to affect any food storage areas. Correct by repairing or replacing the grease trap so it does not leak on the floor.

In the sandwich make area at vent hood above each sandwich oven observed an accumulation of dust and grease. At each hood unit no filters are present. After a thorough professional cleaning, filters should be reinstalled to prevent grease from accumulating on the inside of the ventilation duct. Correct by cleaning now and more frequently to prevent the build up of dust and grease

Observed the floors at the following areas with an accumulation of food debris and grime: 1. Walk in freezer 2. At the main sandwich make line under the sandwich make and dress coolers 3. Under the toasting ovens observed the built in shelving adjacent to the door between the rear prep area and the sandwich make area with an accumulation of food debris. Correct each item above by cleaning now and on a more frequent basis to prevent the accumulation of food debris

At the rear dishwashing area observed several pans stacked for storage while still wet. Correct by completely air drying all properly washed and sanitized dishes prior to storage
