(Critical) Found phf in the reach-in unit in the kitchen and bar to have expired discard dates: cole slaw and milk. Corrected - these expired items were discarded at the time of the inspection. This is the third time in 4 routine inspections that this violation has been cited. An office conference will be required, see violation 3-501. 17 on this report for requirements

(Critical) Found the fryers to have an excessive accumulation of grease and food debris in and around them. To correct - clean now and keep on a more frequent cleaning schedule. This is the 2nd time in the last three routine inspections this has been cited. A risk control plan will be required to be completed by the time the follow-up inspection is performed in 10 days (on or about april 28th)

(Critical) Found several food phf items in the kitchen reach-in unit without discard dates: sour cream, house-made cocktail sauce, some sort of meat stock, sliced tomatoes, prepped iceberg lettuce, and cole slaw. Also found cooked cabbage and raw chopped cabbage in the wic without a discard date. Found open, undated milk at the bar. Corrected - all these phf items were discarded at the time of this inspection. Because this is the third time in the last 4 routine inspections, this violation has been cited, an office conference will be required. (Critical) Found the toilet in the women's restroom to be running. Upon inspection of the toilet tank, it was discovered that the ballcock assembly and flapper were stuck and there was no air gap from the refill tube and overflow standpipe. To correct - replace the ballcock assembly in this toilet tank making sure there is an air gap from the refill tube and standpipe. This must be corrected at the time of the follow-up inspection

Found many areas in the establishment that are in need of cleaning. The floors in the dining room are looking much better. To correct - the floors in the kitchen, behind bars, in storage areas really need to have a thorough cleaning now and on a much more frequent basis

Found the ceiling in the storage area behind the bar to be in disrepair. The ceiling tile(s) are missing, the insulation is exposed, wet, moldy, with an active roof leak. To correct - the roof leak must first be repaired and then the ceiling must be repaired

Found the reach-in unit at the bar (glass front) is holding @ 47*f. This unit is used for bottled beverages, lemons, limes, olives, pickles and cherries for the bar. To correct - repair/replace the unit to hold @ 41*f or less

Found many areas within the establishment that have items that are no longer used, broken, or used on rare occasions. These items inhibit cleaning around them, collect dust and debris, and take up valuable space. To correct - any items that are not used regularly, and that are in good working condition should be stored elsewhere to allow for thorough cleaning in these areas on a regular basis

Found interior of reach-in units in kitchen and bar to have an accumulation of food debris and spills. Also found the interior door of the short juice fridge at the bar to have mold, as well as moldy gaskets. To correct - clean these units thoroughly and keep on a more frequent cleaning schedule

(Critical) Found the grill and fryers to have an accumulation of food debris on the cooking surfaces and the oil in the fryers also has an accumulation of food debris in them. This is the second time in the last 2 routine inspections that this violation has been sited. A risk control plan will be required to be filled out and submitted for review and approval

(Critical) Found mashed potatoes, cooked ground beef, cooked mushrooms, chopped tomatoes without a use by date. Also found veggie soup and boiled eggs in walk-in cooler with a prepared date of 4/18 and a discard date of 4/26. The correct discard date is 4/24. Discard date is 7 days from and including prep date. Corrected - pic discarded all undated food items. All house prepared food items must be marked with a date that it was prepared and or discarded. This is the second time in 2 routine inspections that this violation has been sited. A risk control plan is required to be completed and submitted for review and approval

Found the inside of the fryer cabinet to have an accumulation of grease built-up and dripping inside the cabinet. (Critical) Found the walk-in cooler with am ambient air temp of 45 degrees f, and phf holding at 44 degrees f. The thermometer was reading 31 degrees f. Pic discarded phf items (only a few - mostly beer). This unit needs to be serviced immediately. Call repairman for service. Store all phf in the reach-in in the kitchen, and call me at 734 222-3959 when repairman has diagnosed the problem. I will stop out tomorrow to check on the unit. ,

(Critical) Repeat violation 2 times. Date marking is once again, not in compliance. I found an opened whipped cream and quart of milk, and a containter of hard boiled salad egg not marked with 7 day use by dates. Staff must be held accountable to be doing this. Complete the risk control plan within 10 days and fax in to me. Items not marked were tossed out.

(Critical) I found sanitizer in a wiping cloth bucket at the bar from yesterday at very strong levels (>200ppm). Keep at 50 - 100 ppm. Use test papers to verify. Corrected by making up new bucket to appropriate level.

Replace the missing light shields in the kitchen - one is the ceiling lights and the other is inside the 2 door glass door cooler in the kitchen.
