Observed broken light shield on ceiling in wic. To correct, please repair or replace broken light shield as soon as possible

Observed floor behind ice machine with an accumulation of food debris. To correct, please clean now and on a more frequent schedule

Observed exterior top of dishmachine in back kitchen area with an accumulation of scale and debris. To correct, please clean now and on a more frequent schedule

Observed a ladder and tool box laying on top of open packages of single service containers (pizza boxes, wing boxes) in outside shed. All single service items must be stored in a clean, dry area and not exposed to possible sources of contamination. To correct, please relocate pizza and wing boxes to approved location as soon as possible

Observed the following areas in need of cleaning: *top shelf of wing station in back kitchen area (accumulation of greasy debris). *outside of salad dressing containers in wic (accumulation of sticky food debris). *interior of pizza prep unit in middle of kitchen (under insert pans on top and interior of bottom coolers of same unit) have an accumulation of food debris. *tops of several utensil bins on shelf near ice machine (accumulation of sticky/greasy debris). To correct, please clean all these areas now and on a more frequent schedule

Health Report - 09/05/2012 (2 critical incident(s))

(Critical) Observed 2 pans of rte pasta (chicken alfredo) on pizza buffet in dining room with internal food temps of 105-110f. Pans of pasta are not touching water in steam table and water level was low. Also, observed rte pizza sauce in warmer in prep area with internal food temp of 110-118f (no water in warmer). All potentially hazardous rte foods must be held at 135f or more at all times. Staff must ensure water levels are maintained and food is held at proper temps. Pic temporarily corrected violation by discarding pans of pasta. Pizza sauce has been in unit for an hour (per pic) so pic temporarily corrected violation by reheating pizza sauce to 165f and returning to warmer (with added water) at time of inspection. Please train all staff on proper hot holding requirements as needed. I will check this item at follow up inspection. Correct within 10 days. Please note: this violation has been cited at 2 consecutive routine inspections. As a result, you are required to complete a risk control plan to help control this recurring violation. Please fax copy of risk control plan to carl walczesky at 734 222-3930 within 7 days. If same critical is cited at any of the next 3 routine inspections, you will be required to attend an administrative conference at the washtenaw county health dept. For enforcement actions. ,

(Critical) Observed 3 ice machine discharge lines laying in floor drain in back kitchen area creating a direct connection without proper air gaps. To correct, need to raise all 3 ice machine discharge lines 2 pipe lengths above flood rim of floor drain as soon as possible. I will check this item at follow up inspection. Correct within 10 days. ,

Observed interior door gasket on ric door on pizza prep unit in central kitchen area severely split and hanging. To correct, please replace broken door gasket as soon as possible

Observed no paper towels at handsink in front prep area (near pop machine). All handsinks must be supplied with soap and paper towels at all times for proper employee handwashing

Observed outside trash dumpster lid left open. All doors and lids to trash dumpsters must be closed and tight fitting to prevent entry of unwanted rodents, insects and birds

Observed no chlorine test strips in facility to measure sanitizer concentration in dishmachine. To correct, please obtain as soon as possible

Observed several pieces of wood (many with nails exposed) stacked behind trash dumpster on outside of building creating insect/rodent harborage conditions and is a safety hazard). To correct, need to remove wood as soon as possible

Observed 2 wet mops stored in the mop sink in back kitchen area. All mops must be hung up after use to allow for proper air drying.
