(Critical) Observed rte pizza sauce in warmer in cooking area with an internal food temperature of 78-82f. Sauce was removed from wic and has been in unit for 30 minutes per pic. As stated in past, all phf must be rapidly reheated to 165f within 2 hours and then hot held at 135f (140f per pizza hut sops) or more. Please remember that the sauce warmer is designed to hold temperatures not for rapid reheating. Shift manager (mary kay) stated correct reheating procedures, but they were not followed this morning. Pic temporarily corrected violation by sending sauce through pizza oven and reheating to 170f at time of inspection. I will check this item at follow up inspection to ensure compliance. Please note: since this violation has been cited at 2 consecutive inspections, you are required to complete a risk control plan to help prevent this violation from recurring. Please complete plan and fax to carl walczesky at 734 222-3930 within 7 days. (Critical) Observed loose bags of sliced ham, pork sausage, rte pizza sauce, and leftover whole pizzas on various shelves in wic with internal food temps of 46-50f. All other phf in same cooler that has not been removed from original cardboard boxes were cold holding at 39-41f. Wic is not functioning correctly (two thermometers in wic were reading 52f at time of inspection. Pic called for repair at time of inspection. Pic stated that they just had wic repaired last week. All phf must be cold held at 41f or below at all times. Managers must be verifying ambient air temp along with food temps in wic daily. Pic temporarily corrected violation by discarding ham, pork sausage, pizza sauce and leftover pizzas at time of inspection. Contractor arrived at time of inspection and repaired wic (broken condenser fan on roof). Wic now cold holding at 41f by end of inspection. I will check this item at follow up inspection to ensure compliance. ,

(Critical) Observed rte pizza sauce in hot well on pizza buffet line in dining room with an internal food temp of 115-120f. Also observed rte pizza sauce in hot well next to pizza oven with an internal food temp of 85-95f. Pic stated that buffet line sauce went through oven for reheating this morning (but pic did not verify food temp with thermometer), and sauce next to pizza oven was just placed into hot well unit (from wic) 45 minutes ago. All phf that is reheated for hot holding must be rapidly reheated to 165f within 2 hours before placing into hot holding units. To correct, need to rapidly reheat all sauce in oven or microwave oven to 165f and verify by checking food temps with accurate food thermometer. Pic corrected by spreading both pans of pizza sauce into shallower pans and sending through pizza oven and reheating to 165f before returning to hot holding units. Critical corrected

All can openers must be washed, rinsed and sanitized every 24 hours and must be clean to sight and touch. Pic corrected by sending can opener through dishmachine at time of inspection. Need to complete daily and fax copy of chart to carl walczesky at 734 222-3930 weekly for 1 month to verify wic is still maintaining proper temps as required. Health Report - 09/12/2011 (1 critical incident(s))

(Critical) Observed pans of pizza toppings (ham, sausage, cooked chicken) in pizza prep unit (top portion) with internal food temps of 45-49f. All other phf in same unit cold holding at 41f. No food was observed in bottom portion of make unit. Staff currently using ice to supplement cooler, but unit is not functioning correctly. Ice level had melted and couple of pans were filled with phf to top of pan and pans were not completely covered with ice (on sides of pan). All phf must be cold held at 41f or below at all times. If using ice to help keep foods out of the temperature danger zone, ice level must cover all sides of pan completely and ice must be refilled often. Using ice for cold holding phf is a temporary correction. Pic stated that store is getting a brand new make unit within a month and this unit will be discarded. Food has been un unit less than 2 hours per pic, so pic corrected by refilling ice and separating phf into smaller portions (backups pans returned to wic). Rechecked food temps, all phf now cold holding at 40f. Pic will educate and train all staff on proper cold holding procedures until new unit is installed. Need to monitor phf temperatures often and take necessary actions to keep all food at 41f or below at all times. Critical corrected.

Observed wic temperature at 54f when i arrived for inspection though all phf in this cooler was cold holding at 41f. Upon further investigation, pic stated wic was 46f when she arrived this morning, but all phf was still 41f or lower. Pic discovered that wic blew a circuit breaker and stated that breaker was reset and the temperature started to drop. Pic called for repair at time of inspection. Repairman arrived during inspection and replaced low pressure shutoff switch. Wic now cold holding at 41f by end of inspection. Please continue to monitor air and food temps in wic to ensure all phf is holding at proper temperatures

(Critical) Observed an open beverage cup (half full) on prep table in back kitchen area when i arrived for inspection. After staff realized i was conducting an inspection, an employee placed lid on open cup, but no straw. All employee beverage cups in kitchen must have a lid and straw so employees do not contaminate their hands on the open portion of cup (where you drink from) at any time. Corrected by educating staff on proper eating and drinking procedures in kitchen and foodborne illness risk factors. Critical corrected.

(Critical) Observed food employee eating in back kitchen area then remove pizza from oven and slice without washing hands in between. Employee did put plastic gloves on before handling food, but no handwashing as required. Improper handwashing and poor employee hygiene are the leading causes of foodborne illness transmission. All staff must wash their hands before handling or prepping food and any other time their hands may be contaminated (such as eating). Also, staff must wash their hands when switching between tasks in kitchen. Corrected by educating all staff on proper handwashing procedures (including how and when to wash). Critical corrected

All pots, pans and utensils must be thoroughly washed, rinsed and sanitized and must be clean to sight and touch before placing on clean dish rack. To correct, dishwashers must be scraping and pre-washing pans before placing in dishmachine. Staff must also be inspecting pans before they are placed on clean rack and rewash if needed. Also observed chemical spray bottle stored on edge of shelf above single service items. All chemicals must be stored below and away from all food and single service items at all times to prevent contamination.
